[Experience in the surgical treatment of neovascular diabetic glaucoma].

A new operation for neovascular glaucoma, filtrating vitrectomy, is described. The operation is based on creation of a permanent microfistula in the posterior chamber of the eye and simultaneous replacement of the viscous vitreous body with the isotonic saline. Filtrating vitrectomy was performed in 46 eyes with neovascular diabetic glaucoma. The intraocular pressure was normalized completely without medication in 70 per cent cases in the early postoperative period and in 66 per cent cases in the long term (12-39 months) period.

Filtering iridocycloretraction in chronic closed-angle glaucoma.

In 98 patients, 112 eyes with chronic closed-angle or combined glaucoma were operated on using filtering iridocycloretraction, including preparation of two limbal-based scleral strips that were introduced through scleral incisions into the anterior chamber and the supraciliary space. The intraocular pressure was controlled (less than or equal to 21 mm Hg) in 107 eyes by surgery alone and in two eyes with medication by three to four weeks postoperatively. Ninety-one eyes were followed up for 12 to 30 months (mean, 19 months). The intraocular pressure was controlled in 78 eyes by surgery alone and in four eyes with medication. Three eyes were operated on repeatedly.

Surgical stimulation of the uveoscleral outflow. Experimental studies on enucleated human eyes.

Six series of experiments were performed on 45 isolated cadaver eyes. In the first series, the outflow facility of the human sclera was measured, in the others, five surgical techniques for stimulating the uveo-scleral outflow were evaluted with the help of a constant pressure perfusion technique. It was found that the scleral C-value was pressure independent and its average value was equal to 0.0036 mm3/min/mmHg per 1 cm2 (100 mm2) of scleral surface. After the implantation of a scleral strip into the supra-uveal space, the mean increase in the total C-value was 23% of its initial value, ranging from 7 to 35%. After cyclodialysis the C-value increment varied from 17% to 66%, averaging 30%. The average increase in the outflow facility amounted to 60% (range 24--96%) in those cases where implantation of the scleral strip in the suprauveal space was combined with cyclodialysis. After implantation of a scleral strip, combined with subscleral sinusotomy, the C-value increment varied from 18 to 71%, averaging 41%. After cyclodialysis combined with goniospasis, the mean increase in the outflow facility coefficient was 62% (range 43%--108%).

Implantation of a scleral strip into the supraciliary space and cyclodialysis in glaucoma.

Since 1975 the operation of implantation of a scleral strip (10 mm long and 2 mm wide) has been performed on 75 glaucomatous patients. The following variants of surgical technique were used: (1) implantation of an autoscleral strip into the supraciliary space, (2) implantation of an autoscleral strip combined with cyclodialysis and (3) implantation of a homoscleral (cadaver) strip combined with cyclodialysis. The first technique was employed in 12 cases. The intraocular pressure was controlled by surgery alone in only four eyes, though a decrease in the level of the intraocular pressure and an increase in the C-value occurred in each case. No significant difference between the results obtained with the second and the third techniques was found. Of 63 eyes operated on, the intraocular pressure was controlled in 41 eyes (65%) by surgery alone and in 11 other eyes with the help of miotics 3 to 4 weeks after surgery. Follow-up was for a period of from 12 to 23 months in 51 cases. The pressure was under control in 36 eyes (71%) without medication and in 10 more eyes with miotics. The rate of complications was low. Hyphaema appeared in 10 cases and mild iridocyclitis occurred in 11 cases. There was no case of choroidal detachment, persistent hypotony or flat anterior chamber after surgery.

Early and late results of fascia lata transplantation in high myopia.

Transplantation of a strip of fascia lata to strengthen the posterior sclera in high progressive myopia was performed on 184 eyes of 108 patients with myopia ranging from 7 to 39 D. Three weeks after surgery visual acuity with correction had improved by 0-1 to 0-5 in 151 eyes (82 per cent) and refractive power had decreased by 1 to 8 D in 169 eyes (92 per cent). In 105 eyes (67 patients) that were followed-up for periods of from 1 to 9 years myopia increased in six by 0-5 to 3 D. In 29 patients who had an operation on only one eye and were then followed-up for periods of from 1 to 7 years the degree of myopia in the fellow eye increased by 1-5 to 6 D in 24 (83 per cent), and of the eyes that were operated on myopia increased by 0-5 to 3 D in only 5 (17 per cent).

Results of the carotid and the differential jugular compression tests in normal and glaucomatous patients.

A carotid compression tonographic test was performed in 53 normal persons and 40 patients with primary open-angle glaucoma. Fifty normal and 58 glaucomatous patients were investigated by a differential jugular compression test. In the glaucomatous eyes as compared to the normal eyes the volume changes in the eyeball were found to be significantly greater during compression of the common carotid artery and significantly less under the influence of the jugular compression test.
