[Relationship between Malassezia Yeast and Infantile Seborrhoeic Dermatitis].

We examined 52 patients with infantile seborrhoeic dermatitis (ISD) and 47 healthy 1-month-old infants. Yeast cells on the right side of the face were counted by direct microscopic examination, and isolates from the left side of the face were identified by Tween test. Yeast cells were more numerous patients with ISD than in the healthy infants. M. furfur and M. globosa were isolated from ISD patients at significantly higher rates than from healthy infants.

Identification of Malassezia species isolated from patients with seborrhoeic dermatitis, atopic dermatitis, pityriasis versicolor and normal subjects.

We identified Malassezia species isolated from 42 patients with seborrhoeic dermatitis, 17 patients with atopic dermatitis, 22 patients with pityriasis versicolor, 35 normal subjects and 73 healthy medical students. Regarding the prevalence of Malassezia species in the 35 normal subjects, the frequency of isolation of Malassezia globosa was 22%, M. sympodialis 10% and M. furfur 3%. M. slooffiae, M. pachydermatis, M. restricta and M. obtusa were infrequently isolated from normal skin. Two different species were isolated coincidentally from seven samples. In the patients with atopic dermatitis, M. furfur was isolated more frequently from lesional skin (21%) than non-lesional skin (11%). However, there was no statistical significance. Therefore, this result, by itself, is insufficient to prove that M. furfur should be considered to be an exacerbating factor of atopic dermatitis. In seborrhoeic dermatitis, M. furfur (35%) and M. globosa (22%) were isolated from lesional skin on the face at significantly high rates in comparison with the normal subjects. Therefore, M. furfur and/or M. globosa may be pathogens of seborrhoeic dermatitis. M. globosa was isolated at a frequency of 55% from lesional skin of pityriasis versicolor, while all other species were below 10%. These data suggest that the pathogenic species of pityriasis versicolor is M. globosa.

A case of seborrhoeic blepharitis.

A fifty five-year old woman suffered from itching and scaling of the edge of her eyelid. She had long used topical corticosteroid for this condition. Direct examination of the scale by Parker KOH showed numerous fungal elements of spores and hyphae of Malassezia furfur. She was treated with oral itraconazole (100 mg daily or twice a week) for 8 weeks and was cured clinically and mycologically. The result suggests the possibility of treatment with an anti-fungal drug for seborrhoeic blepharitis or seborrhoeic dermatitis.

[Seborrhoeic dermatitis].

UNLABELLED: The cause of seborrhoeic dermatitis (SD) is still unknown, although many attempts have been made to relate it to infection by Malassezia. Recently Midgley, Gueho & Guillot revised the genus Malassezia to 7 species. We detected Malassezia spores by direct counting using cellotape stripped skin of healthy students or patients; we also cultured Malassezia from these subjects. Strains were classified as species according to Guillot's Tween test. RESULTS AND CONCLUSION: Male students had many more spores than females. Spore numbers were not correlated to sebum levels. M. globosa or M. furfur appeared to be the main strain associated with SD. Treatment with anti-fungal agents showed that the elimination of lesions paralleled the reduction innumber of Malassezia spores. In contrast to corticosteroid treatment, anti-fungal agents showed low recurrence rates. Anti-fungal agents thus have potential value as a new therapeutic agent -an alternative to corticosteroid- for the treatment of SD.

alpha-Tocopherol enhances the hypocholesterolemic action of sesamin in rats.

The effect of alpha-tocopherol on the hypocholesterolemic action of sesamin was examined in rats given a cholesterol-enriched diet. When different levels (0.05 and 0.2%) of sesamin were fed, the supplementation of 1% alpha-tocopherol significantly accentuated the hypocholesterolemic action of sesamin, particularly with the higher sesamin level, although alpha-tocopherol alone did not affect the concentration of serum cholesterol. The dose-dependent promoting effect of alpha-tocopherol on the hypocholesterolemic action of sesamin was confirmed by supplementing different levels (0.2 and 1%) of alpha-tocopherol to a fixed level of sesamin (0.2%). alpha-Tocopherol was still effective at the 0.2% level. The metabolism of sesamin in the liver S9 fraction appeared to be interfered with alpha-tocopherol in vitro, suggesting a possible role of alpha-tocopherol in maintenance of the availability of sesamin.

