[Cellular mechanisms of impaired cardiac energetics in patients with dilated cardiomyopathy: decrease in mitochondrial respiration and creatine kinase expression].

The mitochondrial functional characteristics were assessed in the biopsy specimens from patients with various Functional Classes dilated cardiomyopathy (DCMP). The assessment was made by using endomyocardial biopsy specimens weighing 2-4 mg which had been taken from 39 patients aged 19-64 years during coronary ventriculography and cardiac transplantation. The status of mitochondria and the efficiency of mitochondrial creatine kinase functioning were evaluated by recording the respiration of saponin-skinned muscular fibers. The maximum mitochondrial respiration rate calculated on a dry weight basis was not substantially different in all functional classes of DCMP, while the acceptor control index (Vmax/V0) and the level of creatine-activated respiration decreased with an increase in the functional class of DCMP. The findings show a good positive correlation between ejection fraction and creatine-stimulated respiration values and a linear negative correlation between this parameter and end-diastolic pressures. Thus, the respiratory parameters of mitochondria in the endomyocardial biopsy specimens may be used to assess the severity of cardias lesions.

[Evaluation of the overall contractile function of the right ventricle using two-dimensional echocardiography in patients with myocardial infarction].

The paper presents two methods for calculating the contractile function of the right ventricle (RV): (1) three-plane one with the use of short- and long-axis-cut RV; (2) single-plane one which was modified to the use of long-axis RV images in the projection of 4 chambers. A total of 56 patients with first acute gross myocardial infarction were studied. A control group included 14 healthy subjects aged 25-60 years. The proposed three-plane method for estimating the volume parameters and ejection fraction of the right ventricle was shown to correlate with contrast ventriculography significantly. A high correlation was found between the stroke output of RV (as calculated for the left ventricle by the Chapman method) and that (by the three-plane method). The RV volume values in patients with anterior or inferior myocardial infarction suggest that there is a trend to suppress RV function in the patients, but it is more specific for those with inferior myocardial infarction.

[Effects of restored blood flow on the contractile function of the right ventricle in patients with acute myocardial infarct: data of two-dimensional computerized echocardiography].

The volumes of the right and left ventricles were measured in 78 patients with first acute transmural myocardial infarction at days 1, 3, 7, 14, and 28 of the disease. Thirty four patients were diagnosed as having anterior myocardial infarction, 35 presented with inferior myocardial infarction, and 9 had a concurrent right and left ventricular inferior wall myocardial infarction. A high incidence of right ventricular dysfunction was confirmed both in anterior and inferior myocardial infarction. The most profound right ventricular contractile dysfunctions were detected in patients with proximal right coronary occlusion in the absence of reperfusion. Successful thrombolytic therapy for myocardial infarction was found to affect right ventricular function to a lesser extent than left function.

[Serum neopterin in dilated cardiomyopathy].

RIA was used to measure the level of neopterine and beta 2-microglobulin in the sera of 43 patients with a verified diagnosis of dilated cardiomyopathy (DCMP). In DCMP patients, the mean level of neopterine was 11.25 +/- 1.05 nM/l and was significantly higher than in donors (5.6 +/- 0.5 nM/l). The concentration of neopterine exceeding 9 nM/l was discovered in 55% of DCMP patients. The rise of neopterine concentration was particularly marked in patients with inflammatory reaction in the myocardium according to endomyocardial biopsy. In that patients' group, the mean level of neopterine was 12.9 +/- 2.02 nM/l while the rise of its concentration over 9 nM/l was recorded in 9 (81%) of 11 patients examined. The correlation was established between the rise of the concentration of neopterine and beta 2-microglobulin. In patients with the concentration of neopterine over 9 nM/l, the mean level of beta 2-microglobulin constituted 3.33 +/- 0.32 mg/l and in patients with the normal concentration of neopterine, it amounted to 1.91 +/- 0.18 mg/l. The growth of the concentration of beta 2-microglobulin in the groups of patients under comparison was seen in 79 and 14% of cases, respectively. It is concluded that neopterine measurements are advisable in the laboratory diagnosis of myocarditis in DCMP patients. Potential mechanisms of the rise of neopterine formation are discussed from the standpoint of hyperinterferonemia.

[The localization of the components of the extracellular matrix and cytoskeleton in myocardial biopsies in alcoholic cardiomyopathy].

Distribution of I, III, IV and V-type collagen, fibronectin, vimentin, filamin, and desmin in myocardial biopsy specimens of 8 patients with alcoholic cardiomyopathy was studied by indirect immunofluorescence. It is found that all types of collagen and fibronectin participate in formation of diffuse cardiosclerosis while small cardiosclerosis is formed by interstitial types of collagen and to a lower extent of fibronectin. Filamin and desmin distribution in cardiomyocytes proves deformation and destruction of Z-lines and intercalated discs.

