Evaluation of serum tumor markers in patients with advanced or recurrent breast cancer.

Serum CA15-3, CEA, and BCA225 concentrations were determined in 98 patients with advanced or recurrent breast cancer in an attempt to correlate elevation with clinical status. The rate of serum positivity was 68.4% (67/98), 55.1% (54/98), and 43.9% (43/98) for CA15-3, CEA, and BCA225, respectively. After a 4 weeks-interval, a 20% change of tumor marker concentration from the preceding assay correlated significantly with clinical findings. Significant elevation was predictive of new recurrence or tumor regrowth after complete remission, especially in patients with bone metastasis. The 20% change in concentration at 4 weeks was also useful in patients with tumor marker concentrations persistently beneath the cut-off level for positive. Serological evaluation of tumor markers in patients with advanced or recurrent breast cancer should seek to document 20% changes over a 4 week interval.

Effects of bidisomide (SC-40230), a new class I antiarrhythmic agent, on ventricular arrhythmias induced by coronary artery occlusion and reperfusion in anesthetized rats; comparison with mexiletine and disopyramide.

We investigated the antiarrhythmic effects of bidisomide (SC-40230), a new class I antiarrhythmic drug, in early-phase ventricular arrhythmias induced by coronary artery occlusion and reperfusion in anesthetized rats. The effects of bidisomide were compared with those of mexiletine (MXT) and disopyramide (DSP), established class I antiarrhythmic drugs. Drugs were administered intravenously, 5 min before induction of coronary occlusion. Bidisomide (5 mg/kg) reduced the number of premature ventricular complexes and the incidence of ventricular tachycardia and ventricular fibrillation similarly to MXT and DSP in rats with ventricular arrhythmias induced by coronary artery occlusion. In rats with ventricular arrhythmias induced by coronary artery reperfusion following a 5-min coronary occlusion, the antiarrhythmic effects of 5 mg/kg of bidisomide were similar to those of the same doses of MXT and DSP. All three drugs significantly slowed the heart rate. Our results suggest that bidisomide may effectively reduce the severity of life-threatening ventricular arrhythmias that occur during acute coronary syndrome.

Role of conserved amino acid residues in the complementarity determining regions on hapten-antibody interaction of anti-(4-hydroxy-3-nitrophenyl) acetyl antibodies.

Monoclonal antibodies (mAbs) specific to (4-hydroxy-3-nitrophenyl)acetyl (NP) were prepared at various times after immunization and the amino acid sequences of VH and V lambda 1 in these mAbs were deduced from cDNA nucleotide sequences. Replacements due to somatic mutation were not found in day 7 mAbs but were found in those of days 14, 84 and 294. The affinity of day 7 mAbs to NP-glycine(NP-Gly) was in the order of 10(4) M-1 and it increased about 8000-fold with time after immunization. The extrinsic circular dichroism (CD) spectrum of the NP-epsilon-aminocaproic acid (NP-Cap)/Ab complex was unique for each mAb, although the spectra were grouped into two types, which tended to shift from one type to another with time, suggesting a variation in the micro-environments around NP-Cap in the combining sites. All these data indicate that the structure of the combining site was altered by somatic mutation; however, the fine-specificity measured by cross-reactivity with hapten analogues did not change significantly with time. We examined the amino acid residues in CDRs responsible for recognition of NP-haptens by comparing the amino acid sequences of anti-NP mAbs. Analyses revealed the presence of several conserved amino acid residues in CDRs of VH and V lambda 1, such as Tyr-32H, and Tyr-60H, in addition to a core segment involving Arg-50H.(ABSTRACT TRUNCATED AT 250 WORDS)

Atrophic nerve fibers in regions of reduced MIBG uptake in doxorubicin cardiomyopathy.

A myocardial MIBG-SPECT examination was conducted 2 wk after doxorubicin chemotherapy on a 52-yr-old woman without cardiac symptoms. Despite normal 201TI scintigraphy, reduced MIBG uptake was detected in the apical anterior, inferior and lateral segments of the left ventricle. The patient died of congestive heart failure due to doxorubicin-induced cardiomyopathy 10 mo later. At necropsy, the left ventricle was markedly dilated and the apical anterior, inferior and lateral walls were thin, stiff and whitish. Nerve fibers in the apical inferior wall were atrophic and markedly fibrotic where MIBG uptake was most reduced. Nerve fibers in the septum were normal where MIBG uptake had remained normal. The histologic findings correspond with the findings on the MIBG image. MIBG imaging may detect cardiac sympathetic denervation in doxorubicin-induced cardiomyopathy before cardiac symptoms are manifest and cardiac function deteriorates.

