Effects of dietary sucrose option on the diabetic syndrome of the eSS rat.

The eSS rat is a model of human spontaneous non-insulin-dependent diabetes. Male eSS rats were divided at the age of 4 months into two groups (eSSA and eSSB), both receiving the usual commercial balanced diet with sucrose also made available to eSSA. Sucrose intake did not imply a higher caloric diet, and no differences were found between groups in body weight and plasma triglyceride levels from 6 to 12 months of age. Sucrose option resulted in lower protein, lipid and carbohydrate intakes in eSSA animals. Plasma glucose values were higher in eSSA at different times of the tolerance curve. Likewise, eSSA kidneys showed significantly higher capsular and glomerular diameters and there was a discrete PAS-positive thickening of their basement membrane. We conclude that prolonged ad libitum sucrose intake, without weight gain, causes a moderate metabolic impairment and renal lesions in the eSS diabetic rat.

[Relationship of diet, biomass and expression of diabetes in eSS rats].

The long-term effect of feeding eSS rats three commercial diets on the development of diabetes and its complications has been investigated. These diets differ in their proportions of carbohydrates, fibres, lipids and proteins: diet A is rich in lipids, B in carbohydrates and fibres and C in proteins. However, the proportions of these components lie within the range recommended for rats. Animals receiving diet C showed the highest growth rate and were the first to develop diabetes at the age of four months. They had, moreover, the highest levels of triglycerides, total cholesterol and HDL-cholesterol. Animals fed the A diet were heavier than the other groups at 13 months of age, showing a diabetic glucose tolerance test and the highest values of circulating insulin. They were already diabetic when tested at the age of 6 months. The group fed the B diet remained leaner than the other groups and free of diabetes up to the test performed when they were ten months old. The findings at the age of 23 months were: the A animals developed the largest retroperitoneal and epididymal adipose tissue masses, the C group was the most affected by cataracts which were total, and bilateral in some cases, whereas the B rats were free of them and the A animals showed milder lesions than the C rats. Histological studies of pancreas and kidneys demonstrated that the C animals had fewer Langerhans islands than the other groups and the most severe renal lesions while the B animals had no renal damage. It is concluded that diets leading to overweight, particularly those rich in proteins, make the diabetic syndrome worse in eSS rats.

[Angioimmunoblastic lymphadenopathy and colonic lymphoma].

Angioimmunoblastic lymphadenopathy (AILD), first discovered in the 70's, is an infrequent disease which generally proves fatal within a short time. It is characterized by lymphadenopathies, hepatosplenomegaly, fever and rash. The most frequent laboratory findings are: anemia, leukocytosis with lymphopenia and non specific hypergammaglobulinemia. In spite of being considered a non malignant disease, it produces important immunity disorders which predispose the patient to serious infections, frequently fatal. In the course of time patients are likely to develop malignant lymphomas or other types of tumors. We describe a ganglionar proliferation and general symptoms in a patient who had been diagnosed as AILD by ganglionar biopsy. She was treated with corticosteroids during 8 months after which she had a complete recovery. Three months later the patient was readmitted with enterrorhagias and clear deterioration of her general condition. Tests showed the existence of a colon tumor and absence of adenomegaly in the areas previously affected by AILD. A colon surgery was carried out and a specimen examined. The anatomopathologic examination confirmed the existence of an immunoblastic lymphoma infiltrating the regional ganglionar area. Colon lymphomas constitute only 4% of all colon tumors; they are related to chronic gastrointestinal disease such as Crohn disease, ulcerative colitis, malabsorption syndromes, tumors and others. We conclude that in this patient AILD and prednisone administration constituted favoring factors for the development of an extranodal lymphoma.

[Colonoscopy and "hot" biopsy. Its value in vascular ectasias of the colon].

According to Gerald Rogers et. al., we believe about the usefulness of colonoscopy and endoscopic biopsy using Williams' forcep (hot biopsy forcep) for the diagnosis and final treatment of colon vascular ectasias. This method should be used immediately after the hemorrhage has finished. Angiography is prescribed when: 1) endoscopy suggests or confirm the presence of vascular ectasias in order to establish the degree of lesions and the therapeutic management. 2) When the caecum cannot be reached by the colonoscope. 3) When endoscopy is negative and there is firm clinic assumption of ectasias but the colonoscopic study has been done to much later after the bleeding stopped. Selective angiography would be the first prescription in case of bleeding which doesn't stop quickly with the necessity of immediate therapeutic measures taking into account the difficulties of intrahemorrhagic colonoscopy. Five cases are presented to support this point of view concerning the diagnosis of vascular ectasias of the right colon and considerations are made about local treatment by mean of colonoscopy and hot biopsy forcep which, when it's possible, has less morbidity and mortality than surgery, specially in attention than this is an entity concerning elderly patients.

[Gastric leiomyoblastoma].

Three cases of gastric leiomyoblastomas are presented (according to Stout's denomination) or epithelioid leiomyomas (according to Appelman). The subject is actualized and the low malignant potential of these tumors is pointed out. Leiomyoblastomas have to be classified among the leiomyomas (absolutely benign) and the leiomyosarcomas (malign). The lack of reliable criteria to determine the future of the patient is established, being the evolutive control the only way to make that clear. The mitotic counts--more than 5 for each 50 high-power dry objective field--is the most important isolated criterion to predict the probable malignancy of the lesion.
