Spinal ascending pathways in amphibians: cells of origin and main targets.

As part of a research program on the evolution of somatosensory systems in vertebrates, the various components of ascending spinal projections were studied with in vivo and in vitro tract-tracing techniques in representative species of amphibians (the large green frog, Rana perezi, the clawed toad, Xenopus laevis and the ribbed newt, Pleurodeles waltl). Three main ascending sensory channels, each with largely separate targets, were demonstrated: 1. Ascending projections via the dorsal funiculus include primary and nonprimary projections that ascend to terminate mainly in the dorsal column nucleus at obex levels. A small component ascends farther rostralwards to terminate in the reticular formation, the octavolateral area, the trigeminal nuclear complex, and in the granular layer of the cerebellum. 2. Projections ascending via the dorsolateral funiculus reach other spinal and supraspinal targets than the dorsal funicular fibers, mainly ipsilaterally. At upper cervical cord and obex levels, many fibers innervate a region considered the amphibian homologue of the lateral cervical nucleus of mammals. In the medulla, these fibers ascend ventral to the descending trigeminal tract to terminate in the dorsal column and the solitary tract nuclei, and more rostrally, in the reticular formation, the descending trigeminal nucleus and the medial aspect of the ventral octaval nucleus. Major projections reach the area between the facial motor nucleus and the ventral octaval nucleus, and a mediolateral subcerebellar band. These projections arise in neurons located mainly in the ipsilateral deep dorsal and lateral fields throughout the spinal cord. 3. Ascending spinal projections via the ventral quadrant of the spinal cord (the ventral and ventrolateral funiculi) ascend throughout the brainstem up to the diencephalon. Along its course, this component innervates various parts of the reticular formation, the octavolateral area, the granular layer of the cerebellum, the region ventromedial and ventrolateral to the isthmic nucleus, and the subcerebellar region. In the mesencephalon, the torus semicircularis, the midbrain tegmentum and, sparsely, the tectum mesencephali are innervated. Beyond the midbrain, various dorsal and particularly ventral thalamic nuclei and the posterior tubercle are innervated by this ascending sensory channel. The cells of origin of some of these projections were observed in the dorsal, and to a lesser extent, in the lateral and ventral spinal fields of the spinal cord. Evidence for the presence of these three main ascending sensory channels throughout vertebrates will be discussed. The presence of such channels appears to be a shared character in the brain of both amniotes and anamniotes.

"Eraseroma" as a cause of rhinolith: CT and MRI in a child.

We report a 5-year old girl with progressive difficulty in breathing through the nose whose clinical diagnosis was nasal tumour. CT showed a calcified nodular mass and MRI a nonspecific nodular lesion in the right nasal cavity. The radiological suspicion was a rhinolith. The operative specimen showed that an eraser from a pencil was the primary source. We underline the rarity of this entity and the important role of radiological studies in preoperative recognition.

Early levels of CD4, neopterin, and beta 2-microglobulin indicate future disease progression.

Reduced CD4 T cell level and increased serum neopterin and beta 2-microglobulin levels, which reflect immunological activation and dysregulation, are three important markers of HIV disease. The aim in this study is to delineate more clearly the relation of activation to future CD4 values and disease progression. By analyzing a cohort of 198 seroconverters from the Multicenter AIDS Cohort Study with 9 years' follow-up, the dynamic changes and levels of these three markers and their interrelationships are explored. We observed that the levels of markers in the first year after seroconversion have a much stronger impact on the progression of the disease than the preseroconversion marker levels. The actual change during the year after seroconversion is not as important as the final level reached during that year. The early levels of markers after seroconversion appear to be good indicators of the subsequent course of disease as defined by CD4 level and slightly better than the quantitative changes following seroconversion or the changes in the period 1 to 2.5 years after seroconversion. To investigate the variation between subjects, the 198 seroconverters were stratified into three approximately equal-sized groups in 12 ways based on their pre- and postseroconversion levels and changes in the three markers. The group with the highest CD4 level within a year after seroconversion maintains the highest CD4 level 8 years after seroconversion. The group with the lowest level of neopterin or beta 2-microglobulin in this period has much higher future CD4 counts than the other two groups. The level of markers during the first year after seroconversion has a high predictive power for AIDS onset. Substantial differences in the hazards of AIDS are found between the groups with the highest and lowest CD4 count, neopterin, and beta 2-microglobulin following seroconversion. The three markers are generally correlated throughout the postseroconversion period but can provide distinct information. High current levels of neopterin or beta 2-microglobulin tend to be associated with low future CD4 count, while current levels of CD4 count have less association with future neopterin and beta 2-microglobulin levels.

Early detection of reversed diastolic umbilical flow: should we offer karyotyping?

