[Ultrasonography in the study of lesions of the menisci and the collateral ligaments of the knee. Findings in 48 surgically treated patients].

The authors report the results of the study of 48/112 patients who underwent US examination of the knee for gonalgia. Meniscopathy or capsular-ligamentous lesions were clinically suspected. US results were compared with arthroscopic or athrotomic findings; the latter two methods were considered as the reference gold standard. US exhibited 81.2% overall diagnostic accuracy, thus confirming its limitations, more evident than those of arthroscopy and arthrography, CT and MR imaging. Major limitations of US were its failed recognition of meniscal tears smaller than 5-6 mm, and its failed distinction of degenerative meniscopathies from common meniscal fractures (with the exception of 2 cases). Moreover, US did not allow cruciate ligament lesions to be demonstrated; however, in all these cases, US did demonstrate increased capsular thickness (greater than 3 mm at the lower margin of femoral condyle). This indirect sign, together with roudness of parameniscus and capsular-ligamentous limitans (at the hemirhyme), and the direct sign of inhomogeneous hyperechogenicity of the meniscal triangle (normally hypoechoic) contributed to raising overall diagnostic accuracy of US to 89.9%. In the 5 patients with a suspected lesion of the medial collateral ligament examined within 48 hours from trauma, US gave 2 false-positive results in 2 cases where forced abduction test was also positive. Massive edema and swelling of adjacent structures prevented the correct evaluation of ligament limitans. In these 2 cases, a lesion in the anterior cruciate ligament was found at surgery; one of them was associated with a meniscal lesion already diagnosed at US. Both parameniscal and popliteal cysts were correctly diagnosed with US. Due to the well-known limitations of clinics in the diagnosis of knee pain, US could be suggested as the examination of choice to evaluate suspected meniscopathy or ligamentous lesions, thanks to its low cost and short execution time. The use of US could also spare the patients more invasive examinations--e.g., arthroscopy and arthrography--and more expensive ones--e.g., CT and MR imaging.

The treatment of fractures of the surgical neck of the humerus by osteosynthesis with Kirschner wires.

Seventy-one patients treated for fracture of the surgical neck of the humerus by osteosynthesis with Kirschner wires inserted percutaneously were followed up. The method is described in detail, and its indications and limitations are discussed. The clinical and radiographic results were extremely positive, with minimal complications and a much reduced work load on the part of the surgical team.

[Double fractures of the tibia: comparison of methods of treatment].

The experience regarding the treatment of bifocal fractures at the Orthopedic Clinic of the University of Padua during a five-year period (1980-1985) is reported. The bloodless method has been abandoned in favor of modern osteosynthesis techniques, which have permitted a shorter healing time, with fewer cases of hospitalization and a more favorable outcome.

The treatment of Dupuytren's contracture by radical aponeurectomy.

The surgical treatment of choice in Dupuytren's contracture is radical aponeurectomy. The clinical and pathological reasons for this are discussed. The authors propose precise indications for operative intervention and describe an original surgical technique. They present a review of 76 comparable cases treated by this method and followed up for an average of 8.5 years. The results obtained were up to the best standards in the most authoritative case reviews, with minimal postoperative complications and minimal time required for functional recovery.

[Radiologic changes of the hands in psoriatic rheumatism].

The radiological features of the psoriatic arthritis, already known in the literature, are reviewed by the authors. On the basis of their own record (33 patients), they underline 3 peculiar features of the disease: thickening of the soft tissue involving the whole finger, three joints involvements of the same finger, association of subchondral proliferation and periostitis.

Early synovectomy in haemophilia.

The Authors reviewed, after from 1 to 11 years, 28 joints of patients with A and B haemophilia operated on with synovectomy at an early stage. Investigation is made into the diagnostic elements for evaluating the earliness of arthropathy and indications for surgery. The results from early synovectomy are compared with the case histories reported in the literature when synovectomy was done at an advanced stage.

The Ilizarov method in the treatment of malignant neoplasms of the tibia.

A total of 3 malignant neoplasms of the tibia are presented: 1 is a mesenchymal chondrosarcoma of the tibial pylon (male aged 14 years), and 2 are cases of squamous skin carcinoma of the leg with tibial infiltration (1 male and 1 female aged 32 and 64 years, respectively). The resections carried out (16.5 cm on the average) were treated by bone transport. Simple transport was used in the first patient, double in the other two. At the end of transport a second surgical stage involving astragalotibial arthrodesis was performed in the first case, and revision of the junction site of bone segments transported with application of autoplastic bone grafts was performed in the other two. The regenerate obtained was slowly corticalized in the first patient, submitted to various cycles of chemotherapy during the course of distraction. In the other two cases, which were not treated by chemotherapy during distraction, corticalization occurred over a shorter amount of time. Follow-up was obtained after 10 years in the first case, and after 2 years in the other 2 patients. There was no long-term recurrence.

[Long-term results of the conservative treatment of thalamic fractures of the calcaneus].

We have inspected 79 heel thalamic fractures, that have been treated conservatively, at an average follow-up of 8.8 years since trauma. In addition to the clinical examination, every patient was subjected to podoscopy and radiographic examination in two projections. The functional result was extremely positive in 80% of cases, without a significant relation with the severity of anatomic deformations, that are almost always present. We conclude therefore that the functional result is only partly related to the entity of clinical and radiographic pictures and that the conservative treatment gave good results in most cases, without the risks of a surgical treatment in this anatomic site.

Death rate in patients submitted to the surgical treatment of fracture of the proximal femur.

The death rate 6 and 12 months after surgery in 389 patients who had been treated for fracture of the proximal femur between 1989 and 1993 (1st semester) was studied. All of the patients submitted to surgery were traced through the official records on the population of the City of Padova. The overall death rate after 6 months was 16%, that after 12 months 23%. The best survival rate was observed in patients who had been submitted to gamma nail arthroplasty (p < 0.01).
