Effects of recombinant human granulocyte colony stimulating factor (rhG-CSF) on hemopoiesis and survival rate following allogeneic bone marrow transplantation in mice.

The effects of recombinant human granulocyte colony stimulating factor (rhG-CSF) on the survival rate and hemopoiesis after allogeneic bone marrow transplantation (BMT) were examined. Specific pathogen-free (SPF) C3H mice and germ-free C3H mice, kept in an isolator for germ-free animals, received 10 Gy of total body irradiation (TBI) and all the mice died by day 9 after TBI. These survival rates were improved by BMT. In the case of SPF mice, survival rates at 14 and 100 days were 33% (12/36), 17% (6/36) and in the case of germ-free mice they were 79% (15/19), 74% (14/19) respectively. When SPF mice received rhG-CSF (30 micrograms/kg/day) subcutaneously for 14 consecutive days following BMT, their survival rates were improved to 79% (30/38), 79% (30/38) respectively. The survival rate of rhG-CSF treated SPF mice were equal to that of germ-free mice. When the effect of rhG-CSF treatment on hemopoiesis of SPF mice after allogeneic BMT was examined various hematopoietic progenitor cells in the bone marrow and spleen increased until day 10 after, BMT, while only neutrophils increased in the peripheral blood during the period. No adverse effects of rhG-CSF were observed throughout the study period. It was suggested that in SPF mice treated with rhG-CSF after BMT, the neutrophil recovered in counts quickly and increased neutrophil prevented endogenous infections and improved the survival rate without apparent complications.

Variations in serum levels of insulin-like growth factor-1, growth hormone and thyroid hormones during lactation in dairy cows.

1. Serum levels of insulin-like growth factor-1 (IGF-1) in dairy cows declined after parturition, remained low during the period of early lactation with peak milk production and rose gradually until the end of lactation thereafter. 2. Growth hormone (GH) levels in sera of cows changed in parallel with milk yields. 3. Serum thyroxine and triiodothyronine levels during lactation remained fairly constant.

[Clinical significance of gastrointestinal decontamination under protected environment].

Many infections are caused by the patient's own oro-intestinal microbial flora under a protected environment. Thirty-eight patients with acute leukemia and two patients with blast crisis of chronic myelocytic leukemia were treated under a protected environment with or without prophylactic antibiotics. Antibiotics used for decontamination were vancomycin (V), polymyxin B (P) and nystatin (N). The number of patients in the VPN, PN and the no antibiotic group were 13, 13 and 14, respectively. While the intestinal microbial flora was almost completely eliminated in VPN group, the number of bacteria decreased slightly in PN group. The mean number of pharyngeal and anorectal bacterial species decreased most markedly in the VPN group, but there were no significant differences among the three groups. The number of febrile days was significantly lower in the VPN and PN group than the no antibiotics group with neutrophil counts of less than 100 microliters. The average number of episodes of infection per patient was lowest in VPN group and highest in the no antibiotic group. These data indicate that VPN administration is effective for eliminating intestinal bacterial flora and resultantly protecting endogenous infections.

Effect of a prolonged release formulation of recombinant bovine somatotropin on plasma concentrations of hormones and metabolites, and milk production in dairy cows.

The effect of a prolonged release formulation of recombinant DNA derived bovine somatotropin (rbST) on the plasma concentrations of growth hormone (GH), insulin-like growth factor-I (IGF-I), insulin, glucose, blood urea nitrogen (BUN) and nonesterified fatty acids (NEFA), and milk production in lactating dairy cows was studied. Eight cows were divided into two equal groups. One group was the noninjected control, and cows in the other group received a single subcutaneous injection of 640 mg rbST. Plasma GH levels in the rbST-treated cows were higher than in the control cows for 10 days after the injection. Plasma IGF-I concentrations were significantly higher in the rbST-treated cows than in the control for 14 days after the treatment. In the rbST-treated cows, the plasma concentrations of insulin and glucose tended to be higher than those in the control until 7 days after the injection. Also, plasma NEFA levels were higher in the rbST-treated cows for 10 days. In contrast, plasma BUN levels were significantly lower in the rbST-treated cows for 17 days after the treatment. For 28 days after the injection, the mean daily milk yield in rbST-treated cows was 4.5 kg (21.2%) more than that in the control cows. In the rbST-treated cows, a highly positive correlation was observed between the mean daily milk yield and the mean plasma concentration of IGF-I throughout the postinjection period.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of follicular bronchiolitis with broncho-pleural fistula in rheumatoid arthritis].

