Ultrasonography as the method of incidental detection of mandible lesions.

We present two cases of patients with bone cysts in which the suggestion of a bone lesion was made primarily on the basis of ultrasound examination. Examinations were performed with use of a 7.5 MHz linear probe on Siemens Elegra ultrasound scanner (Siemens, Erlangen, Germany) with application of tissue harmonic imaging (THI) and power Doppler mode. On the basis of such features as uneven and irregular bone surface, widening and distorting of the mandible shape or visualization of the mass inside the bone on ultrasound examination, the presence of the bone tumor was suggested. Ultrasound examination may be the first method suggesting the presence of the unsuspected mandible tumor in patients referred to search for underlying pathology in soft tissues. During the examination of the soft tissues of the head and neck also the surface of bones should be each time carefully evaluated.

Pleomorphic adenoma and adenolymphoma in ultrasonography.

OBJECTIVE: The aim of the study was to define ultrasonographic morphologic features characteristic of pleomorphic adenoma and adenolymphoma (Warthin's tumor). METHODS: Documentation of 31 ultrasonographic examinations of 20 pleomorphic adenomas and 10 Warthin's tumors in 28 patients was analysed. All cases were confirmed by fine needle aspiration biopsy or/and histopathological examination. Estimated ultrasound criteria were: borders (well-defined, predominantly well-defined, ill-defined), shape (lobulated, oval, irregular), echogenicity (increased, decreased), structure (homogeneous, slightly inhomogeneous, inhomogeneous) and presence of irregular anechoic areas within a tumor. RESULTS: 100% of the lesions were hypoechoic. 80.6% of all tumors were well-defined. The remaining 19.4% had predominantly well-defined borders. 55% of pleomorphic adenomas had a lobulated shape. Almost equal percentages of Warthin's tumors were lobulated, had an oval and irregular shape. Irregular echolucent areas were present in six of 11 examinations of Warthin's tumors and in one pleomorphic adenoma. CONCLUSIONS: Ultrasonography is a useful method for the evaluation of pleomorphic adenomas and Warthin's tumors and sometimes it might allow to suggest the nature of a tumor if a certain sonographic pattern is present.

[Classification of the neck dissections].

The distribution of the neck surgery dissections proposed by the Academy Committee for Head Neck Surgery and Oncology, Education of the Head Neck Surgery (year 1991) was discussed in this paper. The authors are in the agreement with proposed distribution and strongly recommend its implementation in Poland. The standard radical neck dissection according to Crile was accepted as the base of this distribution. Neck dissection similar to Crile's operation but with saving up important anatomical structures (n. XI., vena jugularis int., m.scm) was called modified radical neck dissection (type I-III). Excision of the lymphatic system from the well-defined regions of the neck with saving up nonlymphatic structures and/or group of lymphatic system same in Crile's operation with additional nonlymphatic structures and/or another lymphatic nodules nonmentioned in Crile's operation is called enlarged radical neck dissection.

A feasibility study of concomitant boost radiotherapy for patients with cancer of the supraglottic larynx.

Between February 1988 and December 1989, 65 patients with supraglottic cancer completed a course of concomitant boost radiotherapy. Cases with N3 disease, a Karnovsky performance score less than 70, age above 70 years, or a second primary cancer were not eligible for the study. Distribution of the patients was: Stage T1-T2 30%, T3-T4 70%, N0 68%, N+ 32%. The total dose ranged from 60 Gy to 76 Gy (median 66 Gy); overall treatment time ranged from 36 to 56 days with a median of 42 days. The daily dose during the first 4 weeks was 1.8 Gy, and during the last 2 weeks it was 1.6 Gy b.i.d. with a 4- or (after September 1988) 6-h interval. The clinical impression was that the early mucosal reactions were acceptable but more severe than after conventional treatment, with confluent membranous mucosal reaction being observed in 54% of the patients. Also, this reaction was significantly more frequent in patients treated with a 4-h interval (68%) than with a 6-h interval (41%) between the daily fractions. To relieve severe dysphagia, narcotics were required in 22% of the patients. The follow-up time ranged from 22 to 50 months, median 34 months. Treatment-requiring late complications were observed in 8 patients, and the 3-year actuarial risk was 17% with 95% confidence limits (6%, 27%). Two of these patients had severe complications: one of them required a temporary tracheostomy due to arytenoid edema and the other developed a laryngo-cutaneous fistula which healed after pharmacological treatment. Actuarial 3-year local-regional control was 59% (46%, 71%) and 3-year actuarial crude survival 55% (42%, 67%). There was no significant difference in the incidence of late complications or tumor control between the two groups of patients treated with a 4- or 6-h interval between the daily fractions. This study shows that the concomitant boost regimen tried here is feasible, but also stresses that the interval between dose fractions should be 6 h or more.

[Clinical course of laryngeal cancer in relation to sex].

In 1960-1987 2352 cases of larynx cancer were cured and observed. The number of the side men to women was in the ratio of 11.5:1. The cancer of epiglottis was prevailing in both sex but percentage of these cancers was higher in women's cases than men's (67.6% vs 62.7%). Besides women called for the treatment more often with the cancer in initial states of the disease (62.2% vs 53.4%). Five--year survivals (actuarial method) were higher amongst women (69.8% vs 60.0%). In the same state of clinical advancement of the disease, the 5-year survivals were also higher with women. In the cases of cancer in the early states of larynx cancer (I and II) the difference was about 5.0%, and among patients with cancer in the late state of the disease (III and IV) the difference was 8.4%. The last premise caused the authors to put forward the hypothesis that apart from the sooner calling for the treatment of women, there may also be other unknown phenomena, perhaps hormone--immune, conditioning higher index of survivals in women.

Concomitant boost radiotherapy of supraglottic cancer--preliminary results, morbidity.

Forty-two patients with supraglottic squamous cell carcinoma were irradiated with Co60 using concomitant boost technique, a variant of accelerated fractionation. This technique is characterized by administering the boost as a second daily fraction during the basic wide-field irradiations. Daily dose in the first 4 weeks was 1.8 Gy and during the last 2 weeks it was 1.6 Gy twice daily with 4-6 hours separation. Total dose ranged from 60 to 76 Gy, median 66 Gy. The treatment time ranged from 36 to 56, median 42 days. Acute mucosal reactions were more severe than those of conventional irradiation but acceptable. The follow-up time ranged from 9 to 31 months, median 16 months. The actuarial 18 months survival for all patients was 75%. The actuarial 18 months control for primary site and lymph node was 75% for T1-2 stage, and 47% for T3-4 stage. Severe late complications were not observed so far.

[Curability of laryngeal cancer in relation to the starting point].

In 1960-1987, 2387, 2352 cases of larynx cancer were treated and observed. In 1484 (63.1%) cases the cancer of the epiglottis was found, in 851 (36.2%)--in glottis and in 17 (0.7%) in subglottis. Cancers of supraglottis, in 60.0% cases. were in state III and IV of clinical advance and cancers of glottis in the same state were found in 20.6% of cases. Generally 52.4% of cases of epiglottis cancer were cured of and 74.4% of glottis cancer. Relative survivals were accordingly: 59.0% and 87.0%. In the period of 5-year observation 47.6% of cases of epiglottis cancer died and 25.6% died of glottis cancer. The chances of recovering of epiglottis and glottis cancer are similar when the cancer is in the same state of clinical advance. 76.3% of objects with epiglottis cancer and 83.8% of glottis cancer in the early state (I and II) of clinical advance survived for 5 years and 37.6% objects sick of epiglottis cancer and 38.7% with glottis cancer in clinical state III and IV.
