[A new method for determination of antithrombin III and its diagnostic significance].

A new assay based on using an original chromogenic substrate (z-Ala-Ala-Arg-pNa) and designed for the determination of antithrombin III is described in the paper. It was used in examinations of patients with malignancy after surgery. The content of antithrombin III was detected to be lower in 12% of cases in 2 days after surgery. Additionally, patients with DIC, as complications after surgery, were examined to find out that the antithrombin III content was decreasing in them.

[Immunostimulating and anticoagulating activity of fucoidan from brown algae Fucus evanescens of Okhotskoe sea].

Fucoidan--nontoxic sulfated polysaccharide was isolated from brown algae Fucus evanescens in Okhotskoe Sea. Chemical analysis of the compound was performed, it was shown that fucoidan is freely soluble in water and acid solutions. Immunotropic and anticoagulating properties of the compound were evaluated in comparison with heparin. It was demonstrated that fucoidan in wide range of doses stimulated phagocytic and bactericidic activity at leucocytes of mice peritoneal exudate. Heparin on the contrary demonstrated depressive effect on these functions at high dose. It was shown that fucoidan has dose-dependent anticoagulating activity in vitro and in vivo comparable with heparin activity. The results of investigation demonstrated possibility of fucoidan application as immunomodulating and anticoagulating agent of plant origin.

[A method for determining plasminogen with a Russian chromogenic substrate and its diagnostic significance].

Measurement of plasminogen, the key component of fibrinolysis system, is one of the basic methods for estimation of fibrinolysis. Methods based on the use of chromogenic substrates are often used in diagnosis. Plasminogen measurements are important for laboratory diagnosis of thrombophilia caused by deficiency or abnormalities of this fiber, for detection and evaluation of the DIC syndrome, and for monitoring the treatment by fibrinolytic preparations (streptokinase, t-PA, urokinase, etc.). An original chromogenic substrate having no foreign analogs has been created at Institute of Genetics and Selection of Industrial Microorganisms and Research Center of Hematology (Moscow). Unlike previously described plasmin substrates, pNa has been obtained by microbiological methods with Russian commercial enzymes subtilisine 72 and megaterine. This paper presents the results of plasminogen measurements in patients with DIC with the use of the original chromogenic substrate. The results were compared with those of tests with Berihrom-Plasminogen diagnostic kit (Behringwerke AG).

[Use of elimination of plasma phospholipid membranes for detection of "lupus anticoagulant" effects].

The coagulation activity of plasma phospholipid membranes (PM) was measured in plasma depleted for platelets (PDP) in 50 normal subjects and 33 patients with the antiphospholipid syndrome (APS) and the "lupus anticoagulant" in the plasma. The normal value for phospholipid activation of clotting was 99.8 +/- 3.5% (from 75 to 125%), whereas in patients with APS and lupus anticoagulant it was 42.1 +/- 8.2% (p < 0.001). Addition of PM from patients' PDP to normal plasma free from PM did not normalize clotting. Addition of PM from normal plasma to patients' PDP normalized the clotting time. Therapy with discrete plasmapheresis increased the phospholipid activation value in the patients from 42.1 to 73.3% (p < 0.01), which was due to removal of the PM-antiphospholipid antibody complex from PDP. The proposed microfiltration method can be used in the complex of tests for detecting the lupus anticoagulant in patient's plasma.

[Disorders of the hemostasis system in late pregnancy toxemia and their correction].

The coagulation system has been examined in 30 women with late toxemia before and after heparin and trental therapy. Pretreatment fibrinogen levels were elevated and antithrombin III levels were reduced. A hemolysate-calcium mixture showed an increase in coagulation at 8 and 10 minutes of incubation. Staphylococcus agglutination and ethanol tests revealed a significant increase in early PDF and other constituents of the fibrinogen pool. These changes were indicative of prolonged-type disseminated intravascular coagulation in late toxemia. Comprehensive therapy comprising heparin and trental leads to a significant reduction in fibrinogen concentration, alleviation of the hypercoagulatory shift and increase in antithrombin III level. Early PDF and fibrin monomer complexes showed a decline. In addition, the therapy had beneficial effects on the maternal and fetal clinical status.

