[Granular cell tumor of the male breast. A case report].

Granular cell tumor of the male breast is an extremely rare neoplasm. The case of a 64-year-old man with a periareolar lump in the upper medial quadrant of his left breast of recent onset is reported. At clinical examination, it was a poorly circumscribed, irregular, hard, mobile 1 cm mass. No axillary lymph nodes were palpable. The patient was submitted to ultrasound scan which showed a 1 cm periareolar solid hypoechoic lesion with an acoustic shadowing and subsequently to fine needle biopsy that revealed the presence of also atypical ductal hyperplasia. For this reason he was submitted to excision biopsy. The definitive histology was of granular cell tumor. The immunohistochemical investigations showed an intense positiveness for S-100 protein; the staining with PAS showed a diffuse positiveness. At 18 months after treatment the patient had no recurrence of disease. Granular cell tumor of the male breast is a neoplasm whose differential diagnosis with carcinoma is often possible only with definitive histology. For this reason, also in front of a mammary lump strongly suspicious for carcinoma, it's always necessary to think it can be this neoplasm and therefore to wait for the necessary histopathologic confirmation.

Breast Neoplasms, Male↗

[Isolated thyroid and skin metastases of renal carcinoma].

Metastases to the thyroid gland from renal carcinoma are observed in 10% of the uncommon metastatic thyroid tumours. They are the first sign of renal carcinoma in 25% of cases or appear after primary treatment of renal neoplasia, up to 27 years later. Metastases are more frequent in pathological glands, perhaps because intraparenchymal blood flow is slowed and tissue oxygen partial pressure is reduced. Thyroid metastases are frequently associated with lung ones, whether those associated with metastases to other organs (duodenum, pancreas, skin) are very unusual. Clinical manifestations are thyroid node without hormonal modifications or acute swelling of the gland with hormonal hyperfunction; other signs are dysphonia and/or dysphagia. Thyroid nodes are "cold" at 131I scanning and with mixed structure at ultrasonography. Fine needle aspiration is often diagnostic. Problems in differential diagnosis are among clear-cell renal carcinoma and some types of thyroid adenoma, adrenal carcinoma, parathyroid carcinoma, testicular seminoma and hypernephroid thyroid carcinoma. Treatment is surgical: subtotal or total lobectomy, with or without isthmectomy, if metastasis is single, total thyroidectomy in diffuse disease. Some Authors always choice total thyroidectomy to erase microscopic foci of disease in contralateral lobe. Reported survival after treatment is from a few months to 27 years. Survival is longer with a metastasis appearing after nephrectomy and with unique versus multiple ones.

[The role of node-picking lymphadenectomy in the treatment of differentiated carcinoma of the thyroid].

A retrospective analysis of the treatment of spreading of differentiated thyroid carcinoma to regional lymph nodes was performed in 201 patients observed in 1973-1991. In 183 patients with no clinical metastatic involvement (A group), no lymph node excision was performed. In 36 patients with limited metastatic lymph node involvement (B group), a node-picking lymphadenectomy was performed. After 1977, all patients were treated also with metabolic radiotherapy postoperatively. Actuarial 15-year survival was 98.28% in A group and 87.50% in B group (n.s. difference). In 15 patients with lymph nodal relapse, actuarial 10-year survival was 83.4%. The survival figures are not different from those obtained after more radical operations. So, in limited metastatic involvement of neck lymph nodes by differentiated thyroid carcinoma, in the opinion of the Authors, node-picking lymphadenectomy is equally effective as more radical surgical procedures.

[Hyperparathyroidism resulting from chronic renal insufficiency. Diagnosis and therapy].

Twenty-seven patients, eighteen females and nine males, with chronic renal failure and secondary hyperparathyroidism, were treated by subtotal parathyroidectomy. Bone pain, in 24 patients, hypercalcemia in 2 and severe pruritus in 1 were the main indications to surgery. Result evaluation was possible in twenty four patients. Bone pain disappeared or was reduced in 20/22 patients. Serum alkaline phosphatase and PTH returned to normal in 21/24 patients. There patients had persistent hyperparathyroidism because of inadequate surgical exploration. Another group of seven patients with secondary hyperparathyroidism recalcitant to medical therapy or relapsing after subtotal parathyroidectomy was treated with calcitriol ev. After nine months of follow-up PTH and alkaline phosphatase serum levels were reduced to normal value in all patients.

[Carcinoma of the surgically treated stomach. Our experience].

