Head and neck carcinoma in Fanconi's anaemia--report of a case and review of the literature.

Fanconi's anaemia (FA) is a rare autosomal recessive syndrome characterised by progressive lethal pancytopenia, skeletal abnormalities, hyperpigmentation and increased chromosomal aberrations. A high incidence of leukaemia and hepatocellular and squamous cell carcinomas (SCC) have been reported in FA patients. A rare case of SCC of the dorsum of the tongue in a FA patient is presented. A review of the reported cases of head and neck carcinoma in FA patients shows a different male:female ratio than previously reported, and a high incidence of carcinoma of the tongue.

Bipedicled myomucosal flap for reconstruction of the lip after vermillionectomy.

Solar keratosis is a well-documented premalignant condition caused by actinic radiation on the vermillion. The usual treatment method consists of surgical removal of the damaged area by means of sharp dissection, electrosurgery, chemical peeling, or recently CO2 laser beam. The denuded area is left to reepithelize (in cases of superficial removal) or is reconstructed by advancing mucosal flaps. Most of these methods assure a satisfactory clinical cure and a good function. The main disadvantage is esthetic because of a narrowing and flattening of the vermillion and loss of the pout. To overcome these side effects, we use a bipedicled vascularized myomucosal axial flap. Between 1982 and 1989, 19 patients were treated with this method. In this article we present the clinical aspects of solar keratosis, describe the surgical technique step-by-step, and evaluate the long-term outcome of this method.

Salivary gland endoscopy: a new technique for diagnosis and treatment of sialolithiasis.

PURPOSE: To describe the use of endoscopy as a technique for diagnosing and treating sialolithiasis. Sialolithiasis is a common pathology of the major salivary glands, yet its accurate detection and treatment is still intriguing. PATIENTS AND METHODS: Eight cases of sialolithiasis were treated with our new technique which combines endoscopy and treatment. Two illustrative patients treated by this technique are described in detail. CONCLUSION: Endoscopy, has gained popularity in most surgical fields. We found that the available standard equipment used for TMJ arthroscopy can be used for exploration and extraction of calculi from the salivary ductal system.

Gerodermia osteodysplastica: report on two patients and surgical correction of facial deformity.

We present four individuals with Gerodermia Osteodysplastica in a Jewish family from Morocco confirming the autosomal recessive inheritance of the disorder. Three previously unreported findings are described: a) enlarged funnel-shaped mandibular lingula; b) extension of the mandibular premolar and molar roots below the inferior dental canal, and of the second molars into the lower border of mandibular cortical bone; and c) hypercementosis of the maxillary incisors and mandibular molars surrounded by a radiolucent halo in several teeth. The facial deformity resulting from maxillary hypoplasia and mandibular prognathism was corrected by orthognathic surgery: Le Fort I maxillary osteotomy and vertical mandibular osteotomy.

Fractures of the mandible caused by stoning--return of an ancient entity.

Fifteen patients with mandibular fractures related to stone-throwing incidents are presented. Three illustrative cases are discussed in detail. Criteria for selecting management by early surgical intervention or a conservative approach are discussed. The ultimate aim in management is to preserve the greatest possible amount of function with the least risk and morbidity for the patients.

CO 2 laser used in surgical treatment of actinic cheilitis.

This study discusses the satisfactory CO 2 laser treatment of solar keratosis on Eastern European emigrants to Israel. It offers a comparison between cold blade vermilionectomy based on previous data and experience and reports postoperative results concerning cosmetic appearance and function of the lip.

Iatrogenic pneumomediastinum after endodontic therapy.

An unusual case of pneumomediastinum caused by subcutaneous emphysema occurring after the use of hydrogen peroxide solution during root canal treatment is described in a 30-year-old man. A thorough search of the available literature failed to reveal similar cases. A review of the literature, including pathogenesis, clinical and radiographic features, and management of this condition, is presented.

Burns of the oral mucosa. Report of 6 cases.

Burns of the oral cavity may be caused by prolonged use of certain drugs by the patient or by incorrect use of caustics by the dentist. Unwillingly-acquired self-inflicted injuries are also encountered, such as caustic ingestion (out of curiosity or by accident), excessive consumption of fresh fruit and fresh fruit juice, and wrong oral hygiene practice. Our experience with six infrequent and unfamiliar types of oral burns, caused by various components and material, is described and discussed.

Leukocytoclastic vasculitis (anaphylactoid purpura): a unique occurrence in the oral cavity.

Leukocytoclastic vasculitis, immune complex disorder (type III), is a skin disease with both an acute form characterized by bullae, vesicles and ulcerations, and a chronic form characterized by petechiae, macules and ulcerations. The disease presents certain systemic features including diffuse or focal glomerulonephritis and renal failure. The histopathologic characteristics of leukocytoclastic vasculitis in the skin appear primarily in small blood vessels and consist of an infiltration of inflammatory cells, leukoclasis, swelling of endothelial cells, occlusion of blood vessels, accumulation of fibrin and fibrinoid degeneration, as well as the presence of immune complexes in and around blood vessel walls. Although leukocytoclastic vasculitis is described as several diseases which can spread systemically, including the gastrointestinal tract and the kidneys, the manifestations of the disease in the oral cavity have not yet been reported. The present paper reports unique oral lesions in a 38-yr-old woman, diagnosed as leukocytoclastic vasculitis, without any accompanying skin or systemic lesions.

[Melkersson-Rosenthal syndrome].

Melkersson-Rosenthal syndrome consists of facial paralysis, facial edema, and scrotal tongue (lingua plicata). Granulomatous cheilitis, frequently associated with the name of Meischner, is generally accepted as a monosymptomatic form of the same syndrome, affects both sexes equally. The earliest manifestations usually develop in childhood or adolescence. We report a case in a 20-year-old woman in order to increase awareness of this entity, in both its classical and aberrant forms.

A comparative trial of depot pipothiazine.

Thirty-nine chronic schizophrenic patients were selected for a 12-month double-blind evaluation of the effectiveness of pipothiazine palmitate (PPT) and flupenthixol decanoate (FPX) in the maintenance management of their illness. Allocation was at random and, in order to allow constant injection intervals, the patients typically received every 2 weeks either 40 mg of flupenthixol decanoate or alternating injections of 100 mg of pipothiazine palmitate and placebo. At monthly intervals the patients were assessed using both a battery of rating scales (which included the Brief Psychiatric Rating Scale (BPRS), the Extrapyramidal Symptoms Rating Scale (EPS] and a general side-effects evaluation. At 3-monthly intervals they were also rated on the Comprehensive Psychiatric Rating Scale (CPRS) and the Zung Depression Scale. Haematological and biochemical tests were performed every 3 months. Both drugs provided good control of psychotic symptoms and side-effects were not troublesome. No substantial difference was detected on the CPRS and the Zung scales. There was a trend in favour of PPT on the BPRS survey, detectable at 6 months and reaching statistical significance by 12 months. We conclude that the PPT regime is at least as effective as the FPX treatment and probably more so. It is possible that even longer periods of control could be obtained with PPT.
