Laryngeal carcinoma. II. Analysis of treatment results using the Ellis model.

An analysis of 308 patients with carcinoma of the larynx was performed with respect to local recurrence and complications in relation to treatment level. The patients received primary irradiation with some difference in absorbed dose but with an equal fractionation schedule. Analysed in subgroups, correlations between local recurrence and treatment level were either demonstrated or strongly indicated. This is confirmed in a comparison with approximately 1 500 cases collected from the literature. Correlations between complications and treatment level were not found, but nevertheless indicated with respect to late edemas, which again are supported by comparison with approximately 800 cases collected from the literature. The risk of pharyngo-cutaneous fistula after total laryngectomy seems to be rather independent of irradiation level, although slightly dependent on field size. An optimum level of treatment has been estimated and brought into use since January 1978.

Laryngeal carcinoma. III. Treatment results in relation to microscopic score.

Long term treatment results for 254 patients with laryngeal carcinoma, primarily treated by irradiation, have been analysed according to a microscopic score, indicating the degree of malignancy. Deaths from carcinoma are significantly correlated to the microscopic score. It is strongly indicated that such correlations also exist for local recurrence/residual tumour and regional lymph node metastases. Therefore it is concluded that the microscopic grading system is a valuable prognostic factor supporting, and additional to, the T.N. classification. Larger series are needed in order to obtain correlations in subgroups, and accordingly primary therapeutic consequences can only be suggested, in doubtful cases.

Medulloblastoma. Treatment results.

A series of 44 patients with medulloblastoma is presented. The treatment was primary operation followed by irradiation of the entire CNS. The 3-year and 5-year survival rates for children and adults together were 36 and 33 per cent, respectively, and for children alone 37 amd 33 per cent, respectively. No certain difference in the prognosis for boys and girls was found.

Postoperative irradiation of glioblastomas. Results in a randomized series.

A material of 108 patients with glioblastoma is presented. The series was randomized in two groups: cases only operated upon and cases with postoperative irradiation in addition. Patients dead within 2 months after operation were excluded in estimating the real value of the postoperative irradiation. The irradiated cases had a 6-month survival rate of 64 per cent and a one-year survival rate of 19 per cent; the non-irradiated cases a 6-month survival rate of 28 per cent and a one-year survival rate of 0 per cent.

Malignant tumours of the oropharynx.

An analysis of a series of 127 patients with malignant tumours of the oropharynx is presented. All cases of epidermoid carcinoma were retrospectively classified according to the TNM system. Primary treatment consisted in irradiation in all cases of malignant lymphoma and epidermoid carcinoma. The corrected 5-year survival rate for patients with malignant lymphoma was 53 per cent, and for those with epidermoid carcinoma 40 per cent. In epidermoid carcinoma, 60Co treatment seemed to result in a better prognosis than 250 kV radiation therapy. Both in malignant lymphoma and epidermoid carcinoma, the prognosis was definitely related to the spread to regional lymph nodes.

Glomus jugulare tumours. (A series of 21 cases).

Twenty-one cases of glomus jugulare tumours were analysed in retrospect. Six had been treated surgically, ten by irradiation, and five by a combination of incomplete surgical excision followed by irradiation. Recurrences appeared in four cases, but no patient has died of the tumour during the follow-up period which is from a few months to eleven years. Metastases were not found in any case. The optimal time of irradiation in combined treatment is discussed.

Radiography of laryngeal carcinoma. Assessment of value.

The value of various radiographic methods (except laryngography) in laryngeal carcinoma was assessed in a material of 71 patients. Radiography proved of no value in T1 and T2 tumours if a satisfactory clinical examination could be performed but gave valuable information in recurrences as well as in T3 and T4 cases.

Malignant tumours of the nasopharynx.

A series of 115 patients with malignant tumours of the nasopharynx were primarily irradiated. All the cases of carcinoma were TNM classified in retrospect. 60Co irradiation appeared to afford a somewhat better prognosis than conventional irradiation previously used. The 5-year crude survival rate for the patients with lymphoma and with carcinoma was 40.0 per cent and 23.8 per cent respectively. A distinctly better prognosis was found for women than for men and the prognosis was independent of whether or not lymph node metastases were present. There seems to be no indication for maintaining lympho-epithelial carcinoma as a special group of tumour. Surgical procedures are rarely indicated in the treatment of malignant nasopharyngeal tumours.

Epidermoid carcinoma of the lip. Histologic grading in the clinical evaluation.

A retrospective microscopic evaluation of primary biopsies was carried out in a material of 438 patients with carcinoma of the lip. A multifactorial grading system consisting of 8 parameters, each sub-divided into 4 grades, was used. The degree of malignancy showed a statistically significant correlation to the frequency of metastases, frequency of recurrence or residual tumour, mortality and T classification. This grading system may allow a separation of risk groups in which special treatment and follow-up should be applied.

Epidermoid carcinoma of the tongue. Histologic grading in the clinical evaluation.

In a series of 49 patients with epidermoic carcinoma of the tongue microscopy of the primary biopsy was performed, using a multifactorial grading system consisting of 8 parameters, each of 4 grades. The score value (degree of malignancy) revealed a statistically significant correlation with the frequency of regional metastases. As this was particularly marked in the T1 and T2 groups, it seems to be an important supplement to the clinical evaluation of risk groups.

Induction of leukemia by 131-I treatment of thyroid carcinoma.

The records of 194 patients with thyroid carcinoma treated with (131)I, representing all cases thus treated in Denmark from 1948 to 1972, were reviewed. Two cases of myeloid leukaemia were found compared with 0·097 expected cases of non-lymphocytic leukaemia (0·05 > P > 0·01). In 5 series of (131)I treated thyroid carcinomata, 10 cases of myeloid leukaemia occurred in a total of 487 patients, corresponding to a frequency of leukaemia of about 2%. These findings appear to show that (131)I treatment of thyroid carcinoma is associated with a certain risk of development of leukaemia. This risk must be considered when treatment of localized thyroid carcinoma is planned.
