Soluble immune mediators in POEMS syndrome with pulmonary hypertension: case report and review of the literature.

POEMS syndrome is a plasma cell dyscrasia that presents with numerous complications, one of which is rarely pulmonary hypertension. Here we present a case of POEMS syndrome with pulmonary hypertension who improved with steroids and six rounds of plasmapheresis done over 1 month, and we document the baseline immune mediator status and the changes associated with the therapeutic intervention. Serum levels of soluble immune mediators such as interleukin (IL)-5, IL-8, IL-10, and eotaxin were normal at baseline and throughout therapy, whereas those of tumor necrosis factor (TNF)-alpha, soluble TNF-receptor type I (sTNF-RI), IL-6, interferon (IFN)-gamma, IL-2, and sIL-2R, which were abnormally high at baseline normalized with steroids and plasmapheresis. Serum levels of sIL-6R, which were abnormally low at baseline, increased to normal after therapy. The latter results pinpoint not only potential mediators of the systemic manifestations of POEMS syndrome with pulmonary hypertension but also relevant markers in patient follow-up. In this respect, IL-6 has been involved in the pathogenesis of multiple myeloma and Castleman's disease, and the interplay between abnormally high levels of IL-6 and abnormally low levels of its soluble receptor, deficiencies that corrected with therapy in this patient, appears to be particularly relevant to the pathogenic manifestations of POEMS syndrome with pulmonary hypertension. These findings are discussed in the context of our current knowledge of the pathogenesis of pulmonary hypertension and of potential new therapeutic modalities for POEMS syndrome with pulmonary hypertension.

Cardiac transplantation. University of Miami/Jackson Memorial Hospital experience.

Cardiac transplantation at the University of Miami/Jackson Memorial Medical Center began in November 1986. Up to April 1993, 220 potential recipients were evaluated and 84 accepted. Sixty-three patients received transplants while 14 died awaiting a donor heart. Within six years, the average recipient age has increased from 40 to 59 years. The overall one, two, and five-year survival rates (including operative deaths) are 89.5%, 82%, and 76%, respectively. Postoperative complications and mortality were related to infection or rejection frequently encountered within the first 28 months; thereafter the actuarial survival curve showed minimal decay and the complication rate declined. One or more hospital readmissions were needed in 38 of 57 (67%) survivors. Among the 50 long-term survivors, 42% are working and 84% are in New York Heart Association functional Class I.

Atrialization of right ventricular pressure during acute cardiac allograft rejection.

We describe a patient who, during an episode of acute cardiac rejection, developed such severe systolic dysfunction that there was transient near-adynamic function of the right ventricle. This right ventricular dysfunction was reflected hemodynamically by the unusual finding of atrialization of right ventricular pressures. The patient's cardiac function returned to normal after treatment with extensive immunotherapy.

Ovarian pregnancy in association with an intrauterine device.

Within a 1-year period, two cases of primary ovarian pregnancy associated with an intrauterine device (IUD) occurred in our institution. The clinical signs and symptoms were similar to those in any other ectopic pregnancy. Both cases satisfied the criteria of Spiegelberg. The final diagnosis was based on the histopathological findings. A review of the literature is presented summarizing the 53 cases reported until now. The association between IUD, ectopic pregnancy in general, and ovarian pregnancy in particular is discussed.

Cell volume decrease during Friend leukemia cell differentiation.

Friend leukemia cells (GM86, clone 745) were induced to differentiate with dimethyl sulfoxide or butyric acid. The kinetics of induction were measured by cell growth, cell volume distributions, and [3H]thymidine incorporation. From the volume distributions, it was found that the rate of induction was both agent sensitive and concentration dependent. The changes in volume distributions occurred approximately 4 hr earlier with dimethyl sulfoxide induction relative to butyric acid induction. However, the changes with the butyric acid induction were more dependent on concentration. A decrease in labeling indices during the 12- to 20-hr time period was correlated to a decrease in mean cell volume and an increase in the proportion of G1 cells. After the 20-hr time period of induction, an increase in labeling indices and in the percentage of large cells was observed. The data suggest that a transient block of cells in G1 occurred between 12 to 20 hr, and that the early differentiation involved a volume decrease which was related to a redistribution of cell cycle stages. The study was also shown that the changes of cell volume are a rapid monitor to determine the early events of the differentiation process.

Use of propranolol in dysfunctional labour.

Labour pains associated with fear and anxiety increase the blood level of catecholamines. This in turn causes dysfunctional labour due to the weak uterine contractions which follow stimulation of uterine adrenergic beta receptors. Intravenous propranolol was administered to ten primigravidae with typical dysfunctional labour. This was shortly followed by normal uterine activity and delivery without any significant maternal or fetal complications. To the best of our knowledge this is the first attempt to treat dysfunctional labour by the intravenous administration of a beta-blocking agent, and our preliminary results are encouraging.

Pulmonary artery torsion: a potentially lethal complication after orthotopic heart transplantation.

We report a case of a young man with acute right ventricular failure immediately after an orthotopic heart transplantation. The clinical presentation suggested acute reactive pulmonary hypertension. Hemodynamic and angiographic data proved it to be an inadvertent pulmonary artery torsion. The clinical hemodynamic and angiographic characteristics of this potentially lethal complication are discussed.

Clear cell carcinoma of the endometrium and cervix in postmenopausal women: a case report.

Clear cell carcinoma of the endometrium and uterine cervix is a rare entity. Most of the recent literature deals with its occurrence in young women, and links it with intrauterine exposure to synthetic estrogens. Three cases of clear cell carcinoma in postmenopausal women are presented, with their histopathological findings. The clinical picture, incidence, origin and treatment of this neoplasm are reviewed.
