Suppression of host resistance against Listeria monocytogenes infection by 15-deoxyspergualin in mice.

The effects of 15-deoxyspergualin (DSG), an immunosuppressive agent, on host resistance against Listeria monocytogenes were studied in mice. Administration of DSG in the early phase of infection resulted in fatal listeriosis by preventing acquired anti-listerial resistance, even though the infectious dose was sublethal for the untreated controls. In contrast, DSG treatment started after development of the acquired immunity was ineffective. Endogenous production of interferon-gamma (IFN-gamma) and tumour necrosis factor (TNF) in the bloodstreams induced by the infection was normal in DSG-treated mice. Nevertheless, augmentation of macrophage functions such as expression of major histocompatibility complex (MHC) class II antigens, phagocytic activity and listericidal activity induced by the infection was abrogated by DSG treatment. These results suggest that the inhibitory effect of DSG on anti-listerial resistance might be different from cyclosporine A (CsA).

[Ligation of the crura penis: surgical treatment for venous impotence].

Recently it has become clear that venous impotents account for a high percentage of all impotents. In venous impotents a decrease in outflow resistance is seen. We have aggressively performed operations on venous impotents to produce artificial outflow resistance. In this study, we performed ligations of the crura penis in 6 venous impotents. Five patients recovered from erectile dysfunction and 1 patient showed papaverine-induced full erection. There were few severe complications and the duration of operation was very short. This operation is thought to be a simple and effective treatment for venous impotence.

Detection of high levels of immunoreactive human beta-1 interferon in sera from HIV-infected patients.

We have examined human interferon beta-1 (HuIFN-beta 1) levels in the sera from patients with various diseases by using a newly developed highly sensitive enzyme-linked immunosorbent assay system (ELISA). High levels of HuIFN-beta 1 were detected in the sera from Human Immunodeficiency Virus-I (HIV-I) infected patients, especially of asymptomatic carriers. The serum level of HuIFN-beta 1 may be a good indicator of HIV-I infection and of other immunological disorders.

Cytotoxic triterpenes from a Chinese medicine, Goreishi.

Bioactivity-guided fractionation of the methanol extract of Goreishi (the feces of Trogopterus xanthipes Milne-Edwards) afforded one new and three known cytotoxic triterpenes, namely, 3-O-cis-p-coumaroyltormentic acid, pomolic acid, 2 alpha-hydroxyursolic acid, and jacoumaric acid. In the course of this investigation, six additional compounds having no cytotoxic activity were isolated, namely, maslinic acid, 3-O-trans-p-coumaroylmaslinic acid, ursolic acid, tormentic acid, euscaphic acid, and a new triterpene, 3-O-trans-p-coumaroyltormentic acid. The structures of the new compounds were established on the basis of X-nucleus-proton correlation with fixed evolution time (XCORFE) and other spectroscopic evidence.

[Influence of protein intake on phytohemagglutinin skin test in patients undergoing maintenance hemodialysis].

We studied the influence of protein intake on the phytohemagglutinin (PHA) skin test and purified protein derivative (PPD) skin test in 45 patients on maintenance hemodialysis. The patients were 23 males and 22 females, the ages ranging from 19 to 80 years, with an average of 53.6 years. All patients were being maintained on long-term hemodialysis (mean 37.3 months). They were divided into three groups according to protein intake: Group A; less than 1.1 g/kg/day, Group B; 1.1-1.5 g/kg/day, Group C; more than 1.5 g/kg/day. There were no significant differences among the three groups with respect to age, causative disease and the period of maintenance hemodialysis. We measured protein intake, PHA skin test and PPD skin test. Significant differences were found between Group B and Group C with respect to the PHA skin test values and PPD skin test values, the values of Group C being very low compared with those of Group B. The PHA skin test values and PPD skin test values were only slightly lower in Group A than in Group B. The negative reaction rate of PPD in Group A and Group C was much higher than in Group B. The results suggest that excess intake of protein impaired cellular immunity in maintenance hemodialysis patients, and that protein deficiency can also have a detrimental effect on cellular immunity. Therefore, adequate protein intake should be maintained for optimizing cellular immunity.

[A case of benign fibrous histiocytoma of the lung in a child].

A case of benign fibrous histiocytoma of the lung in a 8 years old boy was presented. He was first admitted in May, 1983, with recurrent pneumonia. Chest X-ray showed a ill-defined mass in the right lower lobe. Bronchoscopy revealed a round tumor, 1 cm. in maximum diameter, with complete obstruction of the Truncus Intermedius. Endoscopic resection was performed and partial obstruction of the Truncus Intermedius remained. He was re-admitted with pneumonia of the right middle lobe in March, 1986. Bronchoscopy showed severe stenosis of the Truncus Intermedius. Right middle and lower lobectomy was performed. The 4 X 4 X 3 cm tumor was located in the median of the Truncus Intermedius. Microscopically, the lesion composed of fibroblast-like cells and histiocyte-like cells. The patient's post-operative course was uneventful. He has no signs of local recurrence or metastasis. We believe, this is the first reported primary benign fibrous histiocytoma of the lung in a child in Japan.

