[Auer rods-positive neutrophils observed at diagnosis increased after remission induction in patient with acute promyelocytic leukemia].

50-year-old male was admitted to our hospital because of gingival bleeding and fever in August 1987. The leukocyte count was 13,300/microliters with 80.5% leukemic promyelocytes and bone marrow was hypercellular with 86.4% leukemic promyelocytes. A small number of mature neutrophils containing Auer rods were seen in bone marrow. On a diagnosis of acute promyelocytic leukemia and treated with induction chemotherapy consisting of behenoyl-arabinofuranosyl cytosine (BHAC), daunorubicin, 6-mercaptopurine (6-MP) and prednisolone (PSL) was reformed. After cytoreduction, leukemic cells reappeared in the peripheral blood, concomitant with mature neutrophils having Auer rods. Vitamin D3 was not effective as a differentiation inducing agent. Complete remission was obtained in November 1987 by the reinduction chemotherapy consisting of BHAC, aclarubicin, 6-MP and PSL. In this case, neutrophils with Auer rods might have been derived from the leukemic clone and differentiation of leukemic promyelocytes by intensive chemotherapy.

Beneficial effects of a calcium antagonist, nifedipine, on death and cardiovascular calcinosis induced by dietary magnesium deficiency in adult mice.

The effects of nifedipine on the survival rate and the cardiovascular calcinosis were investigated in dietary magnesium (Mg)-deficient mice. Male mice aged 8 to 10 weeks were fed with a diet which was either normal (0.07%) or deficient (0.007%) in Mg for 30 d. Nifedipine was added to the Mg-deficient diet for 30 d in a concentration of 1000 ppm. About 65% of Mg-deficient mice died within 30 d. In contrast, all the nifedipine-treated mice survived for 30 d. Mg-deficient mice showed hypomagnesaemia and hypocalcaemia which were not influenced by nifedipine treatment. Aortic and cardiac calcium contents increased in Mg-deficient mice, and these increments were normalized by nifedipine treatment. These results suggest that the life-saving effect of nifedipine in Mg-deficient mice might relate, at least in part, to the prevention of a deleterious increase in Ca in cardiovascular tissues.

[Rapid disappearance of M-protein in multiple myeloma complicated by pneumonia during treatment with HLBI].

A 64-year-old female was admitted for treatment of refractory myeloma (IgG-lambda). Because of severe liver cirrhosis, the patient was treated with interferon (IFN) alone (natural IFN 6 x 10(6) IR/day i.m. for 28 days). Pneumonia developed during IFN therapy. The IFN therapy was completed, restoring suppressed IgM and IgA to their normal ranges, while pneumonia was cured by antibiotics. Because the M-component remained, an additional IFN therapy was resumed and M-protein disappeared. As the period within which the M-component disappeared in this case was shorter than that reported previously, we supposed that the pneumonia might have enhance the effects of IFN. To verify this, we administered OK-432 for 4 weeks as a model of immunoactivation by pneumonia between two successive courses of IFN therapy when M-protein reappeared. To monitor the immune state, natural killer (NK) and lymphokine activated killer (LAK) activities were measured: NK activity suppressed by IFN was restored by OK-432 and was suppressed less by subsequent IFN administration. LAK activity was increased by IFN and OK-432. These observations suggested synergic effects of OK-432 on IFN-activated immunity. This case suggests that IFN combined with immunotherapy may be effective in some cases of myeloma.

Anterior communicating artery aneurysm in early childhood. Report of a case.

A case of an aneurysm of the anterior communicating artery in a 13-month-old child is presented. Sixty-six cases of cerebral saccular aneurysm in children under the age of 2 years found in the literature were analyzed. The characteristics that define this group of patients are a high frequency of large or giant aneurysms, a large proportion of aneurysms of the middle cerebral artery and the posterior circulation, and the frequent location at the peripheral site. Only three cases of anterior communicating artery aneurysm were reported in the literature. The authors discuss these characteristics on the basis of development of fetal cerebral vessels.

Vasomotor effect of histamine on pig and cattle coronary artery in vitro.

Vasomotor effects of histamine were examined in isolated coronary arteries from pigs and cattle. Histamine produced a concentration-dependent contraction in these arteries. These contractile responses were dose-dependently inhibited by diphenhydramine. The slopes of the Schild plots, however, were significantly lower than unity in both species. Cimetidine potentiated the histamine-induced contractions at relatively high doses of histamine (larger than 10(-5) M) in pig coronary arteries, but did not show a significant effect in cattle arteries. After the removal of endothelium, the Schild plot of diphenhydramine against histamine gave a straight line with a pA2 value of 7.80 and slope of 1.00 in pigs, confirming the competitive nature of the antagonism. In cattle, the slope was significantly lower than unity; however, in the presence of cimetidine, it was not significantly different from unity. Dimaprit did not contract the cattle coronary arteries with endothelium, but contracted them after the removal of endothelium. These results suggest that histamine-induced vasoconstriction in pig and cattle coronary arteries is mainly dependent on the H1-receptors in the smooth muscle cells, and that H1- and H2-receptors in the endothelial cells of pigs and H2-receptors in the smooth muscle cells of cattle modify the histamine-induced vasoconstrictions.

