A randomised study comparing granulocyte-colony stimulating factor (G-CSF) with G-CSF plus thymostimulin in the treatment of haematological toxicity in patients with advanced breast cancer after high dose mitoxantrone therapy.

54 patients with advanced breast cancer were randomised into a prospective, non-blinded, controlled trial to receive: mitoxantrone 28 mg/m2 intravenous day 1 and granulocyte-colony stimulating factor (G-CSF) 5 micrograms/kg/day subcutaneously days 2 to 16 (n = 27) or the same regimen plus thymostimulin (TS) 50 mg/day intramuscular at days 2 to 16 (n = 27). The median time to reach a neutrophil count greater than 0.5 x 10(9)/l was lower in the G-CSF+TS treated group (9.13 versus 3.24 days; P < 0.0005). More patients experienced neutropenic fever in the G-CSF group than in the G-CSF+TS group (59.3% versus 22.2%, P = 0.0119). The incidence, duration and severity of clinically or bacteriologically documented infection were lower in patients who received TS. 16 patients (59.3%) in the G-CSF group contracted infection, and 4 patients (14.8%) receiving G-CSF+TS (P = 0.0016). These data indicate that the combination of G-CSF and TS is well-tolerated and may enhance haematological recovery following myelosuppressive chemotherapy in patients with advanced breast cancer.

Tegafur-uracil (UFT) plus folinic acid in advanced rectal cancer.

We previously reported positive results to Tegafur-Uracil (UFT) chemotherapy in a group of patients with advanced rectal cancer. We have continued the study and now report the effectiveness of UFT plus folinic acid (FA) in 52 patients with advanced rectal cancer. The therapeutic schedule was UFT, 600 mg/m2/day x 14 days p.o. + FA, 90 mg/m2/day x 14 days p.o. Fifty-two out of a total of 56 patients were evaluated for response and toxicity. A higher incidence of positive responses in patients without previous chemotherapy was appreciated. Twenty-one of the 52 evaluated patients showed a partial response (PR). Responses were strongly correlated with previous chemotherapy (14/20; 70% PR of cases without previous chemotherapy vs 7/32; 22% of cases with previous chemotherapy). All responding patients came forward with a median time to progression of 8.2 months (19.6 months for patients without previous chemotherapy vs 7.7 months for patients with previous chemotherapy, P < 0.01). We concluded that the UFT plus FA could be a treatment of choice for patients with advanced rectal cancer.

A randomized trial of mitomycin/ifosfamide/cisplatin preoperative chemotherapy plus surgery versus surgery alone in stage IIIA non-small cell lung cancer.

The efficacy of surgery or radiotherapy as conventional treatment for stage IIIA non-small cell lung cancer (NSCLC) is limited. Recent studies have pointed out that preoperative chemotherapy may improve survival. To reconcile the two approaches, we undertook a multidisciplinary randomized trial to examine the possible synergism between preoperative chemotherapy and surgery in improved survival. Stage IIIA NSCLC patients were randomly assigned to receive either three preoperative courses of mitomycin/ifosfamide/cisplatin chemotherapy and surgery or surgery alone. The median survival was significantly greater in the chemotherapy plus surgery group than in the surgery group (26 months v 8 months; P < .001). However, the prognostic value of the mutated K-ras gene data presented awaits the analysis of larger sample populations. Similarly, the role of high-dose cisplatin in inducing higher pathologic complete remissions has to be corroborated in future randomized trials.

High-dose ifosfamide and mesna in advanced breast cancer. A phase II study.

Thirty-two patients with metastatic breast cancer had previously been treated with chemotherapy (including anthracyclines). They were included in a trial to receive 6 g/m3 ifosfamide, mixed with 6 g/m2 mesna in 1000 ml saline infusion, infused over 4 h. Therapy was repeated every 21 days; the dose was reduced by 50%. Twenty-eight patients could be evaluated. An average of 4.2 cycles (range 2-8) was applied. One patient (4%) showed complete remission. Ten patients (36%) had a partial response. Ten patients (36%) experienced SD and the remaining patients (25%) PD. We conclude that high-dose ifosfamide shows activity in this group of pretreated patients and merits further investigation.

Advanced and unsuspected secondary abdominal pregnancy misdiagnosed before laparotomy.

