High-resolution millimeter wave spectroscopy and multichannel quantum defect theory of the hyperfine structure in high Rydberg states of molecular hydrogen H2.

Experimental and theoretical methodologies have been developed to determine the hyperfine structure of molecular ions from detailed studies of the Rydberg spectrum and have been tested on molecular hydrogen. The hyperfine structure in l=0-3 Rydberg states of H2 located below the X 2Sigmag+(v+=0,N+=1) ground state of ortho H2+ has been measured in the range of principal quantum number n=50-65 at sub-MHz resolution by millimeter wave spectroscopy following laser excitation to np and nd Rydberg states using a variety of single-photon and multiphoton excitation sequences. The np1(1), nd1(1), and the nf1(0-3) Rydberg states were found to be metastable and to have lifetimes of more than 5 micros beyond n=50. Members of other series, such as the nd1(2), nd1(3), and the np1(0) series, were found to have lifetimes of more than 1 mus. Local perturbations induced by low-n Rydberg states belonging to series converging on rovibrationally excited levels of H2+ reduce the lifetimes in narrow ranges of n values. The hyperfine structure is strongly dependent on the value of the orbital angular momentum l. In the penetrating s and p states at n approximately 50 the exchange interaction dominates over the hyperfine interaction and the levels can be labeled by the total electron spin angular momentum quantum number S (S=0 or 1). In the less penetrating d and f Rydberg states, the hyperfine interaction between the core nuclear and electron spins is larger than the exchange interaction and the Rydberg states are of mixed singlet and triplet character. A procedure based on the Stark effect and on the systematic analysis of selection rules and combination differences was developed to determine the orbital and the total angular momentum quantum numbers l and F and to construct an energy map of p and f Rydberg levels between n=54 and 64 with relative positions of an accuracy of better than 1 MHz. Multichannel quantum defect theory (MQDT) was extended to treat the hyperfine structure in molecular Rydberg states and was used to analyze the observed hyperfine structure of the p and f Rydberg states of H2. The frame transformation between the Born-Oppenheimer channels described by the angular momentum coupling scheme (abetaJ) and the asymptotic channels described by the (e[bbetaS+]) coupling scheme was derived and enables an elegant treatment of all intermediate coupling cases. Purely ab initio quantum defect theory reproduced the experimentally determined positions to within 40 MHz for the p levels and 13 MHz for the f levels. By slight adjustments of the quantum defect functions and their energy dependences and by consideration of the p-f interaction, of the singlet-triplet splittings of the f levels, and of the departure of the ionic levels from pure coupling case (bbetaS+), the agreement between theory and experiment could be improved to 600 kHz. By comparing the results of MQDT calculations of the hyperfine structure of f Rydberg levels with those of coupled equations calculations, the frame transformation approximation of MQDT was shown to be accurate to within 300 kHz. The extrapolated ionic hyperfine structure of the X 2Sigmag+(v+=0,N+=1) ionic level corresponds to the ab initio prediciton of Babb and Dalgarno [Phys. Rev. A 46, R5317 (1992)] within the experimental error.

Experimental and theoretical study of the infrared spectra of BrHI- and BrDI-.

Gas phase vibrational spectra of BrHI- and BrDI- have been measured from 6 to 17 microm (590-1666 cm(-1)) using tunable infrared radiation from the free electron laser for infrared experiments in order to characterize the strong hydrogen bond in these species. BrHI-.Ar and BrDI-.Ar complexes were produced and mass selected, and the depletion of their signal due to vibrational predissociation was monitored as a function of photon energy. Additionally, BrHI- and BrDI- were dissociated into HBr (DBr) and I- via resonant infrared multiphoton dissociation. The spectra show numerous transitions, which had not been observed by previous matrix studies. New ab initio calculations of the potential-energy surface and the dipole moment are presented and are used in variational ro-vibrational calculations to assign the spectral features. These calculations highlight the importance of basis set in the simulation of heavy atoms such as iodine. Further, they demonstrate extensive mode mixing between the bend and the H-atom stretch modes in BrHI- and BrDI- due to Fermi resonances. These interactions result in major deviations from simple harmonic estimates of the vibrational energies. As a result of this new analysis, previous matrix-isolation spectra assignments are reevaluated.

Prospective audit of laparoscopic totally extraperitoneal inguinal hernia repair: a multicenter study of the Swiss Association for Laparoscopic and Thoracoscopic Surgery (SALTC).

BACKGROUND: This article reports the results of a multicenter prospective audit of totally extraperitoneal (TEP) inguinal hernia repair conducted by the Swiss Association for Laparoscopic and Thoracoscopic Surgery (SALTC) from May 1995 to August 1996. METHODS: At 29 Swiss centers 1,605 inguinal hernia repairs were performed in 1,186 patients. Half of the repairs were performed by operators whose experience consisted of fewer than 51 procedures. Patients were followed up for 1 year. RESULTS: Bilateral repairs were performed in 35% of the patients, and 15% of all repairs were for recurrent hernia. Conversion rates to the transabdominal preperitoneal (TAPP) technique and open surgery were 1.8% and 1.6%, respectively. Main postoperative complications were hematoma and urinary retention. At 3 months, seroma was more frequent with slit prosthesis. The recurrence rate was 0.6% at 3 months and 1.6% at 1 year, not depending on the type of implant. The rate for recurrent hernias did not differ from that for primary repairs. CONCLUSIONS: Total extraperitoneal (TEP) repair can be performed with low morbidity and a high level of patient satisfaction. The effects of the learning curve are not to be neglected. The 1-year recurrence rate is 1.6%. Published data on TEP suggest that late recurrences may be less frequent than after open repair.

[Metatarsus primus double osteotomy, a logical and non-disabling surgical technique for treatment of hallux valgus].

The procedure known as the Metatarsus Primus Double Osteotomy ("L'ostéotomie métatarsienne bipolaire" of Schnepp-Carret) for the treatment of the hallux valgus is a logical, efficient and non-mutilating procedure. The technique consists in reducing the hallux valgus deformity by a subcapital bone wedge resection of the metatarsus primus. The bone wedge is then pinched into a second proximal osteotomy of the metatarsus primus, correcting the varus deformity in a valgus direction. This procedure is little known and is not even mentioned in the classic orthopedic literature of German and English language.

Four-part valgus impacted fractures of the proximal humerus.

There is a specific type of displaced four-part fracture of the proximal humerus which consists of valgus impaction of the head fragment; this deserves special consideration because the rate of avascular necrosis is lower than that of other displaced four-part fractures. Using either closed reduction or limited open reduction and minimal internal fixation, 74% satisfactory results can be achieved in this injury.

[Lengthwise growth of the lower extremities after shaft fractures in children].

Clinical and orthoradiographic follow-up of 50 tibial and 36 femoral fractures in children at an average of 4 1/2 years post trauma has revealed the follwing trends: Increased growth is irrelevant with regard to tibial fractures. Most of the conservatively treated femoral fractures were shortened at the time of follow-up, most of the operatively treated were longer than the uninjured side. This may be explained by lack of primary shortening after anatomical reduction. The increased growth of the fractured femur was more pronounced after conservative than operative treatment, probably because of the more marked primary shortening. Increased growth of the femur is almost 50% less in children older than 12 years whether they are treated operatively or conservatively. Therefore anatomical reduction if necessary by an operation has to be achieved in children older than 12 years. Finally an account is given on the shortening effect of a dislocation ad axim and the radiological projection of this dislocation in two plains.
