[Guidelines for quality management of dialysis solutions].

A Best Practice Guideline about Dialysis fluid purity has been developed under the leadership of the Spanish Society of Nephrology. The Guideline has established recommendations for standards for preparing dialysate: water, concentrates and hemodialysis proportioning systems. The Guideline was based on the European pharmacopoeia, the Real Farmacopea Española, the AAMI Standards and Recommended Practices, European Best Practice Guidelines for Haemodialysis (Section IV), literature reviews, according to their level of evidence, and the opinion of the expert spanish group. Two levels of quality of water were defined: purified water and high purified water (Ultra pure) and for dialysate: standard dialysate and ultra pure dialysate. Regular use of ultra pure dialysate is necessary for hemofiltration and hemodiafiltration on-line and desirable for high-flux hemodialysis to prevent and delay the occurrence of complications: inflammation, malnutrition, anemia and amyloidosis. Water, concentrates and dialysate quality requirements are defined as maximum allowable contaminant levels: chemicals (1.1.2), microbial and endotoxins: [table: see text] Monitoring frequency, maintenance and corrective actions were specified. Methods of sampling and analysis were described in appendix (Anexos). For microbiological monitoring, TSA or R2A medium are recommended, incubated during 5 days at a temperature of 30-35 degrees C. The dialysate quality assurance process involves all dialysis staff members and requires strict protocols. The physician in charge of hemodialysis has the ultimate responsibility for dialysate quality. All suggestions and questions about this Guideline are wellcome to www.senefro.org

[Errors in bibliographic references of Nefrología from 1981 to 1995. A quality control].

UNLABELLED: The accuracy of the reference list is considered an essential factor in determining the quality of a scientific publication, and has been reviewed and published in several medical journals. The rate of citation errors in NEFROLOGIA has not been published before. The aim of this study has been to examine the accuracy of the references citation in the numbers of the journal published for a period of 15 years, from the first issue in 1981 to the last one on 1995. MATERIAL AND METHODS: In this period 68 ordinary issues of the journal were published, with 520 original articles that constituted the data base, 87 articles were selected applying a table of random numbers with 5 references of each article selected to be reviewed with a total of 435. The accuracy of the name of the authors, title of the article, volume, initial and last page and year of publication were evaluated, accordingly with the NEFROLOGIA normative. Citation errors were considered major or minor depending on whether they prevented or not the immediate retrieval of the article listed. RESULTS: Of the 435 citations examined, 433 could be located, being impossible in 2 with the data supplied. In 189 (43.45%) 237 errors were detected, with 13.50% of them being considered major and 86.50% minor, with the greatest percentage of errors occurring in the data about the authors of the articles referenced 96 (40.50%). CONCLUSIONS: The issues of NEFROLOGIA published in this period present a high number of bibliographic reference errors, being the rate similar to other evaluated scientific periodical journals. This fact should be improved, and authors and reviewers should thoroughly check their references in order to avoid these mistakes, as they affect the quality standard of the publication.

[Recurrent perforation of of the sigmoid colon associated with Ehlers-Danlos syndrome type IV].

Ehlers-Danlos syndrome type IV is a heritable disease of type III collagen metabolism. Most patients have a defect either in the synthesis or structure of type III procollagen, a finding consistent with the fact that these patients are prone to spontaneous rupture of the aorta and intestines, tissues rich in type III collagen. The diagnosis is suspected in a patient with a combination of clinical manifestations and family history, but it is confirmed only by culture of the patient's skin fibroblasts and the demonstration of a defect in type III collagen metabolism. We present a patient with Ehlers-Danlos type IV syndrome who developed a recurrent colon perforation, and discuss the surgical strategy to prevent recurrences.

[Intervention with community health agents in immunization programs in the gypsy population].