A case of advanced secondary abdominal pregnancy is presented. It happened in a multiparous patient, a heroin addict and with a previous secondary amenorrhea. In spite of having been controlled through the whole pregnancy, the ectopic pregnancy was not diagnosed until a laparotomy was performed, several days after fetal death. After full extraction of the fetus and placenta, followed by left adnexectomy, the postoperatory evolution was normal. Hormonal and echographic studies and fetal monitorings are provided, and the diagnosis and treatment are discussed.

Papillary-cystic neoplasm of the pancreas. Report of two cases and review of the literature.

Two cases of papillary-cystic neoplasm of the pancreas are reported in women aged 22 and 23 years. The patient in the first case presented with acute abdominal pain and hemoperitoneum. This form of presentation has not been previously reported. This type of pancreatic tumor is very rare, exclusively affecting young women, and has a good prognosis despite its various histologic features, which suggest a malignant appearance. The authors consider this neoplasm as having an acinar origin because in the cells of one of the patients we observed ultrastructurally the presence of abundant rough endoplasmatic reticulum with formation of annulate lamellae and a few prezymogen granules.

VP-16 plus cyclophosphamide in the treatment of advanced lung cancer.

Ninety previously untreated patients with histologically documented lung cancer were treated with VP-16 and cyclophosphamide either alone (protocol I) or with methotrexate (protocol II) or Adriamycin (protocol III), with 30 patients in each protocol. The rates of objective response were 57,37, and 27%, respectively, protocol I being significantly better than protocol III (P less than 0.05). Protocol I was significantly less toxic than protocols II(P less than 0.01) and III (P less than 0.001). The overall rate of objective responses was 66% in small cell (SCC) and 22% in non-small cell carcinoma (nSCC). Median survival was 37 weeks in SCC and 21 weeks in nSCC. Median survival of responders both in SCC and in nSCC was significantly longer than in nonresponders. We conclude that VP-16 plus cyclophosphamide is a well tolerated regimen with positive effect in advanced lung cancer. The association of methotrexate or Adriamycin didn't offer any improvement over the basic combination in this study.

Mitomycin C as an adjuvant in resected gastric cancer.

As a result of their previous experience with mitomycin C at high discontinuous doses in advanced gastric cancer, the authors studied its role as an adjuvant for locally advanced cases after surgical complete resection. Results from 70 evaluable patients are presented. Patients were allocated randomly to receive mitomycin C, 20 mg/m2 I.V. direct once every 6 weeks, four courses, or a placebo. After a follow-up period of 250 weeks, seven patients of treatment arm and 23 controls have already relapsed (p less than 0.001). Toxicity was moderate and controllable by symptomatic measures. The authors consider this investigation a positive contribution in the field of adjuvant therapy of gastric cancer.

Patient response to hydrogel contact lenses: a comparative study.

Eleven subject (22 eyes) wore the best fitting lenses of each of four different types of hydrogel contact lenses (Bausch and Lomb Soflens contact lenses, Hydrocurve HCII contact lenses, AOSOFT-TM/Aquaflex hydrophilic contact lenses, and Alcon Tresoft contact lenses) in a random order for 2-3 week periods so that their short-term responses to the lenses could be evaluated and compared. None of the subjects had any contraindications to hydrogel lens wear. All the lenses centered well with proper movement. Using a double masked procedures, we evaluated each patient's response to each lens type according to the standard of success of Sarver, Harris and Polse. Approximately 63% to 72% of the patients were successful with each lens type. We found no statistically significant differences in the patients' responses to the different lens types, even though individual differences were found. The majority of our subjects were successful with several or all of the lens types evaluated.

VP 16-213 (VP 16) and cyclophosphamide in the treatment of primitive lung cancer in phase M 1.

Sixty patients suffering from primitive lung cancer in Phase M 1 were included in two successive chemotherapy Protocols, 30 patients to a group. The first of these (an attempt at cellular synchronization by combining VCR-Cyclophosphamide-Prednisone) is used as a historical group as compared to the second. In the second protocol the combination of VP 16.213 (100 mg/m2, po, day X 5 days) and cyclophosphamide (100 mg/m2, ev, day X 5 days) was attempted. The positive results obtained (more than 60% of the responses were above 50%) together with low toxicity was not accompanied by improvement of survival with respect to that observed in international medical literature. On the other hand, the number of cases studied--30 patients--was too low to permit a better evaluation of our results. Despite this fact, it seems appropriate to continue clinical investigation of the VP 16.213-Cyclophosphamide combination in lung cancer.
