[Reduction in leukocyte elastase levels in patients with sepsis by administration of urinastatin].

Leukocyte elastase (LE) plays a role in the development of shock. Therefore, we examined the effects of urinastatin, a protease inhibitor, on LE in 7 patients with sepsis. LE levels before the administration of urinastatin were high in all patients, especially those in shock. Administration of urinastatin markedly decreased LE levels, with the decrease being more remarkable in patients showing high LE levels before treatment. These results suggest that urinastatin not only reduces LE levels but also suppresses factors that are involved in the development of septic shock.

Isolation of 9-hydroxy-delta-tetradecalactone from lipid A of Pseudomonas diminuta and Pseudomonas vesicularis.

A lipid component was isolated from the fatty acid fraction of acid hydrolysates of lipid A derived from Pseudomonas diminuta JCM 2788 and Pseudomonas vesicularis JCM 1477 lipopolysaccharides. By structural analysis of the lipid and its trimethylsilyl and acetyl derivatives by thin-layer chromatography, gas chromatography-mass spectrometry, mass spectrometry, infrared spectrometry and 13C-NMR, it was identified as 9-hydroxy-delta-tetradecalactone.

Studies on alpha-fetoprotein produced by renal cell carcinoma.

A 58-year-old man was found to have a Stage IV primary renal cell carcinoma (RCC) invading the colon and producing alpha-fetoprotein (AFP). The preoperative serum AFP level of 418 ng/ml had decreased to the normal level of 0.9 ng/ml 3 weeks after the radical operation. As the high serum AFP level in RCC without metastasis to the liver is a rare condition, histopathologic studies were performed on surgical specimens. Histologically, the tumor was clear cell adenocarcinoma and AFP was detected in tumor cells immunohistochemically. A Concanavalin A (Con A) sepharose affinity chromatogram was performed to detect the component of AFP in this case. The Con A nonreactive fraction rate was as high as 47%, which indicated that the AFP of this RCC was of fetal type.

Hb Moriguchi or alpha 2 beta 2(97) (FG4)His---Tyr substitution at the alpha 1-beta 2 interface.

An abnormal profile of cation exchange high performance liquid chromatography for the determination of Hb A1c led to the discovery of a new hemoglobin variant with a His----Tyr substitution at position 97(FG4) of the beta chain. The variant comprised about equal proportion to normal Hb A. It showed an increased oxygen affinity, normal n value and slightly reduced alkaline Bohr effect. However, the magnitude of the functional deviation is less than that of other variants with substitutions at the same position, and the heterozygous carrier was not erythremic.
