[Abdominal pain in the differential diagnosis of acute abdomen-- common imitators of acute abdomen].

The authors analysed abdominal pains in diagnosis of acute abdomen, and the importance of that problem for the beginning of therapy in time. Here were elaborated the anamnestic, symptomatologic, diagnostic and differential diagnostic details, which make sure the recognition of acute abdomen and an early diagnosis of this state. The numerous data, rightly interpreted, enable an early diagnosis of acute abdomen, also by the general practitioner who is the first to meet those patients. We reported especially cases who imitate acute abdomen. These diseases are very important in differential diagnosis of acute abdomen. Both doctors, the general practitioner and the clinical one, have a big responsibility for an early recognition of the disease, in order to be able to do the in time and prognostically good surgical therapy.

[Complications of immunosuppressive therapy after kidney transplantation].

The complications of immunosuppresive therapy that occurred in a series of renal transplant patients presented were many. Of the total 28 renal transplants performed in Sarajevo, 25 were observed. These complications were diagnosed in 19 cases (6 of which exited), Death in each case was due to virual and not bacterial infection which occurred most often in the lungs. No urinary infections occured in any of these cases. The actual cause of death in most of the patients was most frequently due to pneumocystis and alveo-capillary block of the lungs which is very difficult to diagnose. When this complication presented itself the method of choice was rapid manipulation of the immunosuppressive therapy together with the administration of concentrated granulocytes which did not significantly impair the function of the grafted kidney in any of the cases save those with cortizone diabetes. High body temperature was also of diagnostic importance. The authors feel that quick action and accumulated clinical experience prevent the fatal outcome of many renal transplant patients with complications from immunosuppressive therapy.

[Effect of vitamin E on erythrocyte enzymes and total antioxidant status in diabetic patients with ischemic heart disease].

Lipid peroxidation is thought to be one of the major factors involved in atherogenesis. There is an increasing evidence is increasing that oxidation of LDL cholesterol may be instrumental in atherogenesis. Diabetics are known to be at increased risk of cardiovascular diseases, a phenomenon which has previously been linked to the lipid peroxidation process. As a result, a number of studies have been undertaken to evaluate the effects of antioxidant vitamins on coronary heart disease and risks factors of ischaemic heart disease such as diabetes mellitus. Lipid peroxidation and antioxidant status were studied in 51 patients with ischaemic heart disease and some of with having diabetes mellitus (18%). Results were compared before and after supplementation of 450 mg of tocopherol acetate for three months. SOD were found to be elevated in patients with diabetes and in whole groups of patients after supplementation of tocopherol acetate. Also, TAS was found to be elevated in a subgroup of patients without diabetes and no significant changes were found in glutathion-peroxidase after supplementation. We found statistically significantly decreased mean values of glucose after supplementation in all groups of patients. The monitoring of antioxidant parameters in diabetic patients could be of vital importance in the study of the disease.

[Treatment of impotence with implantation of penile prosthesis].

The authors have presented results and importance of the treatment of impotence by implantation of different types of penile prostheses in 46 patients. The aim of the study was to evaluate success in recovery of potency with modern types of prostheses. Problems related to characteristics of prostheses and risk groups of patients for this kind of treatment have been recognized. It has been concluded that penile prostheses are very good means for solving the problem of impotence which is exclusively or mainly of the organic origin when other methods are not suitable for a patient or have not given favourable results.