Between 1977 and 1991, 12 patients (11 males and 1 female; median age of 62.7 years) with gastric stump cancer were observed. All patients were previously operated for benign gastric (4) or duodenal (8) ulcer disease. Median duration interval between first operation and the diagnosis of gastric carcinoma was 22.0 years (range 7-39 years). Only in 5 patients total gastrectomy was carried out. Two out of these 5 patients are still alive at 2 and 24 months after surgery, while the median survival of remaining patients was about 9 months. The usefulness of close follow-up and digestive endoscopy in peptic ulcer patients who had surgical treatment is considered in order to detect an early gastric stump cancer.

[Anaplastic carcinoma of the thyroid. Our experience].

Nineteen patients, 12 females and 7 males, with mean age of 66 years, with anaplastic carcinoma of the thyroid, were treated between 1976 and 1990. At diagnosis, in 4 patients disease presented as intraglandular mass, in 11 as infiltration of the adjacent structures and as distant metastases in 4 cases. A preceding history of goiter was found in 7 patients. Total thyroidectomy was performed in 9 patients, subtotal thyroidectomy in 1 and a diagnostic biopsy only in 4 cases. All patients received external radiotherapy (4000-6000 rads). Median global survival was 6 months with no difference between patients receiving thyroidectomy plus RT or biopsy plus RT. All patients died of tumor except 1 who is alive and free of disease at 120 months. Combination modality treatment of anaplastic carcinoma of the thyroid represent, at times, a rational palliative therapeutic approach, even if, in selected patients with early intraglandular disease, total surgery may represent a curative therapy.

[Carcinoma of the male breast. Review of the literature and case series contribution].

Following and exhaustive review of the literature on this topic, the paper reports the results of the treatment of 16 patients with male breast cancer. From an analysis of findings, it was observed that diagnosis is frequently delayed (mean 1 year), disease is at a locally advanced stage in the majority of patients, and there is a considerable variety of treatments in relation to the long recruitment period for the study. The difficulties of performing radical surgery which does not necessitate complex reconstructive surgery has led to the use of Limberg's flap in the last 3 cases treated. Mean survival rate was 8 years for stage 1, 4 years for stage 2, 3 years for stage 3 and 2.5 years for stage 4.

[Acute abdomen in patients with hematologic neoplasms].

Sixteen onco-haematological patients with acute abdomen are presented. The diagnosis of acute abdominal conditions was: neutropenic enterocolitis, gastrointestinal perforation, bowel neoplastic obstruction hepatic abscess and strangulated tumoral hernia. The operative death was 31.2%; 62.5% died 2-24 months after surgery and 6.3% is alive at 72 months. The Authors conclude that in these patients the extended surgery should be performed only in prognostic all favourable cases.

[Bilateral carcinoma of the breast].

Authors, in the light of literature and personal experience of 27 patients observed, discuss risk factors, treatment and prognosis of bilateral breast cancer. Finally, Authors stress the utility of carefully planned follow-up with mammography and random biopsy to obtain an early diagnosis.

Breast Neoplasms↗

[Splenectomy in idiopathic thrombocytopenic purpura].

Personal experience of 21 patients treated with splenectomy for idiopathic thrombocytopenic purpura (ITP) is reported. After a brief review of the pathogenesis and prognostic factors of the disease, the paper stresses the therapeutic role of surgery and the careful exploration of the abdomen required to prevent any recurrence of the disease.

[Prognosis of breast cancer (T1-4 No Mo) with reference to anatomo-pathological and clinical factors].

Using methods of multivariate statistical analysis, the Authors analyse the prognosis of 135 patients affected by No Mo breast cancer treated with radical mastectomy alone. Among eight risk factors examined, "vascular invasion" and "peritumoral infiltration" seemed to have the greatest influence on prognosis. Therefore, more aggressive treatment may be considered for the patients affected by breast cancer with the two factors former quoted.

Abnormalities in evening plasma prolactin levels in nulliparous women with benign or malignant breast disease.

In the present study the assay results of prolactin concentrations in serial samples of blood obtained from premenopausal women with benign or malignant disease of the breast are compared with--and discussed in relation to--findings reported in earlier studies based on single samples of blood taken at various times from a large and ostensibly normal population of women. The finding of an abnormality in nycthemeral prolactin levels in the established disease is considered to strengthen the concept that the same abnormality found in high-risk groups is of aetiological importance.
