Regional blood flow as a determinant of drug absorption description of an animal model.

Factors that influence recommended dosage guidelines for intramuscularly and subcutaneously injected drugs include a spectrum of variables, including therapeutic objective(s), the drug's therapeutic index, the inclusion of concomitant vasoconstrictors, and so on. However, little account is taken of the likely influence of regional blood flow at the injection site. This may vary according to the site of injection, the age and medical condition of the patient with the potential to markedly affect the rate of drug absorption and the resultant therapeutic and toxicity outcomes. An animal model is described for simultaneously quantifying the drug's pharmacokinetic profile and the regional blood flow (using technetium99 elution) at sites of differing vascularity so that the potential relationship can be ascertained. An application to lignocaine absorption is provided, however, the model would be applicable to a range of drug applications and could serve as a means of investigating differences both within a drug class (e.g., members with differing lipophilicity) as well as across drug classes (e.g., local anesthetics, opiates, insulin preparations, etc.).

Figure eight tension band in the treatment of fractures and pseudarthroses of the medial malleolus.

Between 1 January 1981 and 30 June 1987, 104 closed medial malleolar fractures and seven pseudarthroses of the medial malleolus were treated by tension band wiring. The ligaments, capsule and other fractures were reconstructed. Most of the fractures had healed after 8 weeks. We re-examined 97 of the fresh fractures and all seven of the pseudarthroses at an average of 3.8 years after the operation. Of the medial malleolar fractures 92.3 per cent were excellent. In type A fractures 90 per cent were excellent, in type B fractures 92.5 per cent were excellent and in type C fractures 89.36 per cent were excellent. In the group of pseudarthroses six were excellent.

An evaluation of a new test kit to screen for galactosemia.

A new galactosemia screening kit has been evaluated. The kit employs the Yoshida modification of the Paigen et al. Escherichia coli bacteriophage assay and is produced by Eiken Co. Ltd. of Japan. The assay estimates galactose and galactose-1-phosphate in dried blood specimens. The kit is sufficiently sensitive to detect possible heterozygotes as well as possible homozygotes.

Dihydropteridine reductase in schizophrenic patients.

The activity of the enzyme dihydropteridine reductase (DHPR) has been recently found to be one of the factors controlling the rate of synthesis of dopamine, norepinephrine, and serotonin, thought to be involved in the etiology of schizophrenia. Several lines of evidence suggest that peripheral and brain DHPR enzymes may be identical. In addition, peripheral DHPR activity has been hypothesized to be important in determining the level of phenylethylamine, a putative psychotogen that is produced peripherally and crosses the blood-brain barrier. Since DHPR activity has never been investigated in schizophrenic patients, we measured the whole blood activity in 20 schizophrenic patients and 20 matched controls. There was no difference between the groups in DHPR activity.