In normal pregnancy, end-diastolic flow appears in the umbilical artery around the 13th week of gestation, with a velocity which increases progressively with advancing gestation. The detection of reversed flow in the umbilical artery, the highest expression of an increase in placental vascular resistance, is extremely uncommon in the first half of gestation and, in three of the four cases reported in the literature, there were chromosomal abnormalities. We report a new case of reversed end-diastolic flow in the umbilical artery in a 13-week fetus with increased nuchal translucency thickness, megacystis and tachycardia. Cytogenetic analysis of chorionic villi and amniocytes revealed trisomy 13. The findings provide further evidence for a possible association between reversed end-diastolic flow in the umbilical artery and chromosomal abnormalities. However, the effectiveness of this potential marker in an unselected population requires further evaluation.

Renogram deconvolution in the management of diabetic nephropathy: utility of the measurement of initial tracer uptake.

Our objective was to assess mean transit time (MTT) and initial uptake, both parameters derived from the renal retention function (RRF), in the study of renal function in patients with diabetic nephropathy. We studied 25 patients, 7 with type I diabetes mellitus and 18 with type II diabetes mellitus, all of whom fulfilled the criteria for diabetic nephropathy with proteinuria and/or retinopathy. We found a statistically significant correlation between initial uptake and the other biochemical and renographic parameters studied except proteinuria: serum creatinine (r = 0.66, P < 0.002), creatinine clearance (r = 0.61, P < 0.003), glomerular filtration rate (r = 0.74, P < 0.003) and effective renal plasma flow (r = 0.66, P < 0.003). The other renographic parameters studied (maximal activity of the conventional renogram and MTT of the deconvoluted renogram) did not show any correlation. Initial uptake is a semi-quantitative renographic parameter that can provide complementary information to biochemical data and it may be useful in the management of diabetic nephropathy, especially in patients with high serum creatinine or creatinine clearance.

Training as an influential factor on the locomotor pattern in Andalusian horses.

The physiological and metabolic adaptations to exercise of the Andalusian horse seem to differ slightly from those found in other breeds. The authors studied the locomotor adaptation of 18 Andalusian horses to a training programme in relation to anaerobic energy metabolism, efficacy of the training programme, and changes associated with the onset of fatigue. They also tried to find out if these changes had become diminished by training. Several locomotor variables during trotting and galloping were determined and plasma lactate concentrations were measured before training and after two training periods of different lengths. While trotting after training, an increase in the duration of the restraint phase, a reduction in the propulsion phase and a decrease in the duration and length of step were observed. During galloping, training caused a reduction in stride length but an increase in stride frequency. Significant correlations were obtained between stride duration, frequency and length of step and lactate concentrations. The training might have caused a shift in metabolism from anaerobic to aerobic predominance. This reduction was coupled with a decrease in stride duration, in response to a shorter stance phase duration and an increase in stride frequency. A reduction in the fatigue level was not observed. The study concludes that this training programme was not sufficient for improving the physical capacity of Andalusian horses.

Hypothyroidism alters the expression of prostaglandin D2 synthase/beta trace in specific areas of the developing rat brain.

Lipocalin-type prostaglandin D2 synthase is the enzyme responsible for the synthesis of prostaglandin D2, a major prostaglandin in the central nervous system. We analysed the effects of thyroid hormone deprivation on prostaglandin D2 synthase gene expression in the developing rat brain. By in situ hybridization, the strongest prostaglandin D2 synthase mRNA signal was detected in the leptomeninges and choroid plexus. The signal was greatly reduced in the cerebellar interlaminar meninges of hypothyroid rats aged 15 and 25 days. Immunohistochemical studies defined changes in the location of the prostaglandin D2 synthase protein. In control but not in hypothyroid animals, Cajal-Retzius neurons of cortical layer I, and pyramidal cortical plate neurons were intensely stained on postnatal day 5. Conversely, prostaglandin D2 synthase protein levels were higher in neurons of the CA1 and CA3 regions and the dentate gyrus of the hippocampus of hypothyroid animals on postnatal days 5, 15 and 25, and also in subplate neurons on postnatal days 15 and 25. In agreement with the in situ hybridization and northern blotting data, the major difference was found in the cerebellar interlaminar meninges of hypothyroid animals, where the protein was clearly down-regulated on postnatal days 15 and 25. These results show that hypothyroidism causes both age- and region-specific alterations in the expression and location of the prostaglandin D2 synthase during postnatal brain development, probably reflecting a cell-specific regulatory effect of thyroid hormone on the prostaglandin D2 synthase.

Pathogenesis of simian immunodeficiency virus encephalitis: viral determinants of neurovirulence.