Participation of H1-receptors in histamine-induced contraction and relaxation of horse coronary artery in vitro.

The mechanisms of histamine-induced contraction and relaxation were investigated in rings isolated from a middle part of the left descending coronary arteries of horses. Intact and endothelium-denuded preparations were compared. Rings of horse coronary arteries contracted in response to histamine in a concentration dependent manner, but some of them relaxed with lower concentrations and contracted with higher concentrations. Removal of the endothelium abolished the relaxation and potentiated the contraction. The pD2 values were 4.70 +/- 0.08 in the rings with intact endothelium and 4.95 +/- 0.08 in endothelium-denuded rings. Histamine-induced contractions in intact and denuded preparations were not affected by an H2-antagonist, cimetidine, but were inhibited by an H1-antagonist, diphenhydramine in non-competitive manner in the rings with endothelium and in competitive manner in denuded rings. After precontraction with PGF2 alpha or norepinephrine, histamine relaxed preparations with intact endothelium (pD2 value, 7.80 +/- 0.11), although histamine-induced relaxations were not observed in denuded preparations. The relaxation was competitively inhibited by diphenhydramine. Relaxing response was significantly attenuated by methylene blue, quinacrine, L-nitro-arginine, gossypol and AA861 but not by indomethacin. These results suggest that the histamine-induced contraction and relaxation in horse coronary arteries are mediated mainly by H1-receptors in the smooth muscle and endothelium, respectively, and H1-receptor activation of endothelial cells may liberate vasodilator substances.

Translocation between chromosomes 8q24 and 14q11 in T-cell acute lymphoblastic leukemia.

Cytogenetic study was performed on a patient with T-cell acute lymphoblastic leukemia (T-ALL). It revealed a chromosomal translocation between chromosome bands 8q24 and 14q11. 8q24 is known to encode the oncogene c-myc, while 14q11 encodes the genes for T-cell receptor-alpha (TCR-alpha) and T-cell receptor-delta (TCR-delta). Therefore, this chromosomal translocation t(8;14) (q24;q11) seemed to be unique and specific to T-cell malignancy.

Familial case of May-Hegglin anomaly associated with familial spastic paraplegia.

We report a family with May-Hegglin anomaly associated with familial spastic paraplegia. The propositus was a 39 year old male. His peripheral blood showed a Döhle-like inclusion bodies in WBC, giant platelets, and thrombocytopenia. He had been suffering from progressive gait disturbance of spastic paraplegia since 20 years old. He was in a state of chronic renal failure and showed sensory hearing impairment. His two children showed similar hematological abnormalities and spastic gait. As far as we know, this is the first case of May-Hegglin anomaly associated with familial spastic paraplegia in the literature.

Giant neutrophils derived from tetraploid leukemic clone in an acute myeloblastic leukemia: cytofluorometric study.

Near-tetraploid chromosomes were observed in a patient with acute myeloblastic leukemia with maturation (M2 in FAB classification). Large and morphologically bizarre leukemic cells and giant neutrophils in each maturation stage were observed in both peripheral blood and bone marrow. Cytogenetic studies revealed that the main stem line was 93; XXYY, +9, and DNA cytofluorometry showed that these large leukemic cells and giant neutrophils had 4C DNA content. These findings strongly suggested that these giant neutrophils were derived from leukemic clone with tetraploidy.

Subarachnoid hemorrhage and spinal root injury caused by acupuncture needle--case report.

The authors report a case of subarachnoid hemorrhage and spinal root injury caused by an acupuncture needle buried in the posterior neck about 30 years before onset. A 33-year-old female presented with sudden onset of severe occipital headaches. Plain x-ray films of the cervical spine revealed a fine gold needle, about 1.5 cm in length, between the C1 and C2 vertebrae. The needle was piercing the spinal nerve root through the dural vein, and was removed. Postoperatively, the pain exacerbated by neck movement disappeared.

[Renocolic fistula caused by xanthogranulomatous pyelonephritis: a case report].

Renocolic fistulas caused by typical xanthogranulomatous pyelonephritis are rare. We present a case of renocolic fistula caused by xanthogranulomatous pyelonephritis. A 58-year-old woman was admitted to the Department of Urology, Sakata City Hospital, with the chief complaint of pain in the left flank region. The plain film showed no calculi. An intravenous pyelogram showed left non-functioning kidney. A left retrograde pyelogram demonstrated a communication between the left renal calyx and the descending colon. A computed tomography done after the retrograde pyelography revealed contrast medium in the descending colon and a heterogenous left renal mass and thickness of Gerota's fascia. On the basis of the radiologic studies a diagnosis of renocolic fistula was made. The patient underwent a nephrectomy with closure of the fistula of the colon. The pelvicaliceal system of the resected kidney was distended and contained yellow fatty like tissue. Histologically xanthogranulomatous pyelonephritis was confirmed. The patient made an uneventful recovery.

