[Optic disc drusen--case report].

A case of giant optic disc drusen is presented. Diagnostic difficulties and a role of fluorescein angiography, CT and static perimetry in diagnosing these alterations are described.

[Tuberous sclerosis--case report].

A case of 37-year-old man with tuberous sclerosis is presented. Classic triad (Vogt) which consists of mental handicap, epilepsy and adenoma sebaceum around a nose and cheeks was observed. Ophthalmoscopy showed bilateral retinal astrocytomas with mulberrylike appearance. During 6 years follow-up small progression of ocular changes (findings) was noticed. CT and NMR of the head, ultrasound of the eye fluorescein and ICG angiography are presented.

[Personal observations with the use of soft contact lenses after radial keratotomy].

PURPOSE: The evaluation of conjunctiva and cornea of patients wearing soft contact lenses after RK. MATERIAL AND METHODS: Between the years 1991-1995 RK was performed in 88 patients. Nine of them (10.2%) used soft contact lenses for correction of residual refractive error. All these people were carefully examined. RESULTS: The only complication was superficial neovascularisation concerning mainly peripheral parts of the corneal incisions. The long incisions reaching limbus and early application of contact lenses after RK promote peripheral neovascularisation of the cornea. CONCLUSIONS: Soft contact lenses should not be recommended after RK if corneal incisions reach the limbus. However, in other cases they can be used with careful ophthalmological supervision.

[Artificial intraocular lenses in patients with cataracts and glaucoma].

The authors evaluated 52 glaucomatous eyes in which, after the extracapsular cataract extraction, artificial posterior lens has been implanted. In 32 patients glaucoma has been treated for 2-6 years. In 7 patients of this group, it was not possible to control the intraocular pressure before surgery and in these cases simultaneous trabeculectomy, extracapsular cataract extraction and the implantation of artificial posterior lens were performed. In 13 patients, anti-glaucomatous surgery had been performed before, and in 7 cases, glaucoma was diagnosed in the preoperative period. We considered our results as satisfactory since in 67% visual acuity was over 0.5; 29% of the patients did not require anti-glaucomatous treatment, whereas 71% took anti-glaucomatous drugs in reduced doses. In our opinion, glaucoma is not a contraindication for artificial lens implantation, although surgery can be technically more difficult.

[Use of transconjunctival cryoapplication in chronic pars planitis with involvement of peripheral retina and choroid].

The transconjunctival cryoapplication was performed in 8 patients (13 eyes) with the recurrent pars planitis and the simultaneous involvement of peripheral retina and choroid. Before the surgery, the peripheral fundus of the eye was examined with the Goldmann three-mirror lens and afterwards, the cryoapplication of lesions was performed. The topical treatment, dexamethasone and indomethacin drops, followed the surgery only. In all cases the inflammation disappeared but the macular edema and vitreal exudation regressed slowly. The considerable improvement of visual acuity was achieved in 8 eyes (61%). So far, the results of the observations are satisfactory and they entitle the authors to carry on the application of the treatment mentioned above.

Studies of aqueous humor proteins in rabbits after posterior chamber lens implantation.

In 13 rabbits, 1 month to 1 year after posterior chamber lens implantation/polymethylmetacrylate/, the level of aqueous humor proteins was evaluated and the proteins separation in polyacylamide gel was performed. The studies were also carried out in unoperated eyes of the same animals and control group was composed of the eyes before surgery. It was found that in pseudophakic eyes an increased level of proteins remained during the whole year/the highest one month after surgery, slowly decreasing afterwards/, with the appearance of additional fractions. The moderate increase of the proteins concentration was also observed in unoperated eyes. The increase of aqueous humor proteins in pseudophakic eyes indicates that the presence of polymethyl metacrylate is not completely indifferent to the eyeball in spite of the suggestions derived from the clinical observations.

[Cystoid macular edema in pseudophakia].

One hundred and eighty six patients (217 eyes) after cataract extraction with intraocular lens implantation were analyzed in search of cystoid macular oedema. In general--9 cases of cystoid macular oedema were found, among them 5 patients after intracapsular cataract extraction with implantation of an anterior chamber lens (9.8 p.c.) and 4 after extracapsular cataract extraction with implantation of a posterior chamber lens (2.7 p.c.). One could observe a beneficial influence of a prophylactic application of indomethacin compounds in order to lessen the incidence of the macular oedema.