Seborrhoeic dermatitis: treatment with anti-mycotic agents.

In order to elucidate the effectiveness of anti-mycotics in treating seborrhoeic dermatitis, an attempt was made to isolate Malassezia from seborrhoeic lesions of patients of seborrhoeic dermatitis. The results revealed that, in male patients, 46/49 cases were positive for Malassezia furfur on the face and 30/48 cases were positive for M. furfur on the scalp. In female patients, 7/13 cases were positive for M. furfur on the face, and 6/17 cases were positive for M. furfur on the scalp. Anti-mycotic agents were excellent in 50% and good in 31% of the spore-positive cases, yielding an overall efficacy rate of 81%. In contrast, the treatment of the face with vehicle alone showed only one excellent result out of 8 cases. Although clinical improvement was rapid on the side treated with a topical corticosteroid in the half-side-test, numerous fungal elements remained. While the improvement with anti-mycotic agents was slower than that with the corticosteroid, clinical improvement became evident by the third week of administration and fungal elements disappeared.

Bullous pemphigoid complicated with pemphigus vulgaris?

A 67-year-old housewife had polymorphous eruptions with tense bullae on the extremities and upper trunk. Erosions were noted on the oral and esophageal mucosa. A skin biopsy showed subepidermal blistering with linear deposition of IgG and C3 at the basement membrane zone. In addition, intercellular deposition was observed at the lower part of the epidermis. On the basis of clinical and histological features, a diagnosis of bullous pemphigoid (BP) was made. After she was treated with prednisolone, the whole-skin lesions soon disappeared. An esophageal biopsy showed intercellular deposition of IgG and C3. Indirect immunofluorescence testing using esophageal mucosa of guinea pigs showed the same deposition at the titer of 1:20. Immunoblottings with this patient's serum revealed no reactions with either BP or pemphigus antigens. Her serum reacted strongly with a 190-kD protein, the nature of which was unknown. A treatment with ciclosporin was added to this steroid therapy, and the mucosal lesions improved gradually.

An acoustically investigated case of phonetic change in higher vertical dimension.

This report presents the phonetic changes of a patient whose occlusal vertical dimension is extended. His chief complaint is esthetic restoration of his anterior teeth, but his occlusal vertical dimension is abnormally shortened by long-time attrition. Therefore we extended his occlusal vertical dimension, and reconstructed his occlusion, and carried out acoustic observations.

Fibronectin on prosthetic denture fabrication.

It is well known that fibronectin adheres to various oral bacteria and shows affinity to various components. Thus the existence fibronectin which is present in the salivary flow, acquired pellicle and dental plaque, was investigated in used acrylic resin dentures with a direct immunohistochemical fluorescent technique using antihuman fibronectin goat serum which was labeled with fluorescein isothiocyanate (FITC). It was found that fibronectin frequently was present on the surface of acrylic resin dentures.

Preparation of the bifidus milk powder.

Malt extract and skim milk were used as the culture medium for bifidobacteria. Bifidobacterium longum E194b (ATCC 15707) selected from among four strains grew well in a nitrogen atmosphere regardless of the initial dissolved oxygen level of the culture medium. The optimum temperature range for growth was 34 to 37 degrees C. The viable cell numbers of Bifidobacterium in freeze-dried powder from neutralized bifidus milk were 20 times those found in powder from nonneutralized milk after storage for 30 d at 35 degrees C. The optimum residual water content in the powder for bacterial stability was about 3.5% or less. Bifidus milk powder prepared under these optimum conditions contained at least one billion viable cells even after storage for 30 d at 35 degrees C. The powder must be sealed with material made of aluminium for storage.
