[Evaluation of quality of life in arterial infusion chemotherapy of hepatocellular carcinoma].

We examined the quality of life in the arterial infusion chemotherapy of hepatocellular carcinoma patients using a questionnaire. The questionnaire used a category scale method of five grades. The questions about the quality of life covered ten areas for investigation (appetite, discomfort pain, nausea, daily activities, sleep, fatigue, time with family and friends, thinking about illness and confidence in the treatment). We added up scale points after one week and those after two weeks after the treatment. Patients after one-shot infusion showed aggravated scale points of anorexia and discomfort. Patients after transcatheter arterial embolization showed scale points of abdominal pain, general fatigue and discouragement about illness. Scale points in matters of thinking about illness and confidence in the treatment informed us about confidence in the course of treatment and comprehension of illness by cancer patients. How do we measure the quality of our care? This is difficult, but we thought the rate of being at home in survival might furnish us with much information in respect to the treatment and the quality of our care. In 36 patients with hepatocellular carcinoma treated with transcatheter arterial infusion and embolization, the arithmetic mean survival time after treatment was 412.1 days and time at home was 305.6 days. The rate of being at home doing survival time was 74.2% after the arterial infusion chemotherapy in 39 patients. The rate of being at home in 9 cases with one-shot infusion of Adriamycin was 43.5% (111 days); that in 9 cases with infusion of Mitomycin C microcapsules was 86.6% (716 days); that in 17 cases with transcatheter arterial embolization using spongel was 72.0% (234 days),; and that in 4 cases with infusion using implantable reservoir was 84.6% (220 days). In non-resected patients with chemotherapy, the rate of being at home was 20.3% for 61 cases of gastric cancer patients, 30.7% for 11 cases of colon cancer, 9.6% for 14 cases of gallbladder cancer and 39.8% for 112 cases of lung cancer. The arterial infusion and embolization of hepatocellular carcinoma has made it possible to lengthen the time that patients may stay home and thereby assure good quality of life.

[Plasma kinetic study following oral administration of leucovorin tablets in patients with gastric cancer--influence of subtotal gastrectomy].

Patients with advanced gastric cancer were treated with methotrexate-5-fluorouracil sequential therapy. In order to rescue of methotrexate toxicity on the normal proliferating tissue, leucovorin was administered orally to outpatients. As the comparative study, we examined plasma kinetics following oral administration of leucovorin (15 mg) in 6 patients with gastric cancer (received subtotal gastrectomy) and 5 normal subjects. In the patients, peak plasma concentrations of d-1-formyltetrahydrofolate and its main metabolite 5-methyltetrahydrofolate were found after 2 hrs. (7.35 X 10(-7) M, 3.09 X 10(-7) M), and in normal controls also both after 2 hrs. (1.01 X 10(-6) M, 4.89 X 10(-7) M). The active folate metabolite persisted for more than 6 hrs. at over 1 X 10(-7) M that could rescue normal tissue from methotrexate toxicity after leucovorin oral administration in the patients received subtotal gastrectomy.

A phase II study of vindesine in the treatment of esophageal carcinoma. Japanese Esophageal Oncology Group.

A phase II study of Vindesine for esophageal carcinoma was carried out cooperatively by 10 Japanese institutions. Fifty patients were enrolled in the study over a year and ten months. Four patients could not be evaluated because less than 4 weeks had elapsed since their preceding therapy. There were one complete remission and five partial remissions among the 46 remaining patients, a response rate of 13.0%. The patient experiencing the complete remission is still alive more than 24 months after treatment. The main side effects were depilation, anorexia, stomatitis and leukopenia. Thrombocytopenia and elevations of blood urea nitrogen and creatinine were not marked. There was no mortality connected with the administration of Vindesine. It was concluded that Vindesine is useful in the treatment of esophageal carcinoma as a single agent.

[Arterial infusion cancer chemotherapy in liver tumor using implantable reservoir].

Four primary liver tumors and 19 secondary ones (8 gastric, 8 colorectal, 2 esophageal and 1 gall bladder cancer) were treated by intra-arterial infusion chemotherapy intermittently using implantable reservoir. Drugs used in this series were 5-Fluorouracil, Mitomycin C, Adriamycin, and Cis-platinum. They were infused every 2 weeks for outpatients. The antitumor efficacy was evaluated in terms of tumor regression measured by CT scan. The toxicity was slight and temporary. The total response rate was 23.1%, 50% survival period was 7 months, and 1-year survival rate was 24.2%. The response rate was 33.3%, and the 50% survival period was 7 months for 8 gastric cancer patients with liver metastasis. The result was not good, and we must improve the criteria of indication and devise a regimen. But the implantable drug delivery system has made it possible to lengthen the time that patients may stay home and assure good quality of life because of the freedom of movement, normal physical appearance, protection from infection, and reduction of mental burden, due to subcutaneous placement.