[Expression of E-cadherin in thyroid neoplastic tissues and its correlation with malignant and metastatic potential].

Cadherin is an Ca2+ dependent adhesion molecules, which contribute to cell-cell adhesion and maintenance of tissue structures. Its decrease or functional abnormality plays an important vole in cancer invasion and metastasis. We investigate the expression of E- cadherin in thyroid neoplasms (papillary cancer, follicular cancer, medullary cancer anaplastic cancer and follicular adenoma) and normal thyroid. The decrease of E-cadherin expression in thyroid cancer correlates with its malignancy and metastatic potentiality.

Highly enhanced fucosylation of alpha-fetoprotein in patients with germ cell tumor.

BACKGROUND: We have recently reported that simultaneous determination of the fucosylation and glucosaminylation of alpha-fetoprotein (AFP) was useful in the differential diagnosis of various liver diseases. Here, we measured these indices in germ cell tumor and compared them with those in various diseases to reveal the characteristic feature of the carbohydrate moiety of AFP in this tumor. METHODS: Fucosylation and glucosaminylation indices of AFP were determined for 743 serum samples from 10 patients with germ cell tumor, 437 with hepatocellular carcinoma (HCC), 241 with benign liver diseases (24 patients with acute viral hepatitis, 99 with chronic hepatitis, and 118 with hepatic cirrhosis), and 55 with carcinoma metastatic to the liver from digestive organs (41 from stomach, 10 from gallbladder, 2 from pancreas, and 2 from colon) by crossed immunoaffinoelectrophesis in the presence of concanavalin A and Lens culinaris agglutinin. RESULTS: The fucosylation index of AFP in germ cell tumors (99% +/- 2%, mean +/- SD) was higher than those in HCC (39% +/- 31%), carcinoma metastatic to the liver (76% +/- 27%), and benign liver diseases (4% +/- 7%); the differences were statistically significant (P < 0.001). The glucosaminylation index of AFP in germ cell tumor (45% +/- 20%) also was significantly higher than those in HCC (4% +/- 4%) and in benign liver diseases (1% +/- 4%) (P < 0.001). The difference in mean percentage of the glucosaminylation index, however, was not significant between germ cell tumors (45% +/- 20%) and carcinoma metastatic to the liver (39% +/- 23%). CONCLUSIONS: The current study indicates that the most characteristic feature of glycosylation of AFP in germ cell tumors is fucosylation rather than glucosaminylation.

Heterotopic autotransplantation of the distal pancreas segment after total pancreatectomy for cancer of the head of the pancreas.

Autotransplantation of the distal pancreas segment with pancreaticojejunostomy was performed in four patients with cancer of the head of the pancreas to preserve endocrine pancreatic function after extended total pancreatectomy. All patients had tumor involvement of both the celiac axis and the portal vein. The pancreatic graft was determined to be cancer-free by frozen section histologic and pancreatic juice cytologic examinations. The distal pancreas segment was autotransplanted to the iliac vessels heterotopically and placed in the extraperitoneal pocket to avoid untoward effects of any local recurrence or pancreatic leakage. This procedure, in the form of reconstruction, might be called modified subtotal pancreatoduodenectomy. Postoperatively, all patients remained normoglycemic without exogenous insulin administration, and their quality of life was considered satisfactory.

[Antitumor responses after reinfusion of autologous plasma perfused over immobilized protein A: a study in patients at advanced stages of cancer].

