The gene encoding the capsid protein P82 of the Choristoneura fumiferana multicapsid nucleopolyhedrovirus: sequencing, transcription and characterization by immunoblot analysis.

A gene encoding a capsid-associated viral structural protein has been identified and sequenced in the genome of the Choristoneura fumiferana multicapsid nucleopolyhedrovirus (CfMNPV). The gene has a 1872 nucleotide open reading frame (ORF) encoding 624 amino acids with a predicted molecular mass of 71.4 kDa. Transcription, which appeared to be initiated from a conserved GTAAG motif of baculovirus late genes, was detected at 12 h, reached a maximum at 48 h and declined at 72 h post-infection (p.i.). Part of the ORF was cloned in frame into a prokaryotic expression vector, pMAL-c2, and the fusion protein was used to generate antibodies in rabbits. It was shown, with the aid of the polyclonal antiserum, that this viral protein was detectable at 24 h p.i. in infected cells. The protein appeared as an 82 kDa band in occlusion-derived virus and as an 82 kDa band and a 72 kDa band in budded virus. Amino acid sequence comparisons revealed that this ORF had high homology with the ORF p87 (77% similarity) of Orgyia pseudotsugata (Op) MNPV and the ORF p80 (60% similarity) of Autographa californica (Ac) MNPV. Immunoblots confirmed that the CfMNPV protein had antigenic similarities to the P87 protein of OpMNPV, but not to the P80 of AcMNPV.

Molecular characterization of haemophilia A in southern Chinese.

41 unrelated southern Chinese haemophilia A patients were studied. The 5' promoter region, all 26 exons, their immediate 5' and 3' flanking splice junctions and the 3' untranslated region of the FVIII gene were amplified (including 16 different segments of exon 14) using GC-clamped primers. The GC-clamped PCR products were screened by denaturing gradient gel electrophoresis (DGGE) and fragments showing an abnormal migration pattern were sequenced. 17 mutations were identified, of which four were splicing defects, involving the first 1-6 nucleotide (nt) in the intervening sequences (IVS), six missense mutations, three nonsense mutations and four frameshift mutations. 14 other patients carried the type 1 inversion, affecting the distal copy of the FSA gene at the telomere of the X chromosome and the same gene in intron 22 of the FVIII gene. All the mothers studied (12/14) were carriers of the inversion. Two of these patients with inversion also have a co-existing missense mutation. In most cases the clinical severity of the disease corresponds to the genotype.

Bipolar affective disorder minus left prefrontal cortex equals schizophrenia.

BACKGROUND: An investigation of the relationship between bipolar affective disorder and schizophrenia, following a severe head injury and removal of the left prefrontal cortex. METHOD: A single case report. RESULTS: An individual with past history of bipolar affective disorder suffered traumatic damages to the left prefrontal cortex with a second lesion in the left temporal lobe. The patient developed typical schizophrenia nine months later. The relevance of his brain lesions in determining the schizophrenic symptoms is discussed. CONCLUSION: We propose that the specific pattern of brain injury in this patient was sufficient to change the phenotype from bipolar affective disorder to schizophrenia.

Co-carcinogenesis and field cancerization: oral lesions offer first signs.

This article describes two cases of multifocal synchronous cell squamous cell carcinoma in high-risk patients. The initial presentation in both instances was an erythroplakic oral lesion. The importance of the team approach between the dentist and the oncologist for quality patient care is underscored.

Phase II study of docetaxel for recurrent or metastatic non-small-cell lung cancer.

PURPOSE: We conducted a phase II study to determine the response and toxicity of docetaxel (Taxotere; Rhône-Poulenc Rorer Pharmaceuticals, Inc, Collegeville, PA) in chemotherapy-naive patients with advanced non-small-cell lung cancer. PATIENTS AND METHODS: We treated 41 chemotherapy-naive patients who had stage IIIb or IV non-small-cell lung cancer with 100 mg/m2 of docetaxel intravenously over 1 hour every 3 weeks. Responses were assessed after every one to two treatment courses. Responses of 39 of 41 patients were assessable. The patient's median age was 63 years; 90% of patients had a Zubrod performance status of 0 or 1. The predominant histology was adenocarcinoma (54%), and 90% of patients had stage IV disease. RESULTS: Thirteen patients (33%) achieved a partial response to treatment, and the median response duration was 14 weeks. Grade 3 or 4 neutropenia occurred in 97% of patient; this was usually of brief duration and was associated with serious infection in 17% of patients. Other acute toxic effects included easily treated hypersensitivity reactions (36% of patients) and dermatitis (74%). We also observed fluid retention (with peripheral edema or pleural effusion or both) in 54% of patients. This was a cumulative side effect that generally occurred late in treatment. CONCLUSION: Docetaxel administered at 100 mg/m2 intravenously every 3 weeks has significant activity against non-small-cell lung cancer, with a 33% major response rate. Primary toxicities were neutropenia, hypersensitivity, and fluid retention.

Contents of trace elements in the hair of aplastic anemia patients and their treatment based on an overall analysis of symptoms and signs.

