[First experience with the modified program NHL-BFM-90 application in adult patients with primary diffuse large B-cell gastric lymphosarcoma with unfavourable prognosis].

AIM: To assess efficacy of a modified program NHL-BFM-90 in adult patients with primary diffuse large B-cell gastric lymphosarcoms (PDLBGL) with unfavourable prognosis. MATERIAL AND METHODS: Modified courses of NHL-BFM-90 were conducted in 5 patients aged 27-67 years from January 2004 to September 2005. Four patients received chemotherapy of the first line, in one patient block therapy followed monotherapy with chlorambucil and a CHOEP course. All the patients were in a severe clinical condition and had several initial factors of unfavourable prognosis: size of the tumor more than 10 cm; stage IE and more advanced; B-symptoms; proliferative activity above 70%. The program NHL-BFM-90 was modified because of the patients' age. Chemotherapy was conducted according to the middle arm of the original program NHL-BFM-90, but methotrexate was introduced in a dose 1 g/m2 for 12 hours, while leukovorin was given 18 hours after the start of methotrexate injection. In two cases the blocks were enhanced with rituximab, 2 patients had doxorubicin in block A, in one case block C was enhanced with methotrexate. A total of 23 modified blocks NHL-BFM-90 were performed: one patient was given 6 blocks, two patients--5, one patient--4 blocks and one patient--3 blocks. RESULTS: Four patients after block 2 and one patient after block 3 of polychemotherapy NHL-BFM-90 achieved remission of the disease of 6 to 22 months duration which still continues. Infectious complications related to hematological toxicity arose more frequently at the latest courses of chemotherapy. CONCLUSION: Treatment according to the modified program NHL-BFM-90 in adult patients with PDLBGL and unfavourable prognosis is highly effective. For a mean follow-up of 10.2 months no recurrences occurred. The number of courses can be reduced to decrease accumulated hematological toxicity and in case of rapid achievement of remission.

Ab initio calculations on bismuth cluster polycations.

Ab initio calculations on bismuth polycationic species of the types Bi(n(n-2))+, Bi(n(n-4))+, and Bi(n(n-6))+ (n = 3 - 12) were performed at the Hartree-Fock and density functional theory levels in order to investigate their general properties and the applicability of Wade's rules on bismuth polycations. Some exceptions to Wade's rules were encountered, and, moreover, several predicted and calculated minima show only meta-stable behavior. The bonding in bismuth polycations is characterized by a high degree of electron delocalization and "three-dimensional aromaticity".

[Changes in the cerebral circulation in persons with acquired heart defects].

A comprehensive estimation of the state of cerebral circulation was performed in 57 patients with acquired heart valve diseases (AHVD) of different types, etiology and severity. Doppler sonography, rheoencephalography and quantitative electroencephalography were used. It was determined that the alterations of cerebral circulation in patients with AHVD were first of all caused by disturbances of venous outflow due to congestive heart failure. Other factors, which had also influence on the results of the studies performed are outlined.

[The joint use of plasmosorption and enterosorption in the combined treatment of patients in the acute period of a myocardial infarct].

Plasmosorption and enterosorption were assigned to 35 acute myocardial infarction (AMI) patients. Plasmosorption was performed within an hour since AMI onset followed by a 5-day course of enterosorption (enterosorbent YO-A in a dose 30 g/day). The above combination produced a positive effect on hemodynamics and blood rheology resultant from a reduction in hematocrit value, plasma levels of fibrinogen and its derivatives, plasma viscosity and platelet aggregation. Ibustrin (200 mg) combined with heparin (100000 U) introduced into extracorporeal circulation effectively prevents thrombogenesis. AMI patients treatment with enterosorption plus plasmosorption enhances the response to nitroglycerin, reduces the rate of complications and lethal outcomes.

[Development of metachronous tumors of the colon].

Clinical and endoscopic evidence gained at the follow-up of 93 patients with metachroneous colon cancer suggests that at present there is minimal change to predict the occurrence, time of onset and site of emergence of the secondary tumors as the latter grow rapidly and rise in macroscopically free-of-tumor colon segments. In some patients metachroneous tumors were associated with long histories of benign adenomas. The authors hold that patients operated on for colon cancer should remain under life-long clinical and endoscopic observation.

[Hygienic evaluation of low-frequency magnetic fields of complex spectral composition].

As illustrated by electrical domestic appliances, it is demonstrated that existing variable magnetic fields of the sources using industrial current can to a great extent be nonmonochromatic and possess rather a complex spectral composition. Under the above conditions the root-mean-square value of the magnetic field intensity used at present for meteorologic certification of the sources is unacceptable for hygienic assessment of the existing external magnetic fields. It is established that the root-mean-square value of the rate of the field induction variations representing the degree of body interaction with the electric field induced by the variable magnetic field is regarded as a more adequate and universal characteristic of low-frequency magnetic fields.

[Study of the state of blood plasma by the spin probe method].

To evaluate the potentialities of the spin probe method in examination of the blood plasma state, a study was made into the blood plasma bound nitroxyl radical 2,2,4,4-tetramethyl-1,2,3,4-tetrahydro-5,6-benzo-gamma-carbolin-3-oxyl in healthy rabbits and those with pronounced experimental atherosclerosis. The probe absorption by high molecular plasma components was assayed in order to determine the binding constants and concentration of the binding centers, mobility of adsorbed probes and polarity of their surroundings. In the course of experimental atherosclerosis development these parameters underwent changes correlating with those of blood plasma cholesterol level. The data obtained suggest that the spin probe method may be used for assessement of the plasma state for diagnostic and research purposes.

Adsorption of small molecules to bovine serum albumin studied by the spin-probe method.

A spin-probe technique is used for quantitative EPR studies of adsorption of small molecules on globular proteins, of the rigidity of their binding to the protein and of the polarity of the environment. In the case of bovine serum albumin it is shown that nitroxyl radical (2, 2, 6, 6-tetramethyl-4-oxy-1-oxyl-piperidine)-stearate (I) has an adsorption behaviour similar to that of the fatty acids, nitroxyl radical (2, 2,4, 4-tetramethyl-1, 2, 3, 4-tetrahydro-5, 6-benzo-gamma-carboline-3-oxyl) (II) to that of the tryptophan molecule. Radical I rotates relative to the protein molecule, while Radical II is rigidly bound to the protein.

[Problems of physical training in the therapy and rehabilitation after myocardial infarct (hospital stage)].

Data are presented as to the effects of physical training on the process of rehabilitation of patients surviving myocardial infarction. Not only the restoration of physical activity was taken into consideration, but also the changes in the cardiovascular system, the course of its functional restoration. The observations were conducted in 380 patients during the hospital stage of rehabilitation. The advantages of individual programmes of rehabilitation over standard ones are demonstrated. The criteria for grouping the patients according to the severity of their state are given, as well as those for transferring the patients from one regimen to another. The importance of control of both the immediate, and late reactions to the exercises is emphasized.