A 67-year-old male diagnosed clinically as having rheumatoid pleuritis and bronchiolitis was treated with adrenocorticosteroid. His clinical findings improved, but following the tapering of the steroid dose, exacerbation occurred. After the steroid dose was increased, serological findings improved, but chest X-ray findings revealed no improvement. To re-evaluate the etiology of the bronchiolar lesion, open lung biopsy was performed. The biopsy specimen showed lymphocytic infiltration and formation of lymphoid follicles in and around the bronchioles. The pulmonary lesion was diagnosed as follicular bronchiolitis.

[Ecological study of the pharyngeal bacterial flora in patients with lung cancer].

We investigated the relationship between changes in pharyngeal bacterial flora and the immunological condition of 10 lung cancer patients before and after anti-cancer chemotherapy. There were significant differences in bacterial flora between normal subjects and patients. However, there were little differences in pharyngeal flora in patients before and after chemotherapy. In 4 out of the 10 lung cancer patients, the ratio of anaerobic bacteria/total bacteria of pharyngeal flora decreased. These 4 cases were in the non-responsive (NC, PD) group of anti-cancer chemotherapy. After administration of anti-cancer drugs, the ratio of anaerobic bacteria increased. No aerobic gram negative rods were isolated at any time during the observation period, because immunological suppression was mild. The results indicate the importance of surveillance for the immunological state of the hosts and changes in pharyngeal bacterial flora.

Physicochemical characteristics and flora of diarrhoeal and recovery faeces in children with acute gastro-enteritis in Kenya.

Physiochemical characteristics and flora of diarrhoeal and recovery faeces were investigated in 14 Kenyan children with acute gastro-enteritis. Causative micro-organisms were Shigella, Campylobacter, enterotoxigenic Escherichia coli, rotavirus and unknown in 6, 2, 1, 2 and 3 patients, respectively. The mean values of the pH of the diarrhoeal specimens were significantly higher than those of the recovery specimens. Large amounts of acetic acid and many other kinds of fatty acids were detected in the recovery specimens, but small amounts and few kinds of fatty acids were detected in the diarrhoeal specimens. Bacterial counts of anaerobic organisms, such as Bacteroides, Bifidobacterium, Lactobacillus and Eubacterium, were lower in the diarrhoeal specimens than in the recovery ones. The normal anaerobic intestinal flora is remarkably disturbed in patients with acute gastro-enteritis. This may result in changes in fatty acid contents and in the pH of diarrhoeal faeces.

[Studies of intestinal microbial flora in the post-BMT (bone marrow transplantation) patients under a protected environment].

The changes of microbial burdens of six patients with leukemia (four patients with acute leukemia; two patients with chronic leukemia) were studied before and after bone marrow transplantation (BMT) under protected isolation. Oral nonabsorbable and topical antibodies were administered prophylactically to all patients. Under a protected environment, genus and species number of intestinal microbial flora were not so decreased in all patients who were treated with antibiotics, but no episodes of severe septicemia were detected due to intestinal microbial flora. From many previous reports, the same pathogen was isolated from both blood culture and stool in the patients with septicemia, however, no septicemia developed in our cases in spite of residue of many intestinal bacteria. These data have demonstrated a significant advantage of treatment with protected isolation and intensive antibiotic prophylaxis through oral, topical and intravenous administration for severe infection prevention.

Plasma growth hormone, insulin-like growth factor-I, and milk production responses to exogenous human growth hormone-releasing factor analogs in dairy cows.

