[The results and prospects of preventing arterial hypertension in Ukraine].

An analysis is presented of the status of arterial hypertension prophylaxis in the Ukraine. Drawbacks in solution of this problem are discussed: diagnosis is incomplete, prophylactic management of hypertensive disease is inadequate. Ways and means of improving the diagnosis and prophylaxis of hypertensive disease in the Ukraine including better remuneration of the medical personnel and interests of the population in preserving their health are discussed.

[Scintigraphy of the myocardium in ischemic heart disease].

Altogether 106 patients with different types of acute CHD (large and small local MI, unstable angina) and stable angina were investigated. Combined assessment of perfusion disorder permits differentiation of necrotic and ischemic myocardial lesions. A degree and type of RP accumulation corresponds to a size of necrotic myocardial lesion determined by means of biochemical markers of necrosis. Parallelism of myoglobin concentration, isoenzyme activity in the blood serum and the results of scintigraphy was revealed. In focal RP accumulation, myoglobin concentration reached maximum values; a moderate increase and decrease up to normal values in the absence of accumulation were observed in diffuse accumulation. Diffuse RP accumulation in patients with stable and unstable types of angina was indicative of transient perfusion disorders resulting from myocardial ischemia.

[Errors in postmortem diagnosis in cardiovascular diseases].

An analysis of out-patients case histories and death certificates indicates that in 22.5-35% of cases the diagnoses in death certificates did not correspond with data of examinations carried out during life when pathological examination was not made; when pathological examination or forensic studies were carried these errors reduced to 10-20%. In another 11.5% the diagnosis of ischemic heart disease was not substantiated because additional examination with the purpose to verify the diagnosis were not carried out.

[Functional status of the myocardium of ischemic heart disease patients during long-term treatment with beta blockers].

Propranolol treatment for 6, 12 and 24 months was associated with a pronounced antianginal effect, both subjective and objective, in coronary patients, while the side effects rate was relatively small (3.3-7.3%). As the duration of treatment increases, the negative inotropic effect becomes less marked. Long-term propranolol treatment of coronary patients does not provoke any clinical signs of circulatory insufficiency. The treatment has the best effect in those coronary patients whose "double product" increases by more than 1.0 per 1 W of a standard 50 W load. The drug maximum antianginal effect is retained when this index drops below 1.0 in the course of two-week and six-month treatment.

[Blood vasopressin and the kallikrein-kinin system in chronic ischemic heart disease].

The blood vasopressin content and kinin system activity were studied in 83 patients with chronic ischemic heart disease of various severity. It was established that the blood vasopressin concentration increases with an increase in the severity of the disease. A high degree of correlation between changes in vasopressin content and in components of the kinin system was revealed: increase in the concentration of vasopressin is attended by activation of the kallikrein-kinin system. The mechanisms of the interrelationship of the vasopressin and kinin content are discussed. It is suggested that vasopressin may play a role in the pathogenesis of chronic ischemic heart disease.

[The status and outlook of dispensary care for patients with cardiovascular diseases].

On the basis of the surveys carried out the authors showed inferior quality and low efficiency of dispensarization of patients with cardiovascular diseases, which fact is traceable back to obsolete criteria for assessment of physicians' preventive activities, lack of association between the results on securing dispensarization and economic parameters, and shortage of medical experts in preventive cardiology. It is necessary that the established priorities of activities in administration of prophylaxis of cardiovascular diseases and complications from these be reconsidered, cerebrovascular pathology in the presence of arterial hypertension and without it given more importance, multicenter programmes on prophylaxis of chronic non communicable diseases developed and inaugurated, with a possibility of these being included into international projects.
