This report describes a rooming-in program based on allocation of resources to patients in accord with their health needs. It outlines the advantages of having the healthy infant kept with the mother continually during the puerperium. This promotes easier psychological adaptation of mother and infant, reduces the incidence of cross-infection in the hospital, encourages breast-feeding, and paves the way for providing the newborn with regular checkups at peripheral health centers. The report also points out the need to instill a strong instructional component into the roomin-in program. The basic elements of the overall program are maternal training based on indivisual instruction, group discussion sessions accompanied with instructional slides, and participation by the mother in the care of her infant under supervision of the midwifery team. The adoption of a program of this nature does not imply an increase in existing human and material resources, but rather a rationalization of the distribution of present resources and the added contribution of the mother's active participation in helping to care for her child.

A study was undertaken to look at the clinical behavior of the Dalkon Shield, the Lippes Loop D and the TCu-200 inserted by physicians and midwives. Differences among the three devices were not statistically significant, although lower expulsion rates and higher pregnancy rates were reported for the Dalkon Shield. No differences were observed between insertions performed by physicians and those performed by midwives; this emphasizes the efficiency of using paramedical personnel in the field of intrauterine contraception.
To study the possible role of sexual hormones, Testosterone (T), Estradiol (E2), follicle-stimulating hormone (FSH), and luteinizing hormone (LH) on the growth axis, we examined the correlations between the sex, growth hormone (GH), insulin-like growth factor-1 (IGF-1), insulin-like growth factor binding protein 3 (IGFBP3), FSH, LH, T and E2, in growth retarded children ranging in age from 7 to 13 yr. All hormones were measured by Radioimmunoassay (RIA) in a pool of aliquots of samples obtained every 20 min over 12 h (overnight) in each child. Multivariate statistical analysis was performed. We have found: a) Thai only FSH concentrations were significantly higher in girls than in boys; b) A positive correlation between T-IGF-1; T-IGFBP3; FSH-LH; FSH-IGF-1; FSH-IGFBP3, LH-IGF-1, LH-IGFBP3 the sex-FSH; and the sex-IGFBP3 c) A high positive correlation between plasmatic E2 and IGF-1/IGFBP3 ratio (an index of free, active IGF-1). We concluded that the sex, FSH, LH, T and E2 influence the growth axis. The sex through IGFBP3; LH, FSH, and T through IGF1 and IGFBP3; E2 through the IGF-1/IGFBP3 ratio, an index of active IGF-1.
Von Hippel-Lindau (VHL) disease is characterized by a high predisposition to develop retinal angiomas, hemangioblastomas of the central nervous systems, renal cysts and renal carcinomas, pheochromocytomas, pancreatic cysts, and cystadenomas of the epididymis. The VHL gene was isolated in 1993; this fact allows to carry out presymptomatic diagnostic of this disease. We report on the case of patient with the suspicious++ and of hereditary VHL, although she had not familial history. By means of sequence the VHL gene was studied a mutation in exon 2 (GTT130CTT) that results in an amino acid change was found. This mutation could not be detected in her parents, pointing out that this is a de novo case. Patient has two sons, of 18 and 13 years old. The genetic analysis of the youngest showed that he was not a mutation carrier; while the eldest denied to be explored. The genetic study allows, in cases without familial history, to determine if they have the hereditary form of the disease and to study the siblings periodically until the beginning of the disease.