[Review of 43 autopsy cases of urogenital malignant tumors].

Autopsy findings were reviewed in 43 patients clinically diagnosed in the last 12 years as having urogenital malignant tumors. Clinical diagnoses were 14 bladder carcinoma, 11 prostatic carcinoma, 6 renal cell carcinoma, 5 renal pelvic carcinoma, and 7 other malignant tumors. Autopsy showed that 31 cases died due to carcinoma and 12 because of other causes. The most common ultimate causes of death were DIC and infection, especially pneumonia and sepsis. Autopsy showed 36 of the 43 cases (83.7%) with metastasis. Clinical diagnosis showed 34 cases of metastasis, but the number of metastasized organs and lymph-nodes was much lower than in subsequent autopsy findings. Autopsy proved 5 cases of clinical misdiagnosis (11.6%) and 4 of undiagnosed malignant tumors (9.3%). In 15 cases (32.6%) the ultimate cause of death as revealed by autopsy had not been clinically diagnosed. Five cases diagnosed as having died due to cancer in fact were found to have died due to other causes. Recent diagnostic techniques are greatly advanced, yet many findings are still revealed for the first time by autopsy. Autopsy continues to be a very important final arbiter of progress and the effect of malignant tumors, and serves to remind us of the ongoing need for constant vigilance and improvement of clinical diagnostic techniques.

[Clinical evaluation of total cystectomy for bladder carcinoma: a ten-year experience].

Fifty-three patients with primary bladder carcinoma underwent total cystectomy during the past 10 years. Ages ranged from 32 to 87 years old, with an average of 68.2 years. Radical total cystectomy, including systemic lymphadenectomy was performed in 22 cases, simple total cystectomy in 8 cases and salvage total cystectomy in 23 cases. An ileal conduit was made for urinary diversion in 23 cases and the other 26 cases underwent cutaneous ureterostomy. Postoperative mortality was 9.4% (5 of 53 cases) and postoperative complications were noted in 17 of the 53 cases (32.1%). The 5-year cumulative survival rate by the life table method for all the cases was 42.6%. The 5-year cumulative survival rate of radical total cystectomy cases was 67.8%, that of simple total cystectomy cases was 50.0% and that of salvage total cystectomy cases was 7.5%. A significant difference was seen between the first 2 groups and the last group. The 5-year cumulative survival rate of the patients with low stage cancer (pTis, pTa, pT1 and pT2) was 56.1% and that of the patients with high stage cancer (pT3, pT4) was 22.7%. A significant difference was observed between the two groups. The 5-year cumulative survival rate of the patients with grades G1, G2 and G3 cancer was 66.7, 45.4 and 26.7% respectively. A significant difference was seen among the three grades.

[Renal cell carcinoma in the solitary kidney].

We have treated surgically 5 patients with renal cell carcinoma in the solitary kidney. The cause of renal absence was nephrectomy for renal stones, in 2 patients and renal tuberculosis, renal cyst and renal hypoplasia in 1 patient each. Four of the 5 patients died. One of the 4 patients died 5 days after surgery due to gastrointestinal bleeding, 1 due to metastasis, 1 due to gastric cancer and one due to hemodialysis complications. Surgical management of renal cell carcinoma of solitary kidney is discussed.

[Use of arterial infusion of neocarzinostatin with or without combination chemotherapy as a treatment of carcinoma of the prostate].

Prostate cancer patients were treated with a basic therapy of intra-arterial injection of neocarzinostatin (NCS). This therapy was divided into three types of regimen: NCS intra-arterial injection alone, NCS intra-arterial injection + diethylstilbestrol (DES), and NCS intra-arterial injection + aclacinomycin A (ACR) + peplomycin (PEP). A comparative study was carried out on the clinical efficacies of these three regimens, and at the same time an investigation was made of the prognosis in the cases receiving NCS intra-arterial injection alone and NCS + DES. The clinical efficacy was found to be high in each of the three treatment groups in terms of subjective symptoms and laboratory findings, except for ALP. In the evaluation of efficacy on the basis of histological findings, the rates were 60.0% for NCS intra-arterial injection alone, 71.4% for NCS + DES, and 88.2% for NCS + ACR + PEP. Thus, all three of these treatment regimens gave good efficacy rates, but it is especially noteworthy that the combined chemotherapy regimen yielded the highest efficacy rate. On the other hand, the incidence of adverse reactions was much higher in the case of the combined chemotherapy regimen than in the other two regimens. In the patient group administered the NCS intra-arterial injection alone, the one-year survival rate was 75.0% and the 4-year survival rate was 25.0%, while in the NCS + DES treatment group the one-year survival rate was 87.5% and the 4-year survival rate was 37.5%. For individual patients, the correlation between the clinical efficacy and the prognosis was not strong. However, it was concluded that all three of the chemotherapy regimens are useful as forms of remission induction therapy.

