[Combined thoracoscopy in thoracoabdominal wounds using ultrasound, the CO2 laser and the plasma jet].

Under analysis is an experience with treatment of 178 patients with thoracoabdominal wounds. The authors have developed a rational curative methods and original thoracoscopic techniques, which allowed to considerably reduce the amount of thoracotomies. Thoracoscopy was performed in 157 (88%) of 178 patients. Indications for thoracotomy were established in 7 patients, in 19 patients only drainage of the pleural cavity was made. Curative thoracoscopy was used in 131 patients, in 60 of them ultrasonic glue hermetization of the lung wound was made, 36 patients had laser photocoagulation of the lung wound, 21 patients had plasma coagulation of pleuro-pulmonary defects, in 14 patients--coagulated hemothorax was removed. Clinical effectiveness of thoracoscopic techniques was more than 90%. Thoracotomy was performed in 28 (15%) patients, in 11 patients it was supplemented with diaphragmotomy. Laparotomy was performed in 167 patients. Lethality was 6.8%.

Abdominal Injuries

[Surgical tactics in pleural empyema with regard to the use of a carbon dioxide laser and argon plasma].

The authors analyze the experience gained in treating 30 patients with acute and 23 with chronic pleural empyema. The management policy identified the following points. Laser thoracoscopic necrectomy and empyemic cavity sanitation are indicated in acute destructive pulmonitis. A combination of temporary bronchial occlusion and laser treatment of the empyemic cavity and bronchopleural fistula should be performed after debridement of a destructive pulmonitis area from necrotic tissues. Radical treatment of chronic pleural empyema should be better conducted in the early periods by employing a plasma scalpel at all the stages of surgical intervention for hemostasis, aerostasis and sterilization of the operation field.

Acute Disease

[The thoracoscopic use of the CO2 laser in the combined treatment of pleural empyema].

A new method for treating acute pneumothorax with a bronchopleural fistula was experimentally developed and clinically tested. It is based on thoracoscopic use of CO2 laser radiation. The laser beam energy is employed for evaporation of necrotic tissues in the area of pulmonary destruction, "welding" of the bronchopulmonary fistulae, and general irradiation of the empyema cavity with a defocused beam. Nineteen patients with various disease duration were treated. In 16 patients the bronchial fistulae and the empyema cavity were eliminated and the lung re-expanded. The purulent inflammation in the pleural cavity was acute in these cases.

Animals

[Hemopleuritis in closed thoracic injuries and multiple trauma].

In examination of 817 patients with closed chest injury and multiple trauma 57% of them were found to have obvious or latent hemopleuritis. According to the character of the primary damage and the developing pathophysiological changes, 3 forms of hemopleuritis were revealed: cardiac, pulmonary, and abdominal. A single complex of symptoms forms finally: pulmonary hypertension, venous stasis in the lungs, right-ventricular cardiac insufficiency. As a result, venous blood escapes through the lymph nodes, mainly those of the right lung, and together with the lymph flows in the dilated lymph vessels of the lungs into the pleural cavity. The differential-diagnostic signs of each form of hemopleuritis are pointed out, the prognostic criteria of their development are revealed, and the pathogenetically substantiated therapy is determined.

Abdominal Injuries

[The use of a plasma scalpel during thoracoscopy for the treatment of traumatic and spontaneous pneumothorax].

The authors suggest performing thoracoscopic coagulation of pleuropulmonary defects with a plasma stream (VSVU-160 apparatus) in patients with traumatic and spontaneous pneumothorax. A special nozzle and a manipulation trocar were designed for this purpose. Such combined thoracoscopy was used successfully in 31 patients with open and closed chest injury with rupture of the subpleural bullae. The manipulation makes it possible to reduce the number of emergency thoracotomies undertaken for hemo- and aerostatic indications.

Animals

[Lung edema in closed injury of the chest and polytrauma].

Edema of the lungs complicates the course of closed and concurrent trauma of the chest in 24.2% of cases. Depending on the system with the primary trauma, three types of lung edema may occur according to the clinical course and character of pathophysiological disorders: cardiogenic (28.3%)--in primary trauma or pathology of the heart with the cardiac insufficiency syndrome; noncardiogenic (34.3%)--in contusion or pathology of the lungs with development of the hypertension syndrome; edema of a mixed form (37.4%) induced by hypovolemia in blood loss and shock with the development of the "small cardiac output syndrome". It is just their pathogenetic essence which determines the character of intensive therapy. The lung edema mortality rate is 11.6%, maximum mortality rate (9.1%) occurs in edema of a mixed form.

Adolescent

[Optimization of the preoperative preparation of patients with chronic suppurative lung diseases].

Complex preoperative preparation of 87 patients with chronic suppurative diseases of the lungs included sanitation of the tracheobronchial tree by means of bronchoscopies (in 45 patients) and method of continuous nasobronchial catheterization (42 patients). The use of the latter resulted in rapid arrest of inflammatory alterations in the bronchial tree, effective sanitation of the suppuration focus, shorter period of preoperative preparation and less amount of postoperative complications.

Adolescent

[Use of acupuncture for restoring motor and transit functions of the stomach and intestines in suppurative peritonitis].

With the aim of restoration of motor-evacuatory function of the stomach and intestine, the acupuncture was used in 220 patients with purulent peritonitis. The positive effect after single procedure was noted in all the patients with local peritonitis. In 100 patients with diffuse peritonitis, 2-3 procedures were required to achieve clinical effect. There was no effect in 20 patients. The total lethality was 9.1%, in diffuse forms of peritonitis--14%.