Responses of plasma growth hormone (GH) and insulin-like growth factor-I (IGF-I), and milk production to subcutaneous (sc) injection(s) of two synthetic human growth hormone-releasing factor (hGRF) analogs were studied in dairy cows. Two mg of each hGRF analog dissolved in 5 ml saline per cow were injected into the shoulder area of each experimental animal, and jugular venous blood samples were collected via an indwelling catheter or by venipuncture. Plasma GH and IGF-I concentrations were measured by radioimmunoassay methods. In dry cows, the mean concentration of plasma GH after a single sc injection of hGRF analogs rose to 22.0-28.3 ng/ml at about 5 h from 1.4-1.7 ng/ml at 0 h (just before injection), and returned to the level before injection after 10-12 h. On the other hand, the plasma IGF-I began to increase after a lag of 4-6 h following a single injection of hGRF analogs, and reached maximum values of 71.1-89.4 ng/ml at 20 h from 43.7-46.4 ng/ml at 0 h. The IGF-I concentration at 24 h after a single injection of hGRF analogs was still higher than the value for the dry cows given saline. In lactating cows, the plasma concentration of GH at 2 h after daily sc injections of hGRF analogs during 14 consecutive days (an injection period) was higher than those for the lactating cows which received saline. Also, during the injection period, the concentration of IGF-I was higher in the lactating cows which received hGRF analog injections than in the cows which received saline injections. During the last 7 days of the injection period, the administration of hGRF analogs increased the mean milk yield by 11-19% in comparison with those for the saline injected cows. A positive correlation was observed between the mean plasma IGF-I concentration and the mean milk yield in the lactating cows treated with hGRF analogs throughout the injection and a postinjection (11 consecutive days after cessation of hGRF analog injection) periods. The results demonstrate that a single sc injection of hGRF analogs stimulates both GH release and the circulating level of IGF-I in dry cows, and that daily sc injections of hGRF analogs over 14 days enhance milk production, and plasma GH and IGF-I levels in lactating cows.

Peripheral blood neutrophil function in germfree and conventional rats.

The superoxide anion production and the myeloperoxidase activity between polymorphonuclear cells (PMNs) from conventional and germfree rats were compared. Furthermore, the response to recombinant human granulocyte colony stimulating factor (G-CSF) between both groups of rats was also examined. Our present investigation showed that: (1) the basal superoxide anion production by PMNs in germfree rats (2.1 +/- 0.5 nmol/min/1 x 10(6) PMNs) is less than one fourth of that of conventional rats (9.5 +/- 2.9 nmol/min/1 x 10(6) PMNs). (2) the myeloperoxidase activity of PMNs from germfree rats (38.1 +/- 5.2 nmol/min/1 x 10(6) PMNs) is 1.7 times that of conventional rats (22.4 +/- 4.9 nmol/min/1 x 10(6) PMNs). (3) G-CSF administration increased the number of PMN, 12.4 times in the germfree rats compared with 3.9 times in the conventional rats. (4) the PMNs induced by G-CSF do not differ in the superoxide anion producing activity. However the myeloperoxidase activity decreased in the G-CSF induced PMNs of both groups of rats. From our data, the conventional rats could have a more effective killing activity than the germfree rats. Furthermore, germfree rats may be used as a powerful tool for assay of other biological substances relating to immune system and its regulation.

Effect of recombinant human granulocyte colony-stimulating factor (rhG-CSF) on allogeneic bone marrow transplantation in mice.

The effects of recombinant human granulocyte colony stimulating factor (rhG-CSF) on the survival rate and hemopoiesis after allogeneic bone marrow transplantation (BMT) was examined. Specific pathogen-free (SPF) C3H mice, kept in a flexible isolator for germ-free animals, received 10Gy of total body irradiation (TBI), and an allogeneic BMT 24 hours later. The 14 day survival rate was 33%. When the mice were received rhG-CSF (30 micrograms/kg/day subcutaneously) daily for 2 weeks following BMT, the 14 day survival rate rose to 79%. The effect of rhG-CSF treatment on hemopoiesis in BMT mice resulted in higher neutrophil counts in the peripheral blood by day 14 post-BMT. Administration of rhG-CSF after allogeneic BMT benefited survival rate and neutrophil recovery without any apparent complications.

Ecological mechanism of protection of intestinal bacterial flora against Salmonella typhimurium infection.

Our results indicate that the responsiveness of the phagocytic and intracellular killing process of peripheral blood granulocytes and peritoneal phagocytic cells in infected germfree mice is due primarily to the OCl- (hypochloride ion) with myeloperoxidase involvement, while the response in infected conventional mice is brought about mainly by the O2- (superoxide anion). These facts are believed to be invaluable footholds in elucidating the quantitative and qualitative differences in phagocytic cell response depending upon the presence or absence of intestinal flora.

