Our experience in the diagnostics of liver and spleen hemangiomas.

We studied the potentials of conventional imaging methods (sonography, computed tomography) and invasive imaging methods (digital subtraction angiography, fine needle biopsy under sonographic guidance) in diagnosing liver and spleen hemangiomas. 384 patients underwent ultrasound investigation for suspected focal lesions of liver and spleen. Hemangioma was diagnosed in 83 patients. Computed tomography with contrast enhancement was performed on 62 of these 83 patients. The diagnosis of hemangioma was confirmed in 38 (61.3%) patients. In 22 patients the combination of sonography and computed tomography failed to provide indubitable diagnosis. Digital subtraction angiography was performed in 11 of them; the remaining 11 patients underwent fine needle biopsy under sonographic guidance. The authors achieve precision in the diagnosis of liver and spleen hemangiomas by employing combinations of above mentioned imaging methods.

Our experience in the imaging diagnostics of abdominal aortic aneurysms.

The present study focuses on the use of imaging methods for diagnosing aneurysms of the abdominal aorta. We performed ultrasound tomography, computed tomography and digital subtraction angiography in 42 patients (17 men, 45 women, aged 25 to 64 years) with clinically suspected aneurysm of the abdominal aorta. The results obtained are discussed determining the proper sequence in using these imaging methods in the overall diagnostic approach. The high percentage of true positive results (100%) achieved in digital subtraction angiography makes it the leading diagnostic method. It is recommended that ultrasound and computed tomography examinations should precede the digital subtraction angiography.

An algorithm of diagnostic-therapeutic invasive sonography.

The present study describes an algorithm of diagnostic-therapeutic invasive sonography that was used in 276 patients. 276 manipulations using ultrasound guidance were performed on 371 objects. These include therapeutic punctures (evacuations and drug applications), percutaneous nephrostomies and dilatational minilaparatomy with drainage system. We achieved complete therapeutic success in 77.5%, partial therapeutic success in 14.1% and failures only in 8.4%. These satisfactory results indicate that the developed algorithm makes possible the individual approach to the patient, allows correct choice of technique, method, and strategy; it helps to achieve the therapeutic target as well to include a diagnostic element at a certain stage of the therapeutic process.

Algorithm of diagnostic invasive sonography.

An algorithm developed for diagnostic invasive sonography is presented. It was used in examining 350 patients. 385 manipulations were performed under ultrasound control in relation to 371 pathologic objects. A technical efficiency ranging from 97 to 100% was achieved for the various manipulations. This algorithm helps in finding the specific approach to each case on the basis of clinical data available, the characteristic of the pathologic object, and its localisation by organs and regions thus avoiding any risks and complications and achieving the planned target for the chosen manipulation.

The role of ultrasound tomography in diagnosing adrenal pathology.

The present study included 97 patients (52 female and 45 male) aged 16-52 years. Conventional ultrasound examination of adrenal glands was made in all cases using a Sonoline SL-2 ultrasound tomograph. The diagnostic results from the examination were compared to the results from the hormonal studies, scintigraphy, computer tomography, angiography, surgery and the histological diagnosis. The diagnostic parameters of the sonographic method were assessed on the basis of the results obtained. The role of the ultrasound tomography in the diagnostics of different adrenal diseases was determined.

[Fine-needle puncture and aspiration of pathologic lesions in the abdomen using ultrasound guidance].

The aim of this study was the echo-guided fine-needle aspiration puncture of abdominal lesions to be evaluated. 91 punctures of 85 patients with local lesions were performed. In 6 cases the puncture was repeated because of the lack of material for cytologic study just blood presence in the needle. In 47 cases with carcinoma or metastatic masses the puncture allowed aspiration of malignant cells. In 7 cases the puncture was therapeutic aspiration of pus collection and antibiotical application. The accuracy of this method was 92.9%, sensitivity 87.8% and specificity 96.1%. No significant complication to report occurred.