[Functional status of the hypertrophic left-ventricular myocardium in patients with hypertension].

27 patients with IIB stage of essential hypertension (EH) and left ventricular hyperthropy (LVH), 14 of them with IHD, were studied. In all of them coronaro- and ventriculography was performed. In 14 cases atherosclerotic alterations of coronary arteries were present. Nobody had abnormal pump function or contractility of myocardium. End-systolic, end-diastolic and stroke volume indexes in EH patients with pronounced coronary atherosclerosis were significantly increased. The examined patients demonstrated significant alteration of the left ventricular myocardium relaxation function. The conclusion is drawn that on a certain stage LVH is of compensatory character and plays an important role in supporting adequate myocardial pump function.

[Quantitative contrast angiography of the right ventricle: the normal indices and the problems in analyzing global and regional contractility].

Normal parameters of the right ventricular (RV) volumes, global and regional contractility were obtained in 20 normal subjects (15 men and 7 women, mean age, 47 +/- 8 yrs) by quantitative contrast RV ventriculography. Analysis of different methods of the angiographic RV volumes calculation was made. It was shown, that Ferlinz's method is adequate to get real values for RV. A new method for computer assessment of RV regional contractility was proposed--shrinkage of 5 areas in percent. The most difference (15 and 17%) between "internal" and "external" observer was found in the anterior and anteroapical segments of RV, differences in another segments were not significant (5-8%).

[Volumetric load in the diagnosis of early right-ventricular heart failure in patients with alcoholic heart damage].

Coronaroventriculographic data are presented for 32 patients with alcoholic heart damage (AHD). Volumetric load (400-450 ml polyglucin administration) was used to detect preclinical heart failure. Coronarography demonstrated atherosclerotic coronary arterial lesions in 7 of 32 patients. A comparative assessment of central hemodynamic and right-ventricular myocardial contractility parameters in the patients and control subjects showed that the ejection fraction was reduced in AHD patients because of a significant increment of end diastolic and end systolic volumes as well as significantly reduced right-ventricular stroke index. Repeated ventriculography after volumetric stress showed significantly increased volumes and reduced ejection fraction that were indicative of latent right-ventricular heart failure.

[Comparative angiographic evaluation of the functions of the left and right heart ventricles in dilated cardiomyopathy].

The paper is concerned with comparative analysis of disorder of left and right ventricular function (LV and RV, respectively) in 30 patients with dilatation cardiomyopathy after coronography, left and right ventriculography, myocardial biopsy. Disorder of LV and RV contractility was noted, however in most cases LV dysfunction was more noticeable. Direct correlation was revealed between the gravity of LV dysfunction and heart failure, whereas no significant differences were noted between RV parameters and heart failure, IIA-IIB stages.

[Total and regional myocardial contractility of the right ventricle during a volumetric loading test in patients with an alcoholic heart lesion].

The authors presented the results of right contrast ventriculography with a volumetric exercise test in patients with alcoholic heart affection. An increase in right ventricular end-systolic and diastolic volumes (most of the former), a decrease in the ejection fraction in response to an increase in the right ventricular end-diastolic pressure were noted during the test. A diffuse decrease in right ventricular regional (segmental) contractility in the absence of zones of akinesia or local contractility disorder was also revealed.

[Angiographic study of left and right ventricular function in patients with alcoholic lesions of the heart and dilatational cardiomyopathy].

An angiographic study of left- and right-ventricular function in 57 patients with alcoholic heart and dilatation cardiomyopathy demonstrated preclinical disorders of left-ventricular myocardial contractility and more marked right-ventricular changes in patients with second-stage chronic alcoholism. In cases of dilatation cardiomyopathy, left-ventricular dysfunction was predominant, while right-ventricular changes were less pronounced.

[A comparative assessment of the effect of Omnipaque and Verografin on ECG indices and myocardial contractility in patients with unstable angina during coronary ventriculography].

Altogether 30 patients with unstable angina (UA) were examined. In 15 patients omnipaque was used during coronography and ventriculography (VG), in the rest of 15 patients--verografin at common dosages. A study was made of the changes in the indices of central hemodynamics and myocardial contractility resulting from the action of these radiopaque media during VG, and of ECG changes during coronography. VG with verografin was accompanied by more noticeable disorders of left ventricular contractility, an increase in myocardial oxygen consumption; coronography with omnipaque was characterized by later ECG changes, less noticeable disorders of intraventricular conductivity. It is advisable that coronaroventriculography in patients with UA should be performed using a new group of radiopaque media, in particular omnipaque.
