[Isolated hypoplasia of the right ventricle].

Four cases of hypoplastic right ventricle with intact ventricular septum are presented. All of them were diagnosed by means of an angiographic study, and one case was examined postmortem. The anatomic, physiologic and clinical features of this malformation are analyzed and the differential diagnosis with similar congenital cardiac diseases is discussed.

[Posttraumatic retropharyngeal hematoma of delayed onset].

Retropharyngeal hematoma (RE) is a rare occurrence. A case of RE in a 48-year-old man who suffered a whiplash trauma in a motor vehicle accident is reported. He had been receiving long-term aspirin therapy for ischemic heart disease. Management consisted on tracheostomy and medical treatment and the patient evolved satisfactorily. Whiplash injuries that take place in predisposed subjects can originate RE. Diagnosis is made by X-ray and CT. If acute airway obstruction occurs it is safer to perform tracheostomy than endotracheal intubation. If the lesion is non-expanding, medical treatment is preferred by most authors, so do we.

[Head and neck melanomas: diagnosis and treatment].

Melanomas are rare tumors that are usually lethal. Melanomas of the head and neck represent 15-20% of all malignant melanomas and mucosal melanomas 1-2%. We reviewed every case of melanoma of the head and neck diagnosed in our department in 15 years and analyzed presentation, clinical evolution, diagnosis and treatment. We emphasized early diagnosis.

Comparative study of flow parameters of excipients for direct compression excipients determined using a ring shear.

In a previous research it has been attempted to define the selective criteria for excipients used in direct compression by measuring their flow rate and their flow regularity. The control of flowability of Avicel PH 101 and Cellutab has now been realized using a ring shear cell. A comparison of results obtained shows a different behavior in both excipients. In this way, we obtained higher values of cohesion and friction coefficients for Avicel PH 101 than for Cellutab, and values of flow factor that indicate good flow for Avicel PH 101 and excellent flow for Cellutab.

[CNS vasculitis after ophthalmic herpes zoster infection].

We present a case of cerebral vasculitis secondary to infection with Varicella Zoster Virus which appeared in a 27 year old patient after a latency period of eight weeks. We emphasize the usefulness of angioresonance as a fitting means of evolutive follow up without the need to carry out control cerebral angiography. We likewise point out the usefulness of acyclovir as the sole treatment necessary in non-complicated cases.

[Kukuchi's disease of the neck: report of two cases].

Kikuchi's disease is an unusual disease, described in 1972, that is characterized by lymph node enlargement and fever. Cervical lymph nodes are affected so often that the ear, nose and throat specialist should be aware of this entity during differential diagnosis. Two cases treated in our service are reported and the literature is reviewed.

[Anesthesia paresthetica: contribution of a new case and evolutive study using magnetic resonance].

Patients with subclinical cobalamin deficiency may develop a subacute myeloneuropathy after nitrous oxide anesthesia. Frequently they have hematologic manifestations, however there is only a few reports of spinal lesions shown on MR and particularly of the effects of cobalamin replacement therapy on these lesions. We describe a patient with unsuspected cobalamin deficiency who developed subacute combined degeneration of the spinal cord after nitrous oxide anesthesia. MR showed high intensity in dorsal columns on T2-weighted image, and posterior resolution of abnormalities after therapy. We discuss the importance of preoperative hematologic manifestations for select high risk patients and prevent this complication. On the other hand, we consider MR as a useful tool to improve our knowledge in the pathophysiology of the neurologic manifestations of the process.

Actinomycotic brain abscess.

Actinomycosis is a chronic, suppurative and granulomatous bacterial infection, which tends to form abscesses. Involvement of the central nervous system (CNS) is rare. We describe the clinical and neuroradiological findings of four patients (3 men and one female) diagnosed of actinomycotic brain abscesses. Risk factors were dental pathology, intrauterine device use, alcohol abuse and Rendu-Osler-Weber disease. Clinical manifestations were those of a space-occupying lesion. CT scan findings were varied and little specific: hemispheric or periventricular location, pyocephalus, rim-enhancing and edema. MRI was superior in disclosing pyocephalus. Diagnosis was performed by microbiological and pathological findings. Despite surgery and antibiotics, two of the patients died. In conclusion, the infection of the CNS by Actinomyces is potentially fatal and should be considered in those patients with cerebral abscess.