To examine the relationship between macrophage tropism and neurovirulence, macaques were inoculated with two recombinant hybrid viruses derived from the parent viruses SIVmac239, a lymphocyte-tropic, non-neurovirulent clone, and SIV/17E-Br, a macrophage-tropic, neurovirulent virus strain. The first recombinant, SIV/17E-Cl, contained the portion of the env gene that encodes the surface glycoprotein and a short segment of the transmembrane glycoprotein of SIV/17E-Br in the backbone of SIVmac239. Unlike SIVmac239, SIV/17E-Cl replicated productively in macrophages, demonstrating that sequences in the surface portion of env determine macrophage tropism. None of five macaques inoculated with SIV/17E-Cl developed simian immunodeficiency virus (SIV) encephalitis. The second recombinant, SIV/17E-Fr, which contained the entire env and nef genes and the 3' long terminal repeat of SIV/17E-Br in the SIVmac239 backbone, was also macrophage tropic. Six of nine macaques inoculated with SIV/17E-Fr developed SIV encephalitis ranging from mild to moderate in severity, indicating a significant (P = 0.031) difference in the neurovirulence of the two recombinants. In both groups of macaques, CD4+ cell counts declined gradually during infection and there was no significant difference in the rate of the decline between the two groups of macaques. This study demonstrated that macrophage tropism alone is not sufficient for the development of neurological disease. In addition, it showed that while sequences in the surface portion of the envelope gene determine macrophage tropism, additional sequences derived from the transmembrane portion of envelope and/or nef confer neurovirulence.

Smoking and bronchial responsiveness in nonatopic and atopic young adults. Spanish Group of the European Study of Asthma.

BACKGROUND: Smoking may influence the response of the lungs to other inhaled substances. A study was undertaken to assess the effect of the interaction between smoking and the immunoresponse to common aeroallergens (atopy) on bronchial responsiveness. METHODS: A random sample was selected from the general population census of five areas of Spain (Albacete, Barcelona, Galdakao, Huelva, and Oviedo). A total of 1169 (35%) subjects completed a face-to-face respiratory questionnaire, a methacholine bronchial responsiveness challenge, and underwent measurements of total and specific serum IgE levels to mites, pets and moulds. A survival model (Weibull) was used to examine the methacholine dose-response relation, adjusting for bronchial obstruction. RESULTS: Smokers showed greater bronchial responsiveness than never smokers (p < 0.05) at any dose of methacholine, but only among non-atopic individuals. Atopy had a large effect on responsiveness at low levels of methacholine, but smoking did not increase responsiveness in atopic subjects. There were no differences in intensity or cessation of smoking between atopic and non-atopic subjects, suggesting that smoking self-selection does not fully explain these results. CONCLUSIONS: The association between smoking and bronchial responsiveness varies with atopy, which may be explained by different immunological and/or inflammatory effects of smoking on atopy.

Syphilis in injecting drug users: clues for high-risk sexual behaviour in female IDUs.

The objective was to measure the gender-specific differences for syphilis and for the sexual transmission of human immunodeficiency virus (HIV) in a cross-sectional analysis of injecting drug users (IDUs) admitted to detoxification between February 1987 and January 1990. HIV was determined by enzyme-linked immunosorbent assay (ELISA) and confirmed with Western blot. For syphilis reactive samples to a rapid plasma reagent (RPR) were confirmed with treponemal tests (FTA-ABS or MHA-TP). Of the 386 heterosexual IDUs, 68% were HIV-positive and 4.7% had serologic syphilis (RPR and FTA-ABS or MHA-TP positive). Syphilis was higher in women (12%) than in men (3%), and women reported a significantly (P < 0.001) higher number of sex partners. Men had an IDU as a sex partner more often than women did (P = 0.001). Serologic syphilis in women was associated with having had more than one sexual partner in the previous year (P = 0.028) but this association was not present in men. HIV infection was not associated with syphilis in male IDUs. However, HIV was present in all women with syphilis that reported more than one partner.

The use of functional indexes to evaluate fitness in Andalusian horses.

The fitness of 8 Andalusian horses between 3 and 4 years of age was analysed. The animals were subjected to an exercise test on a sandy track consisting of 2 stages of different intensities. The first stage was of submaximal intensity at 4 speeds which increased progressively (4.17, 5.56, 6.94 and 8.33 m/sec.) covering a distance of 1,000 m in each level. Between each of these speeds, the horses rested for 2 min. The second stage was a maximal speed test over the same distance carried out 2 min after the ending of the maximal phase. Data of heart rate, plasma lactate concentration, velocity, PCV and pH in the blood were obtained. Maximum heart rate, maximum velocity, VLA2, VLA4, peak lactate, minimum pH and maximum PCV were considered functional indexes. A principal component enabled us to segregate horses according to their fitness and in relation to the information provided by the trainers in charge of these horses. The most discriminant variables in order to segregate horses were pHmin, VLA4, HRmax, VLA2 and Vmax. Differences between horses in relation to PCVmax were not found. The influence of each one of these functional indexes on the test exercise tolerance was discussed.