OBJECTIVE: To measure how the vaccination coverage of the Gypsy child population in the Cartuja and Almanjayar quarters evolved throughout a Health Education Intervention using Community Health workers belonging to the Gypsy community. DESIGN: A descriptive study, pre-test/post-test, with no control group. SETTING: Community level. Primary Care. PATIENTS AND OTHER PARTICIPANTS: The whole of the Gypsy child population under 14, consisting of 1,073 children, and four Community Health workers. INTERVENTIONS: Health Education for mothers and carers, by means of home visits carried out by four Health workers, previously trained and contracted for this purpose for a year by the Andalusian School of Public Health. MEASUREMENTS AND MAIN RESULTS: We measured the Vaccination Coverage in September 1988 (42.2%) in order to establish the situation both at the start of the intervention in March 1990 (46.2%) and at its end in March 1991 (68.2%). We measured Vaccination Coverage of the different vaccine doses through frequency allocations. CONCLUSIONS: The Health Centre increased Vaccination Coverage by 4% in 18 months. The intervention increased it by 22.2% in 12 months. This demonstrates the intervention's effectiveness and underlines the importance of carrying out this kind of intervention when working with at-risk population groups.

[Non-A non-B hepatitis associated with cytomegalovirus infection].

A clinical case of a 58 year old male suffering from hepatitis non A, non B, and terminal chronic renal insufficiency under periodic dialysis, is presented. Following a 600 cc of packed red blood cells transfusion, there was an increase of antibodies IgG against cytomegalovirus (from 1/160 to 1/1560), detected by ELISA. At the same time, there were an increase of transaminases up to 404 UI/ml of GPT. Total bilirubin persisted normal. The possible etiology of the hepatitis non A, non B caused by cytomegalovirus is discussed and the results compared to those observed by other authors in immunosuppressed renal transplant patients.

[A case of rheumatoid factor associated with pancreatic carcinoma].

A case of adenocarcinoma of pancreas's tail, whose first manifestation was a prolonged fever syndrome, is presented. The only laboratory feature was the presence of a rheumatoid factor which appeared weeks before the diagnosis was made. There was no other pathology to justify its presence. The levels of RF decreased after the extirpation of the tumor and, coincidentally, with the clinical improvement. The level of RF was normal 4 months after the operation. The presence of RF in neoplasia and its possible relation to tumoral evolution is discussed.

[Corynebacterium D2-induced encrusted cystitis].

Two cases of patients with cystitis cystica secondary to urine infection, caused by resistant corynebacterium D2, are presented. The clinical features were important dysuria and frequency, alkaline urine Ph (Ph 8 and 9). The urine cultures were obtained with the standard techniques, after 72 hours of incubation. They were treated with vancomycin, ciprofloxacin and hydroxinamic acid with good response.

[The incidence of neoplasms of the epithelium associated with monoclonal gammopathy at a general hospital. A 6-year analysis].

The epithelial neoplasias associated to monoclonal gammapathy (GM) seen in a general hospital over a period of six years are reviewed. 8 cases were detected, 2 of which behaved as a second neoplasia since they occurred in patients previously diagnosed of multiple myeloma. The incidence and characteristics of the monoclonal component of the neoplastic process are described as well as its possible etiopathogenic implication.

[Erectile dysfunction of venous origin: diagnosis by means of cavernometry].

We report on a case of erectile dysfunction from venous leakage diagnosed by cavernometry and resolved by ligation of the vena dorsalis penis. We discuss the possibility of suspected venous leakage by penile arterial evaluation with Doppler before and after intracavernous papaverine injection. Diagnosis is established by cavernometry.

[Incidental adrenal tumor: non-functioning cortical adenoma].

Adrenal incidental tumors are masses incidentally discovered on ultrasonography or CT for other abdominal disorders. Management depends on tumor size, nature, content, and hormonal behaviour. We report on a large, nonfunctioning adrenocortical adenoma that was incidentally discovered in a 59-year-old male patient with right-sided costo-lumbar trauma.
