Lipoprotein (a) and other risk factors in patients with non-insulin-dependent diabetes mellitus.

BACKGROUND: Studies have established a relationship between lipoprotein (a) [Lp(a)] levels and cardiovascular disease, but few have studied Lp(a) in patients with non-insulin-dependent diabetes mellitus (NIDDM). METHODS: We determined Lp(a) concentrations, levels of glycated hemoglobin, and the personal and family history of atherosclerosis in 88 patients with NIDDM (53 men and 35 women; age, 33-70 years) and 90 age- and sex-matched controls. Twenty-three patients with NIDDM had cardiovascular disease (CVD group) and 65 did not (non-CVD group). RESULTS: Lp(a) levels were higher in CVD than non-CVD patients (P < 0.01). Triglyceride levels negatively correlated with Lp(a) (r = -0.51, P < 0.05), independently of the metabolic control of diabetes. Patients with poor metabolic control (glycated hemoglobin > 7.5%) had higher Lp(a) levels than the control group (P < 0.05). Lp(a) levels were higher than 0.30 milligram in 11% of patients without CVD and 55% of those with CVD (P < 0.05). Cluster analysis showed that Lp(a), as well as total cholesterol, triglycerides, apolipoprotein B100, and age were independently related to CVD in patients with NIDDM (P < 0.001 for triglycerides and P < 0.05 for the other variables). CONCLUSIONS: Lp(a) levels can be considered an independent risk factor for the development of atherosclerosis in NIDDM.

The solid variant of adenoid cystic carcinoma of the cervix.

We studied seven examples of the solid variant of adenoid cystic carcinoma of the uterine cervix in postmenopausal women who presented with vaginal bleeding and a large ulcerated or polypoid cervical mass. The tumors lacked the characteristic cribriform pattern of conventional adenoid cystic carcinoma. The neoplastic cells were small, undifferentiated, or basaloid and grew in cords, nests, trabeculae, and nodules. Foci of squamous cell carcinoma were seen in three tumors and areas of necrosis in four. A characteristic feature was the production of abundant periodic acid-Schiff's procedure (PAS)-positive basement membrane material that was immunoreactive for collagen IV and that in some areas compressed tumor cells. Electron microscopy on three cases showed globules and cylinders of redundant basal lamina. The tumor cells were joined by desmosomes and contained bundles of tonofilaments. Material similar to basement membrane material appeared to be intracytoplasmic in two tumors. No neurosecretory granules or myoepithelial cells were found. Four deaths were tumor related. Two patients are currently alive, but with local recurrence or metastases; another is alive and well 19 months after surgery. We believe that the solid variant of adenoid cystic carcinoma of the cervix is a distinctive neoplasm that should be separated from small cell carcinomas with or without endocrine features, adenoid basal cell carcinoma, and squamous cell carcinoma.

Molecular dynamics of water in foods and related model systems: multinuclear spin relaxation studies and comparison with theoretical calculations.

A review of recent studies of molecular dynamics of water in foods and model systems is presented, and the theoretical results are compared with experimental data obtained by several techniques. Both theoretical and experimental approaches are discussed for electrolytes, carbohydrates, and food proteins in solution. Theoretical results from Monte Carlo simulations are compared with experimental NMR relaxation data for quadrupolar nuclei such as those of deuterium and oxygen-17. Hydration studies of wheat, soybean, corn, and myofibrillar proteins by multinuclear spin relaxation techniques are discussed, and several new approaches to the analysis of the experimental data are considered. Correlation times of water motions in hydrated food systems are determined from NMR and dielectric relaxation data. The values of the correlation times for dilute solutions of electrolytes and carbohydrates estimated by NMR are in good agreement with those calculated from dielectric relaxation data, but seem to differ significantly from those proposed from Monte Carlo simulations. Several new and important results concerning the hydration of potato and cereal starches are presented, showing the very different hydration behaviors of these two major groups of starches. The combination of molecular dynamics computations with NMR relaxation techniques will hopefully stimulate novel technological developments in food engineering based on such fundamental studies.

The use of biotechnology in the conservation of tropical germplasm.

The rapid erosion of the genetic diversity of both wild and cultivated plants has recently attracted more and more international concern. As a consequence, germplasm conservation techniques have become accessible to protect the third world's germplasm diversity. Several approaches are used for germplasm conservation and utilization. The evaluation and use of these techniques has to be focussed with one important objective; to preserve with as much integrity as possible, the genetic variability of the selected species. Questions have risen about the best way to preserve genetic diversity of the humid tropics, since it is being recognized that these conditions hold much of the world's gene pool of plants. Seed banks are considered the best system for orthodox seed storage. For many tropical species that cannot support dehydration, tissue culture and cryoconservation are the best alternative. Important considerations regarding the adaptation of modern techniques to tropical species have to be carefully analyzed. Quantitative studies of genetic indices of somaclonal variation, or types or mutant plants can be observed. Consideration of plant germplasm as a base for genetic improvement has come a long way, but much remains to be done, particularly, with the plant diversity that exists in the tropics. All efforts to conserve and use genetic resources will contribute to the benefit of future human generations.

Structure-activity relationships of polymethoxyflavones and other flavonoids as inhibitors of non-enzymic lipid peroxidation.