[Oral administration of 5-fluorouracil (5-FU) with intravesical chemotherapy as prophylaxis against recurrence of superficial bladder tumors].

The oral administration of 5-FU together with the intravesical instillation of carboquone and cytosine arabinoside was attempted in cases of superficial bladder tumor for the purpose of prophylaxis against postoperative recurrence. The recurrence rate in 25 patients treated by this intravesical chemotherapy and oral 5-FU administration was 9.1% after one year, 14.0% after two years and 27.8% at three to five years. The recurrence rate in 13 patients with intravesical chemotherapy alone was 8.7% after one year, 31.5% after two years and 58.9% at three to four years. The recurrence rate in six patients with oral 5-FU administration alone was 18.2% after one year, 41.6% after two years and 100% after three years. These results suggest that oral administration of 5-FU in combination with intravesical chemotherapy is useful as prophylaxis against recurrence of superficial bladder tumors.

[The treatment of an impotent male with venous leakage].

The intracavernous pressure to produce a passive erection was recorded in a 40-year-old man suffering from impotence. The patient had passive erection with a flow of 160 ml/min. The flow needed to maintain erection was 40 ml/min. The pressure in the flaccid state was 15 mmHg, and 151 mmHg in the state of erection. In the state of erection, the perfused liquid was demonstrated to escape through the deep dorsal vein of the penis. No other causes of impotence were revealed, and we diagnosed his erectile failure to be due to venous leakage. We performed ligation of this vein. After the operation, nocternal penile tumescence was improved. His potency recovered completely, and no complication was noted.

Specific inhibition of urease by N-acylphosphoric triamides.

Benzoyl- and isopentenoyl phosphoric triamides (BPA and IPA) strongly inhibited urease activities from jack bean, soybean, watermelon seed, Proteus mirabilis, P. rettgeri, P. vulgaris, Mycobacterium smegmatis, and Ureaplasma urealyticum. Their I50 values (the final concentration causing 50% inhibition), independent of enzyme source, were 2-21 nM, which are about 1,000-fold lower than that of caprylohydroxamic acid, one of the most potent urease inhibitors. ATP-urea amidolyase activity was inhibited 50% by BPA at a higher concentration of 0.28 mM, but was not affected by IPA even at 1.3 mM. Thirteen kinds of hydrolases (trypsin, chymotrypsin, thermolysin, leucine aminopeptidase, papain, lipase, alpha-amylase, glucuronidase, asparaginase, arylsulfatase, alkaline phosphatase, acid phosphatase, and true cholinesterase), two oxidoreductases (catalase and alcohol dehydrogenase), three transferases (glutamic-oxaloacetic aminotransferase, gamma-glutamyl transpeptidase, and arylsulfotransferase) and two kinases (pyruvate kinase and creatine kinase) were not affected at all even at 1 mM BPA and IPA. Exceptionally, pseudo-cholinesterase from human serum was inhibited by BPA and IPA, whose I50 values were 70 nM and 10 muM, respectively, using acetylthiocholine as a substrate. These values increased to 0.55 muM and 54 muM, respectively, when acetylcholine was used as a substrate. These results show that N-acylphosphoric triamides potently and specifically inhibit urease activity at concentrations of nM order.

Stone formation by Ureaplasma urealyticum in human urine and its prevention by urease inhibitors.

When Ureaplasma urealyticum T-960 was inoculated into normal human urine (10(8) viable cells per ml of urine), a white precipitate formed, with an increase in pH of the infected urine. This precipitate was identified as a mixture of struvite and whitelockite by analysis of the infrared spectrum. Its formation was completely prevented by the addition of 10 microM N-benzoylphosphotriamide, 20 microM N-isopentenoylphosphotriamide, or 0.5 mM caprylohydroxamic acid without the alkalinization of the urine, and the Ureaplasma color change units were also decreased markedly by these compounds. The apparent concentrations for 50% inhibition by N-benzoylphosphotriamide,N-isopentenolyphosphotriamide, and caprylohydroxamic acid against Ureaplasma urease were 7 nM, 2 nM, and 2.2 microM, respectively. From these results, it seems that stone formation by U. urealyticum is prevented with these compounds, that prevention being directly attributable to the inhibition of urease activity, which causes the death of the cells.

Prognostic validity of the specific red cell adherence test in upper urothelial tumours.

The specific red cell adherence (SRCA) test for blood group antigens was used in 32 patients with upper urinary tract tumours to determine whether their survival could be predicted by this test. The SRCA results correlated well with survival in each grade and stage of the tumours. Overall, patients with positive tests showed a significantly higher 5-year survival rate than those with negative tests (87.2% vs. 24.6%). These findings add support to the prognostic importance of this test in upper urinary tract tumours.