[A case of psoriatic arthropathy complicated with clinical feature of ankylosing spondylitis].

UNLABELLED: A 25 years old man with psoriatic arthropathy complicated with clinical feature of ankylosing spondylitis was reported. 230 Japanese cases of psoriatic arthropathy were discussed. RESULT: 1) commoner in male 2) the cases that skin lesions were prior to arthritis and the cases that arthritis was prior to skin lesions were almost the same number 3) rheumatoid factor was seropositive in 17% of the cases 4) HLA-B27 was present in 9 of 50 cases and 7 of these 9 cases had central type arthritis 5) the cases had accompanied nail dystrophies in a high rate 6) concerning on skin lesion, many cases had atypical psoriatic lesion 7) 21.1% of the cases had central type arthritis 8) there were no certain effective therapy 9) prognosis of central type cases were relatively worse.

Pigmented dermatofibrosarcoma protuberans (Bednár tumor). An autopsy case with systemic metastasis.

An autopsy case of pigmented dermatofibrosarcoma protuberans (Bednár tumor) with systemic metastasis is reported. No previous example of this tumor showing widespread metastasis has been reported in the literature. The patient, a 45-year-old man, developed a tumor on the right upper arm. The tumor recurred twice and metastasized to other parts of the skin, lungs and brain during the 8-year clinical course. The primary tumor contained melanin-laden tumor cells and showed a storiform growth pattern. Autopsy confirmed multiple metastatic lesions in the skin, lungs, brain, thyroid, pancreas, stomach, small intestine and thigh muscles. The recurrent and metastatic tumors lacked both melanin production and the storiform arrangement, and instead revealed "fibro-sarcomatous" change with a herring-bone or interlacing pattern of growth.

[A case of Pryce's type I intralobar pulmonary sequestration diagnosed by noninvasive techniques].

The patient was a female who came to our hospital with a chief complaint of hemoptysis. Radiography of the chest demonstrated a funicular shadow, and CT of the chest also revealed a similar shadow in contact with the thoracic aorta enhanced by the contrast material. Pulmonary arteriography and aortography showed that the left S8, S9 and S10 areas were supplied by abnormal blood vessels bifurcating from the thoracic aorta, rather than the pulmonary artery. This finding was supported by scintigraphy of the pulmonary blood flow and pulmonary RI angiography. Since bronchography revealed no obvious abnormality in the bronchial system, the diagnosis of Pryce's type I intralobar pulmonary sequestration was established, and left lower lobectomy was carried out. While the evidence of abnormal blood vessels is important for the definitive diagnosis of pulmonary sequestration, concurrent image processing of the results of scintigraphy of the pulmonary blood flow and pulmonary RI angiography permits noninvasive demonstration of the fact that the sequestral pulmonary tissue is regulated by the systemic circulatory system, rather than the pulmonary circulatory system and thus is a useful technique.

Ecological studies on intestinal microbial flora of Kenyan children with diarrhoea.

The intestinal microflora was analysed together with short-chain fatty acids (SCFA) and bile acids in faeces from nine children with acute diarrhoeal disease in Lari, Kenya. Enteric pathogens such as enteroinvasive E. coli, enteropathogenic E. coli, Yersinia enterocolitica, rotavirus, Giardia lamblia and Entamoeba histolytica were isolated either singly or in combination from diarrhoeal faecal specimens. The most striking finding in these patients was a marked reduction of anaerobes. Analysis of the SCFA revealed a significantly higher quantity of the volatile fatty acids (VFA) such as acetic, propionic, and butyric acid in recovery period faeces in comparison to diarrhoeal faeces, although no significant difference was seen in the quantity of non-volatile fatty acids. On analysing bile acids in faeces, conjugated primary bile acids were detected from all cases in diarrhoea whereas the free form of secondary bile acids was seen only in recovery. The pH of recovery faecal specimens was significantly lower than that in diarrhoeal faecal specimens. There was a parallel between the decrease in number of anaerobes and fluctuation in the amount of SCFA, showing that the drastic reduction of VFA accompanying decrease of anaerobes during the diarrhoeal state, and the rise in pH thought to arise from these facts, result in an increase of water content.