Multiple types of voltage-dependent Ca channels in mammalian intestinal smooth muscle cells.

(1) Whole-cell and single channel recording techniques have been applied to smooth muscle cells isolated from guinea-pig taenia coli to examine whether multiple types of Ca channels exist. (2) Whole-cell recordings under physiological Ca concentration (1.8 mM) revealed two current components with fast and slow inactivating kinetics. The fast inactivating component was present when cells were held at very negative potentials (-80 mV). It was insensitive to the dihydropyridine (DHP) derivative, nifedipine. In contrast, the slow inactivating component was present at less negative holding potentials. It was blocked by nifedipine. (3) The two current components were found to have closely similar voltage dependencies for activation. (4) These results suggest that the fast inactivating decay of the Ca current was mediated not only by the entry of Ca into the cell but also by a voltage-dependent process via a different type of Ca channel with fast inactivating kinetics. (5) Recordings from cell-attached membrane patches with 100 mM external Ba clearly showed the existence of multiple types of Ca channels with different conductances. (6) The large conductance channels (30 pS) activated at more positive potentials (0 mV) and their averaged current decayed much more slowly. The DHP Ca antagonist, nifedipine, inhibited the large conductance channels increasing the proportion of blank sweeps and reducing the averaged current. On the other hand, the DHP Ca-agonist, BayK 8644, increased the average current by increasing the mean open-times of the large conductance channels. The presence of micromolar Cd in the patch pipettes produced a flickering block of the large conductance channels.(ABSTRACT TRUNCATED AT 250 WORDS)

Surgical treatment and radiological consideration of supergiant cerebral aneurysms.

We have collected four cases of supergiant cerebral aneurysm, each greater than 6.0 cm in diameter. Two of these cases were saccular aneurysms of the anterior communicating artery and middle cerebral artery, of which the necks were clipped with resection of the aneurysms. Another patient had a fusiform aneurysm of the middle cerebral artery, which was resected accompanied by a reconstructive procedure of cerebral blood flow. The last patient had a fusiform aneurysm of the internal carotid artery. Following internal carotid artery occlusion surgery with superficial temporal artery-middle cerebral artery anastomosis, the aneurysm completely disappeared radiologically within several months.

Lateral situated sinus pericranii. Case report.

We hereby report a case of a laterally situated sinus pericranii, which is rare. According to the articles regarding this kind of lesion, carotid angiograms revealed no pathological findings, and venograms taken with direct injection of contrast medium into the mass were useful. It should be stressed that the lesion could be demonstrated in the late phase of carotid angiograms, and digital subtraction angiography was especially useful for diagnosis. The literature is also reviewed.

[T cell receptor delta chain gene rearrangement in acute unclassified leukemia].

A 31 year-old male who was treated with radiation under the diagnosis of malignant lymphoma was admitted to our hospital because of systemic erythema and tumor of bilateral upper arms in October, 1987. Leucocyte count of peripheral blood showed 4,400/microliters with 36% leukemic cells and bone marrow was hypercellular with 85.6% leukemic cells. Leukemic cells were negative for peroxidase reaction and lineage specific monoclonal antibodies such as CD3, CD4, CD8, CD10, CD19 and CD20. T cell receptor (TCR) delta gene was rearranged but TCR beta, TCR gamma and immunoglobulin (Ig) genes were in germline configuration. He was treated with combination regimen of doxorubicin, vindesine, prednisolone and L-asparaginase, and complete remission was obtained. These observations suggest that TCR delta gene rearrangement is useful for determination of clonality in cases without rearrangements of the other TCR and Ig genes.

Enhanced contractile response to noradrenaline and calcium influx in thoracic aorta isolated from dietary magnesium deficient rats.

Dependency on extracellular calcium influx in contractile response to noradrenaline has been investigated in the thoracic aorta isolated from dietary magnesium (Mg) deficient rats. Adult male Wistar rats were fed with a Mg deficient diet (0.001% Mg) for 30 days, together with control groups (0.07% Mg). The contractile response to noradrenaline in the thoracic aortas from Mg deficient rats was significantly greater than that in the controls. However, there were no significant differences between control and Mg deficient rats in the contractile response to high potassium. The rate of 45Ca uptake induced by noradrenaline was greater in Mg deficient rat aortas than in the controls, but that induced by high potassium showed no significant differences between control and Mg deficient rats. Calcium entry blockers (verapamil and nifedipine) caused a concentration-dependent depression of the noradrenaline-induced contraction. The aortas from Mg deficient rats were more sensitive to these drugs than were those from the controls. These results suggest that the noradrenaline-induced contraction depends much more on extracellular calcium influx in Mg deficient rats than in the controls.