Polymethoxylated flavones and C-glycosyl derivatives isolated from medicinal plants besides other flavonoid compounds were studied for their influence on lipid peroxidation induced by FeSO4+ cysteine in rat liver microsomes. A number of hydroxyflavones (e.g. luteolin); C-glycosyl-flavones (e.g. orientin); methoxyflavones (e.g. gardenin D) and flavonols (e.g. datiscetin), as well as the flavanol leucocyanidol and the biflavone amentoflavone behaved as inhibitors of non-enzymic lipid peroxidation. Structure-activity relationships were established and it was observed that the structural features for active polyhydroxylated compounds were different from those of polymethoxylated flavones, antiperoxidative flavonoids possessing a high lipophilicity.

Recombinant gamma-interferon as adjuvant to hepatitis B vaccine in hemodialysis patients.

Patients undergoing long-term hemodialysis are at high risk of acquiring hepatitis B yet tend to have poor rates of response to hepatitis B vaccine. The effect of recombinant human gamma-interferon (2 million units/m2) on the response to a recombinant hepatitis B vaccine was evaluated in a prospective, randomized controlled trial in 81 hemodialysis patients. A similar proportion of both groups of vaccinees ultimately developed antibody to HBsAg including 81% of the 41 recipients of vaccine alone (group I) and 89% of the 40 recipients of vaccine with gamma-interferon (group II). However, the antibody to HBsAg response occurred earlier in recipients of vaccine with gamma-interferon, so that at 4 mo 63% of group I and 88% of group II had antibody to HBsAg (p less than 0.025). Furthermore, titers of antibody to HBsAg tended to be higher in the vaccinees given interferon; the final geometric mean titers were 232 IU/L in group I and 330 IU/L in group II (p = not significant). Retrospective testing for antibody to hepatitis C virus revealed that 21 (26%) hemodialysis patients were seropositive at entry into this trial, but the presence of antibody to hepatitis C virus did not appear to affect the response rate to the hepatitis B vaccine. These results suggest that the effects of gamma-interferon as an adjuvant in increasing the response rate to hepatitis B vaccination deserve further evaluation perhaps most appropriately in persons who have not responded to an initial course of vaccine.

Multinuclear spin relaxation and high-resolution nuclear magnetic resonance studies of food proteins, agriculturally important materials and related systems.

An overview of the applications of Nuclear Magnetic Resonance (NMR) techniques in Agriculture and Food Chemistry, covering high-resolution, solid-state, pulsed gradient and two-dimensional techniques is presented. The systems investigated by such techniques range from purified proteins to mixtures, starch granules and wheat grains. Both hydration and structural/composition approaches that employ NMR techniques are discussed from the point of view of applications rather than technique development. Hydration models derived from multinuclear studies are discussed for food proteins and enzymes. The role of protein-protein interactions in the analysis of the NMR results on hydration is also discussed. Amongst the food proteins considered are: wheat gliadins, glutenins, corn zeins, soy glycinins and conglycinins, as well as muscle proteins. Several new applications and new directions of development of NMR in Agriculture and Food Chemistry are suggested, and potential practical applications are pointed out.

[Dyspeptic syndrome in heart diseases].

Many investigations have ascertained the relation between dyspepsia and diseases or disorders of gastrointestinal tract; on the contrary, the topics concerning the dyspeptic syndrome during acute cardiac disorders are very few. For this reason, Authors studied the upper tract of digestive apparatus in some patients affected with congestive heart failure and ischemic heart disease. Data have showed alterations of motility in esophagus, stomach and duodenum in every patient and lesions of gastric mucous membrane in more than half of them.

Chest physiotherapy in preterm infants with RDS in the first 24 hours of life.

To evaluate if chest physiotherapy is beneficial to premature infants with respiratory distress syndrome (RDS) during the first 24 hours of life, 20 infants were randomly assigned to two groups; 10 infants in Group I received routine chest physiotherapy and suction, and 10 infants in Group II received suction only. The birth weight, gestational age, postnatal age, Apgar scores, blood gases, acid-base status, and ventilatory requirements prior to study were comparable between the two groups. There were no significant differences between the groups in the amount of endotracheal secretions removed, the PO2/FIO2 ratio, blood gases, and pH during the study. The incidence of patent ductus arteriosus (PDA), bronchopulmonary dysplasia (BPD), Grade I and II intraventricular hemorrhage (IVH), and mortality was comparable. However, five of 10 Group I and zero of 10 Group II infants developed Grade III or IV IVH (P less than 0.05).

Prevention of gastric inflation during mask ventilation in newborn infants.

Ten premature infants who had recovered from the respiratory distress syndrome were subjected to cricoid compression during mask ventilation. Each infant was ventilated with an AMBU face mask for 3 min with the nasogastric (NG) tube, without the NG tube, and without the NG tube but applying cricoid pressure during ventilation. The results of the study indicated that, similar to insertion of an NG tube, cricoid compression can prevent gastric inflation during mask ventilation.

Centrocentral anastomosis in the prevention and treatment of painful terminal neuroma. An experimental study in the rat.

In this experimental study, microsurgical centrocentral anastomosis was applied to an experimental model of painful terminal neuroma resulting from left sciatic nerve section in the rat. The anastomosis consisted of end-to-end suturing of the sciatic nerve fascicles to the tibial branch, with the interposition of a nerve graft taken from the same anastomosed fascicle. As a control parameter for the experiment, the autotomy which follows sciatic nerve section in the rat was evaluated. Autotomy is considered an objective indication of abnormal sensations that are provoked by the formation of a terminal neuroma. Histological study of the proximal stump of the sciatic nerve was also performed. The observation period was 10 weeks. The study demonstrates that centrocentral anastomosis reduces the size of the neuroma formation and the incidence of autotomy.