Early versus deferred zidovudine monotherapy: impact on AIDS-free time and survival in the multicenter AIDS cohort study.

The objective of this study was to compare the time to AIDS and to death between men receiving zidovudine therapy before or not before the diagnosis of AIDS. For the time to AIDS comparison, 821 men receiving zidovudine therapy before the diagnosis of AIDS were pair matched with men who did not receive zidovudine therapy until after diagnosis on their CD4 cell count (+/- 75 cells/mm3), haemoglobin level (+/- 0.75 g/dl), number of clinical symptoms and study visit at the time of initiation of zidovudine therapy and were monitored for a median of 2.08 years. For the time to death comparison, 186 men who received zidovudine therapy prior to AIDS diagnosis were pair matched on the same variables to men who received zidovudine therapy only after the AIDS diagnosis, and were monitored for a median of 2.88 years. Only men with < 350 CD4 cells/mm3 who received zidovudine therapy prior to AIDS diagnosis remained AIDS free significantly longer than their pair match who did not (P < 0.0001). The median extension of time to AIDS was 0.61 years for men with < 200 CD4 cells/mm3 and 1.13 years for men with 200-349 CD4 cells/mm3. Cox regression analysis showed a significantly increased time to AIDS for the men with < 350 CD4 cells/mm3, both before and after adjustment for the use of prophylactic drugs against Pneumocystis carinii pneumonia. No difference was seen in the time to death between men receiving zidovudine therapy before or only after AIDS diagnosis. Zidovudine treatment of asymptomatic HIV-1-infected men provides significant benefit to men with < 350 CD4 cells/mm3 by extending AIDS-free time, but does not extend survival. The analytical technique used is applicable to other observational studies of treatment.

Oltipraz chemoprevention trial in Qidong, People's Republic of China: study design and clinical outcomes.

In 1995, 234 adults from Qidong, Jiangsu Province, People's Republic of China, where hepatocellular carcinoma is the leading cause of cancer deaths and exposure to dietary aflatoxins is widespread, were enrolled and followed in a Phase II chemoprevention trial. The goals of the study were to define a dose and schedule of oltipraz for reducing levels of validated aflatoxin biomarkers and to characterize dose-limiting toxicities. Healthy eligible individuals, including those infected with hepatitis B virus, were randomized to receive either 125 mg of oltipraz daily, 500 mg of oltipraz weekly, or placebo. Blood and urine specimens were collected to monitor toxicities and evaluate biomarkers over the 8-week intervention period and subsequent 8-week follow-up period. Unique trial aspects included a synchronous follow-up schedule, daily observed administration of all medications, timely international data transference, and use of biomarkers as outcomes. One hundred thirty-two participants took their medications without interruptions, approximately 77% contributed all nine urine samples, and 78% contributed all seven blood samples. Fifty-one participants (21.8%) reported clinical adverse events. An extremity syndrome, developing soon after the start of treatment, was the only event that occurred more frequently (P = 0.002) among the active groups (18.4 and 14.1% of the daily 125 and weekly 500 mg arms, respectively) compared with placebo (2.5%). The oltipraz arms did not differ in symptom type or severity, and there were no indications of exacerbated drug intolerance among the few participants infected with hepatitis B virus. The good compliance with an intense follow-up schedule shows that chemoprevention trials with biomarker end points may be conducted in such populations.

[Sequential chemotherapy and radiotherapy in the treatment of locally advanced carcinoma of the head and neck].

In patients with non-resectable carcinoma of the head and neck, radiotherapy produces an average survival of 12 months. Neoadjuvant chemotherapy followed by radiotherapy has been shown to prolong survival and preserve organ structure and function, and to reduce systemic relapses in patients with advanced, resectable disease. An analysis was made of 35 patients treated with sequential chemotherapy and radiotherapy, 20 with non-resectable disease and 15 with resectable disease who refused surgery. Treatment consisted of 4 cycles of cisplatin + 5-fluoruracil in a 96-h continuous intravenous infusion in 16 patients, and the same treatment with leucovorin in 19 patients. After chemotherapy, the patients underwent cobalt irradiation of the primary tumor and neck. Chemotherapy produced a complete response rate of 46% and a partial response rate of 28%, yielding an overall rate of 74%. After radiotherapy, the CR increased to 63%. After 92 months of maximum follow-up, overall survival was 20% and median survival was 25 months. Locoregional relapses were the main cause of treatment failure. Our results suggest that sequential chemo-radiotherapy achieved encouraging CR rates with no significant increase in toxicity, prolonged survival in non-resectable patients, and enabled organ preservation in patients with resectable disease.
