Ultrasonic observations of the vitreous body immediately following cataract extraction.

An A-scan ultrasound examination of the vitreous body was performed in 50 cases of cataracta senilis, both immediately preceding the operation and twice following. In 4 cases, the vitreous body was acoustically homogeneous, and in another 4 extensive degeneration was observed both before and after the operation. In 42 cases the vitreous body was acoustically slightly heterogeneous. Cataract extraction did not affect the acoustic structure of the vitreous body. In 16 cases the other eye had been similarly operated on an average of 2 years earlier; in all cases the acoustic structure of the vitreous body in this eye was similar to that in the eye now operated on.

Ultrasonic and biomicroscopic observations on the vitreous space in patients with idiopathic detachment of the retina.

Biomicroscopy and ultrasonography were used to examine the vitreous bodies of 22 unselected eyes with idiopathic detachment of the retina. In addition, the subretinal space was examined acoustically. Optical examination showed severe degeneration of the vitreous body in 17 cases, slight degeneration in four cases, while in one case optical examination was not possible. Acoustic examination yielded the same results. The subretinal space was acoustically heterogeneous in all the eyes examined. In two eyes, the subretinal fluid reflected rapidly moving echoes, in the other 20 the movements were slow. The echoes from the degeneration of the vitreous body were 5-10 dB higher in amplitude than those from the subretinal space. Both optical and acoustic methods should be used to ascertain the disposition of the other eye to detachment of the retina.

[Tachyphylaxis in timolol therapy for chronic glaucoma (author's transl)].

Forty-three patients with open-angle glaucoma were treated locally with 0.5% Timolol. The drop in intraocular pressure on the day following commencement of Timolol therapy was remarkable. However, 14 patients, ten of whom had responded insufficiently to other glaucoma drugs, developed tachyphylaxis either two or three days after being put on Timolol therapy; in some cases tachyphylaxis was observed in follow-up examinations two months later. Intraocular pressure in these eyes was in some cases even higher than it had been prior to treatment with Timolol.

Ultrasonic investigation of the eye with a new contact method and with three different real-time presentations.

The ultrasound examination was carried out by means of a crystal with a 40 degrees sector scan, which was pressed either against the eyelid or directly against the sclera. The real-time display was obtained by either A-mode, B-mode or combined A/B-mode scanning. In clinical applications, both the resolution capacity and the sensitivity of the device were good. A-mode scanning gave the greatest accuracy in amplitude measurement, while the A/B-mode yielded the most concrete topographic picture of the target.

Ultrasonic findings in the vitreous body at various ages.

A total of 444 healthy eyes in 290 subjects of varying age were examined ultrasonically with a frequency of 6 MHz. In subjects under 20 years the structure of the vitreous body was acoustically homogeneous, and in those between 21 and 40 there was slight degeneration in 5% of the eyes. In the age group 41-50 the incidence of degeneration was 19%, in the group 51-60 63% and in that over 60 more than 80%. There was a clear increase in acoustically observable degeneration after the 40th year; nevertheless, in about 10% of those over 60 years an acoustically homogeneous vitreous body was observed. Ultrasonic observation can be used to supplement optical observations, and in the case of elderly patients is often the only way of obtaining information on the structure of the vitreous body.

Diagnostic ultrasound in cases of acute iritis.

The A-scan ultrasound technique was used to examine the vitreous bodies of both the sick and the healthy eye in 47 patients with acute iritis. The acoustic findings were then compared to observations made with the slit lamp. Of the 47 iritis eyes, only 19 were amenable to a least some degree of optical examination. The most common factors preventing optical examination were opacity of the aqueous humor and posterior synechia. Comparison of optical and acoustic findings were difficult, since optical examination was possible in only 19 eyes. It would seem, however, that there is a farily close correlation between acoustic and optical findings. Ultrasound often offers the only possibility of ascertaining the presence and degree of exudation in cases of acute iritis, and such information may be important in planning treatment and following the course of the disease.

[Ultrasonic investigations of the vitreous space in idiopathic retinal detachment (author's transl)].

63 patients suffering from unilateral, idiopathic detachment of the retina were examined by the A-scan ultrasound method. A vitreous degeneration always appeared in eyes with detachment of the retina, and this was severe in 52 eyes. In the other eye the degeneration of the vitreous body was much smaller and in 11 eyes it was not at all proved by ultrasound. In 24 eyes with detachment of the retina the subretinal space could be examined and it was always acoustically heterogenous. The observations concerning acoustic characteristics of the vitreous body may be important in planning of operative treatment and prophylaxis.

Ultrasonic findings in the vitreous space in patients with detachment of the retina.

Ultrasonic observations were made of the vitreous body and the subretinal space of the sick eye and of the vitreous body of the other eye on 32 unselected subjects with idiopathic detachment of the retina. In almost all sick eyes a severe state of degeneration of the vitreous body was observed, and in all cases examined the subretinal space was acoustically heterogeneous. In the other eye degeneration of the vitreous body was often observed, although to a lesser degree than in the eye with retinal detachment. The ultrasonic examination of the vitreous body may yield information as to the disposition of the eye with regard to detachment of the retina.

Ultrasonic findings in the vitreous body in patients with acute anterior uveitis.

The vitreous body of both the healthy and the affected eyes of 25 patients suffering from unilateral acute anterior uveitis was examined by ultrasound, and the results were compared with the optical observations made on the affected eye. In 14 eyes the optical examination of the vitreous body was impossible either due to exudation in the anterior chamber or to posterior synechias of the iris or to cataract. In 17 eyes the vitreous body was acoustically highly inhomogeneous, in three eyes slightly inhomogeneous and in five eyes no acoustic changes due to exudation were found. In cases of acute anterior uveitis, ultrasound examination often provides more information than optical examination by slit lamp. Ultrasound can also be useful in the treatment and follow-up of the disease.

Ultrasonic findings in the vitreous body at different ages and in patients with detachment of the retina.

The effect of age on degeneration of the vitreous body was studied in 77 patients and in 123 eyes. The patients were divided into three groups by age, with a mean age of 16, 35 and 72 years, respectively. In the two younger groups the vitreous body generally appeared to be acoustically homogeneous, while the oldest group showed heterogeneity of the vitreous body in a majority of cases. Syneresis of the vitreous body yielded a highly characteristic echogram. A total of 22 eyes with unilateral idiopathic detachment of the retina were examined, and in all of them degeneration of the vitreous body was clearly detected. Examination of the other, "normal" eye revealed only four eyes with an acoustically homogeneous vitreous body. Six patients had severe syneresis in bthe eyes. The evident conclusion is that detachment of the retina even in a single eye is often accompanied by degeneration of the vitreous body in the other eye as well.

Eye injuries cuased by tear-gas hand weapons.

Six patients hospitalized by eye injuries caused by tear-gas hand weapons are presented. Five of the lesions resulted from a short-distance shot of aerosol irritant projectors. Severe swelling and bloodshot of the lids and conjunctiva occurred in all cases. Epithelial defects, swelling and blurring of the parenchym were observed in the cornea. The treatment of each case took weeks, even months. Such an injury can because of scars in the cornea permanently lower the visual acuity.