[Comparative study of various rapid methods of determining fibrinogen transformation products in the diagnosis of intravascular coagulation and fibrinolysis].

Rapid methods for determination of fibrin-monomer complexes and fibrinogen/fibrin degradation products were studied and compared in 76 patients with different abnormalities in the hemostatic system (acute thromboses, thromboembolism of the pulmonary artery, disseminated intravascular coagulation, immune thrombovasculitis, etc). The control group consisted of 36 healthy donors. The fibrin-monomer complexes were determined by the paracoagulation tests, the ethanol test (ET) and protamin sulfate tests (PST), whereas fibrinogen/fibrin degradation products (FDP) by the staphylococcus adhesion test (SAT) in which use was made of the Newman D2S strain variety obtained by the authors. It is inferred that the ET, PST and SAT are the most suitable for use in clinical medicine, since they are accessible, simple and quick in performance. However, these tests cannot be regarded as similar or interchangeable, since they are used for studying different products of the coagulation and fibrinolytic transformation of fibrinogen.

[Structure, function and the role of factor XIII in human pathology].

Modern views on the functioning of coagulation system are constantly replenished with the facts obtained using the newest methods of molecular biology and medicine. The purpose of many components of this system is non-uniform and covers the whole spectrum of biochemical reactions responsible for homeostasis. Current data on one of such components--the factor XIII (fibrin stabilizing factor) are stated. Except for the description of the enzyme structure and functions, information is presented about the methods of its detection, which development has resulted in accumulation of knowledge on the role of fibrin stabilizing factor in bleedings and thrombosis genesis. Data are given on hereditary and acquired deficiency of the factor XIII, as well as on conditions accompanied with the increase of its activity.

[Effect of anesthesia and anticoagulant prophylaxis on the occurrence of postoperative thromboembolic complications in orthopedic patients].

Different prevention regimens of venous thromboembolism, by using the low molecular-weight heparin Fraxiparin, were studied in 152 patients who had undergone major orthopedic leg surgery under spinal and general anesthesia. The regimens differed in the time of Fraxiparin administration (before or after surgery). Fraxiparin and spinal anesthesia were found to promote the lower incidence of venous thromboembolism. The preoperative preventive treatment of venous thromboembolism, by using a low molecular-weight heparin in patients undergoing spinal anesthesia is unnecessary because of a high risk of epidural hematomas. If the risk for venous thromboembolism is higher before surgery and remains the same after it, thromboprophylaxis should be prolonged for at least one month.

[Effect of heparin on fibrinogen formation during experimental toxic syndrome of disseminated intravascular coagulation].

The formation of circulation components of fibrinogen pool in toxigenic syndrome of disseminated intravascular blood coagulation (DBC-syndrome) caused by the Vipera lebetina turanica venom has been examined by the gel-filtration method. Simultaneously it has been studied what components of fibrinogen pool are removed in paracoagulation tests and with addition of the coagulating enzymes (thrombin, reptilase and the Echis multisguamatus venom) to the plasma. The preliminary heparinization of animals poisoned by Vipera lebetina turanica venom was found to prevent the fibrinogen formation mainly at the fibrin-monomer formation stage. Besides, the effect of polymerization inhibition was revealed in the plasma of such animals.

[Significance of heparin elimination from plasma in the assessment of coagulogram and antithrombin III activity].

An original adsorbent for non-fractionated and low-molecular weight heparin (fraxiparin,Sanofi) has been developed and tried in experiments and in a clinical setting. Tests carried out in 58 patients with disseminated intravascular coagulation and thromboses treated with heparin demonstrated the possibility of obtaining objective data on the status of the blood clotting system. Heparin adsorption is particularly important in measurements of heparin III activity, for even low concentrations of heparin mask the defect of this anticoagulant.

[A new sorbent for hemosorption].

SIAL, a new granulated white adsorbent, has been obtained. It is characterized by highly porous surface with hydrophilic-hydrophobic properties due to combined effects of the mineral matrix (aluminum oxide) and the coating (organosiloxane fluid) permitted for medical use. The detoxication properties of the adsorbent have been demonstrated in experimental hemosorption. It was shown to exert a moderate effect on the contact phase of blood clotting and on platelet aggregation characteristics.
