[Efficiency of includes of bioactive substances in diet of patient with hepatic encephalopathy].

We includes 66 patients with liver cirrhosis of Child-Pugh B class with hepatic encephalopathy of 0 to 2nd stages in randomized interventional study. 36 patients received standard treatment and 30 patients received standard treatment + bioactive substances in formula CognoBlend in capsules (2 capsules twice a day) in course of 5 weeks. Formula includes extracts of plants: Bacopa monneria, Gingko biloba, Cat's Claw, Gotu Kola, Rosemary. In group combined treatment was significant improvement of clinical signs, psychometric tests, electroencephalography and serum biochemistry than in group with standard therapy, on term of 2 to 5 weeks.

[Microcirculatory disorders in chronic erosions of the stomach].

Microcirculatory disturbances in chronic gastric erosions (CGE) were evaluated in 95 chronic gastric (CGE) patients using Doppler laser flowmetry to examine terminal blood flow of the gastric mucosa. Microcirculatory impairment in the fundal area was found in 93.7% patients with CGE, in the antral one in all the examinees. Patients with multiple and recurrent erosions and erosions associated with Helicobacter pylori had most pronounced microcirculatory disorders, especially in the antral stomach. When chronic erosions were treated with triad therapy on the basis of De-nol, the percentage of eradication exceeded 80%. The addition to therapy of the immunomodulator galavit promotes more effective and adequate restoration of blood flow in CGE patients.

[Interferon status of patients with non-specific ulcerous colitis and its correction with interferon inducers].

AIM: To examine validity of using interferon inductors (amixin and cycloferon) in combined treatment of patients with non-specific ulcerous colitis (NUC). MATERIAL AND METHODS: 113 NUC patients received basic antiinflammatory therapy (glucocorticoids and preparations of 5-aminosalycylic acid). Patients of groups 1 and 2 received interferon inductors (amixin and cycloferon in tables), respectively. Patients of groups 1a and 2a received placebo. All the patients were examined clinically with evaluation of interferon status and cellular immunity before and after the treatment. Colon mucosa biopsies obtained endoscopically were studied histologically. RESULTS: Alpha- and gamma-interferon production by leukocytes was substantially suppressed in all the examinees against normal levels of serum and spontaneous interferon. The interferon inductors stimulated relief of clinical symptoms and eliminated endoscopic signs of the disease. Amixin and cycloferon normalized interferon status in 36.3 and 33.3% of the treated patients, respectively. No response was registered in 9.1 and 8.4%, respectively. Improvement was seen in 54.6 and 58.3%, respectively. CONCLUSION: Cycloferon and amixin, interferon inductors, are effective in the treatment of NUC.

[Use of ednit in the treatment of portal hypertension in patients with chronic hepatitis and liver cirrhosis].

AIM: To study ednit (ACE inhibitor) effectiveness in the treatment of portal hypertension in patients with chronic hepatitis (CH) and hepatic cirrhosis (HC). MATERIAL AND METHODS: The examination of 87 patients with active CH and 54 patients with HC at the preascytic stage included two-dimentional ultrasonic scanning of the abdominal organs with doppler fluometry of the vessels in the territory of the portal vein, tetrapolar rheohepatography, ultrasound investigation, angiography of the abdominal vessels. The patients received combined therapy with ednit in a dose 5 mg twice a day for at least 5 weeks. RESULTS: Patients with active CH and HC having portal hypertension treated with adjuvant ednit in the above dose achieved marked clinicobiochemical remission quicker, portal-hepatic circulation improved. CONCLUSION: The addition of ednit to therapy of active CH and HC with portal hypertension is effective as it lowers total peripheral vascular resistance which eventually results in a fall of pressure in the territory of the portal vein and improvement of hepatic circulation. The highest effect is reached in administration of 5 mg twice for 24 hours.

[Clinical features of gastrointestinal diseases in patients with primary mitral prolapse].

Whether patients with mitral prolapse (MP) have specific features in the course of their gastrointestinal diseases (GD) was studied in 420 MP patients with various GD. Echocardiography and examination of the vegetative status were conducted in all the patients. On demand, esophagogastroduodenoscopy, ultrasonic investigation of the abdominal organs, multistage fraction duodenal intubation were made. The findings evidence of more severe course of GD in MP patients. This manifests in early onset, severe pain and dispeptic syndromes, marked mucosal inflammation and is explained by dysfunction of the vegetative nervous system and generalized dysplasia of the connective tissue.

[Central and pulmonary hemodynamics in patients with various forms of ischemic heart disease].

Two-dimensional and Doppler echocardiographies were used to examine right cardiac and pulmonary hemodynamics in patients with various types of coronary heart disease (CHD). All the patients were divided into 3 morphofunctional groups of CHD according to the left ventricular volume/mass index. They all were found to exhibit decreased left cardiac contractility and pump function, parallel with moderately elevated pulmonary pressure and right cardiac compensatory hyperfunction, which was largely evident in hypertrophic morphofunctional CHD. The degree of pulmonary hypertension was related to the severity of left ventricular pump dysfunction, but to its disturbed contractility in the dilated type.

[Pulmonary hemodynamics in patients with hypertension during acute drug tests using obsidan and corinfar].

Two-dimensional and Doppler echocardiographies were used to examine 102 hypertensive patients who were found to have pulmonary hypertension, whose manifestation was determined by the degree of arterial hypertension, cardiac contractile insufficiency and diminished left heart pump function. The degree of pulmonary hypertension was related to the hemodynamic type and maximal with hypokinetic circulation when the pump function of the heart was minimal. Obsidan caused a decrease in pulmonary and systemic arterial hypertension, producing higher effects with hypokinetic hemodynamics. Corinfar was demonstrated to attenuate pulmonary hypertension to a greater extent. Its action on systemic arterial hypertension was highest in patients with hyperkinetic circulation. Heterodirectional hemodynamic effects of these drugs make it expedient to study their combined action on the hemodynamic of hypertensive patients.

[Hemodynamics of the lesser circulation in patients with ischemic heart disease during acute obsidan test].

During acute obsidan testing, the hemodynamics was evaluated in the pulmonary circuit and right heart cavities of 60 patients with Functional Classes III-IV exertional angina pectoris, by using two-dimensional and Doppler echocardiographies of the pulmonary artery. According to the left ventricular myocardial volume/mass ratio, the patients were divided into 3 morphofunctional types of coronary heart disease (CHD). All the patients were found to have diminished contractility and pump function in the left heart along with moderately elevated blood pressure in the left heart and compensatory hyperfunction in the right heart, which are largely evident in the hypertrophic morphofunctional type of CHD. The degree of pulmonary hypertension was related to the severity of left ventricular dysfunction, whereas in the dilated type it was associated with its contractile disorders. In patients with the dilated and mixed types of CHD, obsidan caused a marked decrease in pulmonary hypertension, while in those with hypertrophic CHD it promoted its slight increase.