The content of 12 trace elements in the hair of 20 aplastic anemia patients was determined and compared with that of normal subjects as control. The results showed that patients with deficiency of yin had a significant decrease in lithium, calcium, strontium, and chromium, those with deficiency of yang had a distinct decrease in zinc magnesium barium, strontium, calcium, and lithium, and those with deficiency of both yin and yang had a general decrease in all the 12 trace elements. Changes in trace element content in hair may serve as a guide to opening up new vistas in the treatment of aplastic anemia on the basis of an overall analysis of symptoms and signs.

Alleviation by leucovorin of the dose-limiting toxicity of edatrexate: potential for improved therapeutic efficacy.

Edatrexate (10-ethyl-10-deaza-aminopterin; CGP 30 694) is a methotrexate (MTX) analogue that shows promise against non-small-cell lung cancer (NSCLC) and other tumors. Since edatrexate's mechanism of action is the same as that of MTX, we used leucovorin in an attempt to alleviate its dose-limiting toxicity, stomatitis. In four patients with NSCLC who had experienced significant stomatitis after treatment with edatrexate, cyclophosphamide, and cisplatin, we observed a remarkable reduction in stomatitis following the administration of low-dose leucovorin. On the basis of the results obtained in these individuals, we treated 15 additional patients with this three-drug regimen plus leucovorin rescue. These subjects could tolerate the treatment with lesser degrees of stomatitis and received higher edatrexate doses in subsequent courses as compared with the patients who previously received this regimen without leucovorin rescue. This approach is expected to improve the therapeutic indices of edatrexate and edatrexate-containing chemotherapy regimens by modifying the dose-limiting toxicity of this antineoplastic agent.

Relation of musical wind instruments to malocclusion.

Eighty-four seventh graders of both sexes, not undergoing orthodontic treatment, participated in a double-blind study to determine the effect on the anterior teeth of playing a musical wind instrument. The instrument players were just beginning their studies. Seventh graders not playing a musical wind instrument served as the control group. Impressions and plaster casts were made of each mouth at the beginning and conclusion of the study. The plaster casts were marked with the same numerical code assigned to each student. Overjet and overbite measurements of the anterior teeth were taken from the plaster casts and recorded. Findings indicate that, on an individual basis, the effect on the anterior teeth of playing a musical wind instrument is unpredictable. Only on a group basis can a class of musical wind instruments be theorized to have a certain effect on the anterior teeth. The playing of a musical wind instrument should not be used as a substitute for orthodontic treatment.

Use of microdissected airways to define metabolism and cytotoxicity in murine bronchiolar epithelium.

The use of the mouse for carcinogenesis bioassays has raised questions regarding the cell of origin of lung tumors. Since a feature of chronic lung injury from aromatic hydrocarbons is an apparent alteration in target cell susceptibility, the present study was designed to test the feasibility of using microdissected pulmonary airways to evaluate the metabolism and cytotoxic response of one of the potential targets of pulmonary carcinogens, the bronchiolar Clara cell. Airways were microdissected from mouse lungs that had been filled by injection of agarose (1%) into the trachea. Ultrastructural integrity of the explants has been maintained for up to 8 h in culture. The cytotoxic response of bronchiolar epithelium in explants incubated with naphthalene (0.5 mM) was identical to the vacuolation and exfoliation observed in bronchioles of mice 24 h after intraperitoneal administration of naphthalene (100 or 300 mg/kg). Pre-incubation of the explants with piperonyl butoxide, a cytochrome P-450 monooxygenase inhibitor, prevented naphthalene-induced cytotoxicity. Naphthalene monooxygenase activity was easily measurable in all levels of airway, including trachea, lobar bronchi, major and minor daughter pathways, and distal bronchioles. No metabolism was detected in lung parenchyma or large vessels. Dihydrodiol and a glutathione adduct derived from 1R, 2S-naphthalene oxide were the sole metabolites detected by HPLC in incubations of airway explants. Formation of a single diastereomeric glutathione conjugate indicated that the metabolic epoxidation of naphthalene was highly stereoselective. Glutathione S-transferase activity was measured in all compartments, with the highest activities in trachea and lowest in distal bronchiole and pulmonary vein. Explants maintained pools of reduced glutathione for up to 4 h in culture. We conclude that microdissected airways have excellent potential for: (1) defining the capability of bronchiolar epithelium to catalyze xenobiotic biotransformation, (2) comparing activity in target and nontarget lung compartments as a means of identifying specific metabolic pathways associated with the cytotoxic response, and (3) use with a variety of species, including nonhuman primates and humans, as a means of providing appropriate data for extrapolation of effects in the intact animal to the human, where bioassay is not possible.

Unerupted maxillary anterior supernumerary teeth: report of 204 cases.

Two hundred and four cases of unerupted maxillary anterior supernumerary teeth were found among Hong Kong school children between the ages of six to 9.5 years. The anomaly was prominent in the male with a sex ratio of 5.4:1. There was no significant difference in the distribution of unilateral and bilateral supernumerary teeth, between the two sexes. Most of the supernumeraries were located within the central incisor region. The inverted type was found to be associated with bodily displacement, median diastema and torsiversion, while the normally oriented ones were related to delayed eruption. The association of maxillary anterior supernumeraries with clinical symptoms was compared with studies on Caucasian populations, and guidelines were described for their early diagnosis.