[Clinicopathological study of early gastric cancer--indication of conservative surgery and radical endoscopic treatment of early gastric cancer].

Clinicopathological factors such as depth of cancer invasion, size, gross type, frequency of metastases to regional lymph nodes, and distant prognosis were evaluated in last consecutive 339 cases with solitary early gastric cancer. The conservative surgery, that is, subtotal gastrectomy with complete dissection of lymph nodes of group 1 and selective celiac group and partial bursectomy, would be indicated for early gastric cancers located in antrum or corpus. But if the metastasis to the group 2 lymph nodes is suspected during the surgery, it is necessary to dissect lymph nodes en bloc more than group 2. The results, concerning the type of early gastric cancer without lymph node metastasis and the indication of endoscopic treatment, were as follows; 1. Intramucosal cancer of elevated type less than 2 cm in diameter. 2. Intramucosal cancer of depressed type less than 1cm in diameter, without peptic ulcer within the lesion, and a differentiated tubular adenocarcinoma histologically. 3. Intramucosal cancer of flat type less than 2 cm in diameter. But it is difficult to detect the depth of cancer invasion and lymph node metastasis preoperatively. We would emphasize that endoscopic treatment should be indicated in the case for which surgical treatment is not indicated.

[The significance of endoscopic treatment of early gastric cancer].

Gastric endoscopy could be an useful procedure in treating patients with early gastric cancer in certain conditions. We treated 24 patients with gastric cancer for whom no surgery was indicated because of serious complications or patient's refusal. Endoscopic local injection (ELI) with Mitomycin C (MMC), 5-fluorouracil (5-FU), and Picibanil (OK-432) was performed in 16 patients and polypectomy in 8 patients as radical treatment for primary gastric cancer. Of the 16 cases, ELI was effective in 9 cases of early gastric cancers, consisting of 4 cases with complete response (CR), additional 4 with a partial response (PR), but was ineffective in the remaining of all 3 cases of advanced cancers. ELI with MMC + 5FU was performed in 8 cases with an excellent response; 3 cases with CR and 3 cases with PR; 5 patients are alive, with the longest survival period of 54 months, and 2 patients died of other diseases. On the other hand, ELI with OK-432 was carried out in remaining 8 cases with no effective results, showing only 1 case with CR and another case with PR. Polypectomy was effective for 8 protruding type of early gastric cancers with the longest cancer-free period of 31 months. At present, although preoperative evaluation of the depth and range of cancer invasion is difficult, we would like to emphasize that ELI and polypectomy are procedures worthy to be attempted in patients with early gastric cancers with no nodal metastases for surgery is not indicated.

Duplication of the stomach with a triple antrum appearance.

We report a rare case of duplication of the stomach concomitant with idiopathic portal hypertension. The woman underwent a gastrectomy, under a tentative diagnosis of gastric carcinoma. The resected specimen revealed a submucosal type duplication of the stomach located on the greater curvature. The lesion had three communications with the true stomach and had the gross appearance of a "triple antrum".

[clinicopathological study on gastric carcinoma following preoperative anticancer chemotherapy].

Clinicopathological study was undertaken on gastric carcinoma, 130 cases and 145 lesions, treated with preoperative anticancer chemotherapy, using Oboshi-Shimosato's criteria for histologic grading of effects. The results were as follows: Not only morphologic manifestations observed by X-ray and endoscopy, but also histopathologic findings of biopsied specimens taken after treatment with 5-FU for 12 consecutive days, were effectively used for judging the sensitivity of cancerous tissue to anticancer agents in vivo. Effective rates were high in cases treated with oral administration of 5-FU more than 500 mg/day and 5.0 gr as a total dose (p less than 0.01), and the incidence of anticancer-induced-gastritis was also seen high in cases with oral administration of 5-FU more than 500 mg/day and 6.0 gr as a total dose (p less than 0.005). The proper dosage of 5-FU orally administered was, therefore, 500 mg/day and 5.0 gr. as a total dose. Histologic effects of anticancer agents on primary lesions were in accordance with those of metastatic foci of lymph nodes (p less than 0.01). 5-FU when orally administered resulted in a low rate of significant toxicity (23.5%), and no serious side-effects. In conclusion, it is considered that preoperative anticancer chemotherapy on gastric carcinoma apparently causes cancer cell death, decrease viability of cancer cells, makes it possible to detect in-vivo sensitivity for anticancer agents, and will improve the late survivals of patients with gastric carcinoma.