Four patients at advanced stages of cancer (Pts.#1 and #4; breast adenocarcinoma, Pt.#2; rectal adenocarcinoma, and Pt.#3; adenocarcinoma possibly originated in the pancreas) received reinfusions of plasma perfused over protein A columns (IMURé Corporation; Seattle, Wa., U.S.A.). The treatment schedule was essentially the same as was reported by "the Protein A Clinical Trial Group" (Messerschmidt, G. L., et al.). Twice a week, patients received 230-250 ml of protein A-treated plasma. No other anticancer modality was employed. Pt.#1 received only one treatment due to increased general malaise. Pts.#2 and #4 completed 12 and Pt.#3 received 13 plasma-therapies. General malaise on the day after plasma return was a common untoward effect, which usually disappeared spontaneously in 48 hours. Pt.#2 experienced chills during the first two plasma returns. Pt.#3 complained dyspnea at the 6th treatment. Otherwise, no serious adverse effect was observed. No remarkable changes were obvious in the profile of daily blood chemistry. Tumor ulcerated to the chest wall in Pt.#1 appeared to be dried and, in parts, to be edematous or have reduced the size 16 hours after the first plasma therapy, while no changes were obvious in the skin adjacent to the tumor. In Pts.#2 and #4, there appeared to be tumors were isolated at palpation. Ascites was reduced in Pt.#3. Increases in tumor markers were retarded in Pts.#2 (CEA) and #4 (CA15-3). CA19-9 in Pts.#3 and #4 did not increase during the perfusion therapy. Histology of tumor obtained from Pt.#4 six weeks after completion of the protocol revealed disruption of the tumor structure, necrosis of the tumor cells and increased deposit of fibrous materials. No objective reduction in the tumor sizes was achieved in any of the patients, and the overall result was "PD". It appeared to be full of suggestions, however, that the autologous plasma in such amounts of 230 ml or 3,000 ml did, after extra-corporeal manipulation, cause some changes in the tumor as were revealed by macroscopic and microscopic observations.

Primary structure of a base non-specific and adenylic acid preferential ribonuclease from Aspergillus saitoi.

The complete primary structure of a base non-specific and adenylic acid preferential RNase (RNase M) from Aspergillus saitoi was determined. The sequence was determined by analysis of the peptides generated by digestion of heat-denatured RNase M with lysylendopeptidase, and the peptides generated from RCM RNase M by digestion with staphylococcal V8 protease or chemical cleavage with BrCN. It consisted of 238 amino acid residues and carbohydrate moiety attached to the 74th asparagine residue. The molecular weight of the protein moiety deduced from the sequence was 26,596. The locations of 10 half cystine residues are almost superimposable on those of RNase Rh from Rhizopus niveus and RNase T2 from Aspergillus oryzae which have similar base specificity. The homology between RNase M and RNase Rh and RNase T2 amounted to 97 and 160 amino acid residues, respectively. The amino acid sequences conserved in the three RNases are concentrated around the three histidine residues, which are supposed to form part of the active sites of these RNases.

[A trial of mini-splint therapy for internal derangement of TMJ--application for cases without reduction by manipulation technique].

The mini-splint was applied to 21 patients with internal derangement of the temporomandibular joint (TMJ) who had failed in treatment by the manipulation technique. This appliance was constructed of two splints, which were built on the bilateral upper first bicuspids (teeth number 14 & 24, FDI) by methyl-meta-acrylate. As a result, the mean of the maximum interincisal distance was improved from 25.0 mm before to 41.0 mm after splint therapy. Among 21 patients, 18 patients were improved in interincisal opening, 12 patients were free from TMJ pain, and 3 patients were relieved from joint noise. Our studies indicate that this technique is useful as one of the treatment methods of TMJ internal derangement.

[Therapeutic embolization of infantile hemangioendothelioma of the liver].

A distinct syndrome in infantile hemangioendothelioma consist of high-output congestive heart failure, hepatomegaly and cutaneous hemangiomas. Early therapy aimed at reduction of the shunt is essential if life is to be saved once cardiac failure becomes manifest. We report a case of infantile hemangioendothelioma with massive A-V and P-V shunt successfully treated by transcatheter embolization.

Age Factors

[Multidisciplinary treatment of cancer of the pancreas].

During the past 18 and a half years, 285 patients with cancer of the pancreas have been treated in our hospital. By means of en-bloc resection of the lesion with the portal vein and/or hepatic artery, the resectable rate of cancer of the head of the pancreas was improved to 53% (1979-1983) from 22% (1966-1978). Postoperative deaths within 30 days after pancreatectomy were 9.8%. The five years survival rate in those with Stage I and II was 28.7%, but that with Stage III and IV was 15.2%. Intraoperative and/or external radiation therapy was adopted on 16 patients, in some of whom hyperthermia was further combined. As a result, remarkable effects on performance status were obtained. Intra-arterial infusion chemotherapy using a transfemorally implanted catheter was performed on ten patients with unresectable lesions, the median survival of whom was however, only four and a half months.
