Loneliness, emotional repression, marital quality, and major life events in women who develop breast cancer.

Women awaiting mammograms at a breast clinic were given questionnaires to investigate the role of psychosocial variables in the development of breast cancer while controlling for established breast cancer risk factors. Questionnaires to determine loneliness, emotional repression, marital quality, and major life changes were completed by 826 female volunteers who were later classified into groups according to their diagnoses. The total emotional repression score showed a hierarchy of most repression to least repression for the most-diseased to the most-healthy subjects. A breakdown of the emotional repression scale revealed that each group was significantly different from the other in suppression of anger and unhappiness. Women in the new cancer group showed significantly more loneliness than the women in the fibrocystic and normal groups. The newly diagnosed cancer group also had a higher proportion of women who experienced the death of a spouse or close family member within the past two years compared to the other groups.

Mammography utilization in the poor and medically underserved.

There is a significant difference in the utilization of mammography for screening and diagnosis between women of lower socioeconomic groups and the rest of the female population. Information obtained by the American Cancer Society through public hearings in 1989 revealed significant problems in how the poor deal with cancer. Barriers to methods of early detection seemed to revolve around access, education, and cost. A recent study of breast health performed by the Wirthlin Group for the American Cancer Society showed similar barriers. All women, regardless of income, expressed the fear of finding breast cancer as one of the major barriers to having mammography. Although the poor and medically underserved group of women showed similar barriers to mammography, certain barriers seemed specific to ethnic group, age, and place of residence. Evaluation of these barriers to breast health will allow the development of specific programs to meet the needs of this population.

Fine needle aspiration biopsy of the breast using ultrasound techniques--superficial localization and direct visualization.

From July 1982 to December 1987, 519 fine needle aspiration biopsies were performed at the Indianapolis Breast Center. Both the superficial localization and the direct visualization methods were used. The fine needle aspiration biopsy technique (FNAB) was definitive in 92% of malignant masses, with minimum of 6 month follow up to date. The technique significantly alters the traditional approach to the management of breast cancer in that it allows preoperative discussions of options for treatment and eliminates the cost of excisional biopsies before definitive therapy.

Ultrasound in the evaluation of solid breast masses.

Ultrasound image characteristics and radiographic features of 31 benign and 41 malignant breast masses were cross-tabulated and analyzed to determine the ultrasound image characteristics most useful for diagnosis and the frequency with which some imaging features occurred. The most common malignant mass (the ductal carcinoma) exhibited a jagged wall (88%), homogeneous internal echoes (12%), nonhomogeneous internal echoes (70%), internal echoes not discernible due to attenuation effects (18%), and attenuation shadowing (97%). In contrast, the fibroadenoma (the most common benign mass in this study), exhibited smooth walls (94%), homogeneous internal echoes (89%), and no demonstrable posterior shadowing (67%).

Factors critical to highly accurate diagnoses of malignant breast pathologies by ultrasound imaging.

In an interdisciplinary program on diagnoses of breast pathologies by ultrasonic imaging, analyses of results have been completed for a symptomatic population totaling 1892 subjects. Application of a comparatively simple ultrasound instrument in conjunction with specific examination techniques has allowed the achievement of a diagnostic accuracy of 95% for the 73 malignant masses which have been encountered in the examined population.

A preliminary analysis of the ultrasound imaging characteristics of malignant breast masses as compared with x-ray mammographic appearances and the gross and microscopic pathology.

Thirty correctly diagnosed carcinomas were chosen for retrospective analysis of their ultrasound and X-ray imaging characteristics, and correlated with pathologic examination. It was determined that the ultrasound image correlated will with that revealed by radiographic techniques. The posterior attenuation shadow and the jagged wall were the prime indicators of malignancy in this study, occurring in 90 and 87% of the cases, respectively. However, in about 67% of these masses, attenuation shadowing was revealed only by close-interval (1mm) stepwise scanning. A nonhomogeneous internal echo pattern was found in 73% of the cases. Thirteen of the 30 masses showed disturbed architecture away from the overt lesion when imaged by ultrasound techniques. Seventy per cent of these showed histological abnormalities in remote regions. Finally, it was shown that the attenuation shadowing exhibited by malignant breast masses is related to the collagen content of the lesion, expressed as fibrosis.

Emergency room radiography of asthma: an efficacy study.

The chest radiographs of asthmatic patients in an emergency room setting over a four-year period were reviewed. A total of 997 radiographs was reviewed; radiographic findings were compared with history and clinical data. In adults 2.2% of the radiographs were abnormal; in children 13%. It is concluded that a chest radiograph in adults in unnecessary unless the patient is unresponsive to bronchodilators and is being admitted to the hospital. In children, however, if rales and rhonchi are present, in addition to wheezing, a chest radiograph may be useful.

Spectrum of angiographically demonstrable renal pathology in young hypertensive patients.

The renal diseases which cause systemic hypertension in the first two decades of life differ from the adult in their incidence and etiology. Seventeen patients (11 days to 20 years old), studied angiographically, demonstrated a wide spectrum of renal pathology including arterial thrombosis, fibromuscular hyperplasia, vasculitis, neurofibramatosis, cystic disease, pyelonephritis, Page kidney, and congenital anomalies.
