Class I malocclusion with severe open bite skeletal pattern treatment.

A case report of a Class I malocclusion with a severe skeletal open bite, excessive overjet, a high mandibular plane angle, and a forward maxillary rotation is presented. Treatment has eliminated the causative factors (i.e., mouth breathing, enamel hypoplasia of the first molars, and abnormal tongue posture and function). A normal growth pattern has been restored, ensuring a good and stable orthodontic result.

Cephalometry

[Biological aspects and perspectives applicable to the chemoprevention of cancer of the upper respiratory-digestive tract].

After defining chemoprevention a description is given of the phases of differentiation in normal epithelial cells and the features of proliferation in neoplasias developing in this epithelium. The latest studies on carcinogenesis indicate various types of prevention with which one can alter this transformation process. Epidemiological studies have shown that subjects with low serum levels of Vit-A or Beta carotenoids are at high risk of developing epithelial cancer. Three main categories of agents inducing cell transformation are described: a) physiological induction agents; b) non physiological induction agents; c) cytotoxic drugs. In regard to clinical use, some studies have focussed on the importance of Vit-A in chemoprevention of risk conditions (pre-cancerous lesions) and in prevention of cancer recurrence. The authors point out the increasing interest in the use of retinoids in the chemoprevention of head and neck cancer and report some personal clinical experience.

Neoadjuvant or definitive alternating chemotherapy and radiotherapy for infiltrating bladder cancer.

Thirty-two patients with infiltrating bladder cancer were treated with transurethral resection followed by one course of alternating chemoradiotherapy before radical cystectomy (group A, 20 patients) or two courses as definitive procedure (group B, 12 patients). One course consisted of: cisplatin 20 mg/m2 i.v. and 5-fluorouracil 200 mg/m2 i.v. for 5 consecutive days, the first and the fourth weeks; radiotherapy 20 Gy in 10 fractions in the second and third weeks. At the seventh week the same integrated therapy was restarted in group B. All 32 patients were evaluable for toxicity after the first course: no grade IV toxicity was observed. Significant increase in hematological toxicity was observed in 12 patients who received the second course of chemoradiotherapy: two patients had grade IV toxicity, and five patients had grade III. Fifteen patients of group A underwent radical cystectomy: 40% had a pathological (p) complete response (CR) and 13.3% a partial response Five patients in group A did not receive either the second course of therapy or cystectomy because of age (three patients), vascular obliteration (one patient) and enteritis (one patient). Actuarial disease-free survival in group A is 78% at 21 months. All patients of group B obtained clinical (c) CR and all but one have no evidence of disease at a median follow-up of 10 months (range 6-13). The high pCR and cCR obtained in patients of group A and group B, respectively, appears promising. A longer follow-up and a larger number of patients is required to determine the role of this integrated treatment.

Radioimmunoimaging in malignant melanoma with monoclonal antibodies.

Twenty-five patients with cutaneous melanoma were imaged with F(ab')2 fragments of antimelanoma monoclonal antibody (MoAb) 225.28S labeled with 99mTc and 131I or 111In. Out of 16 patients without evidence of metastatic lesions, 6 false-positive cases were observed. Only 4 out of 9 patients with known lesions showed positive findings (globally 8 of 19 metastatic sites). In conclusion, antimelanoma MoAb was of little help in radioimaging and stating this disease.

[Problems of professional responsibility in orthodontic practice in the public sector].

Characteristics aspects of orthodontics profession are emphasized. Problems about patient consent for treatment, his information and operator obligation to attend to treatment goals on particularly considered. Orthodontist behaviour errors causing a damage to the patient during diagnosis, treatment planning, orthodontic appliance planning and/or carrying out and therapy execution are analysed. Biologic factors having a significant influence on treatment effects are illustrated. Damages possibility even after long time from therapy are evaluated.

[Injury to the TMJ in dental practice: criteria for review and evaluation in the public sector].

The Authors point out dental fields when a treatment error may cause considerable civil TMJ damages. Various damage kinds were examined and diagnosis standards to point them out were illustrated. Evaluation hypothesis using a table system when a sagittal mandibular excursion permanent deficit is present was suggested. The propose analysed the main percentages be bunded in Literature. The distance between upper and lower first bicuspid in maximum non forced mouth opening was considered as a necessary landmark to estimate the mandibular function deficit.

[Force systems for correction of lower molar inclination].

The authors analyzed the uprighting movement and suggested forces system developing wished movements considering M/F ratio generated on brackets. The effects of these system on anchorage segment were evaluated. Use of concurrent or non-concurrent was analyzed in the light of their "cohernce" with treatment goals.

[Cephalometric evaluation of lower incisor position].

The Authors examined some cephalometric values usually utilized to evaluate lower incisors inclination or anteroposterior position. Mean values and standard deviations of these parameters in a 194 subjects sample was calculated. Validity of these valued in different typology and skeletric classes was finally verified.

A phase II study with danazol in metastatic breast cancer.

Danazol is a synthetic steroid that inhibits the gonadotropin secretion. Its efficacy was tested in 27 patients with metastatic breast cancer at the dose of 200 mg three times daily. Characteristics of patients were as follows. The median age was 57 years (range, 44-85). All patients were postmenopausal, and all had previously received a median of two (range, 1-3) endocrine therapies. Estrogen receptor status was known in 12 (positive in five of 12; negative in seven of 12). Dominant sites of metastases were lung in seven, bone in ten, liver in three, and soft tissue in seven. Six of 27 patients were unevaluable for response (early death, four; lost to follow-up, two). Three of 21 patients showed an objective response, eight of 21 obtained stabilization of disease, and eight progressed. The therapy was well tolerated in the majority: gastric pain was observed in three and nausea in two. Danazol could have a role in the treatment of metastatic breast cancer as an alternative regimen when other treatment has failed.

[Rheumatoid arthritis. Considerations on a rational therapeutic strategy].

Rheumatoid Arthritis (RA) is a progressive disease of unknown aetiology which may be caused by faulty immune mechanisms. Early diagnosis and correct treatment can be extremely effective. Fast and lasting results can only be obtained by an appropriate combination of NSAID and DMARD drugs which both reduce subjective symptoms and halt the progression of the disease.
