[Influence of glucocorticoid steroid therapy on gastric and duodenal mucosa and Helicobacter pylori infection in children with nephrotic syndrome].

UNLABELLED: The purpose of the study was to evaluate the influence of steroid therapy on endoscopic picture of gastric mucosa. The study comprised 22 children (17 boys and 5 girls) with the nephrotic syndrome aged 4-12 years (the mean 9 years) treated with prednisone. Children were divided into two groups taking into account duration of therapy. The group I (10 children) comprised patients with therapy duration not exceeding 3 months. The group II (12 children) comprised children treated 5 years in average. All patients underwent gastroscopic examination. Macro and microscopic lesions of gastric mucosa, their intensity and coexistence of Helicobacter pylori (HP) infection were estimated. CONCLUSIONS: 1. In nephrotic children during prednisone therapy different types of gastric and duodenal mucosal inflammatory changes occur. 2. Duration of prednisone therapy does not influence significantly on location, type and severity of mucosal inflammatory changes. 3. Frequency of coexistent HP infection increases with prednisone therapy duration. 4. Prednisone therapy of nephrotic children is indication for gastric and duodenal endoscopic examination.

[Acrodermatitis enteropathica with mild course].

Acrodermatitis enteropathica is a disorder due to zinc deficiency in a human body. It is disease of autosomal recessive inheritance, that usually occurs in infants, typically develops in early months of life. Classical findings include acrodermatitis, diarrhea and alopecia. Natural course of disease is various from observed ones in 70 tumoral cases to subclinical cases without severe diarrhoea with improvement of post maturation period. We describe a 13-year-old girl with acrodermatitis enteropathica diagnosed after nine years from first symptoms.

[The assessment of Helicobacter pylori eradication with the urea breathing test (13C UBT)].

UNLABELLED: The purpose of the study was to assess the efficacy of Helicobacter pylori (HP) infection treatment in children with the nephrotic syndrome, using non-invasive urea breathing test (13C UBT). The study comprised 20 children with nephrotic syndrome treated with prednisone. Children were divided into two groups: those who recurred prednisone for a short time (1-3 months) and those treated longer than 6 months. In all children gastroscopic examination, histopathologic assessment of gastric mucous membrane specimens and 13C UBT were performed. 65% of nephrotic children was HP (+). In those treated for 1-3 months incidence of HP infection was 40%, whereas in those were treated longer than 6 months it was 90%. In all children treatment of HP infection was instituted. Seven weeks after cessation of treatment its efficacy was assessed by 13C UBT. The treatment was successful in all nephrotic children. In the nephrotic controls with negative histopathologic assessment for HP infection the results of UBT were normal. CONCLUSION: 1. Children with the nephrotic syndrome are more susceptible to HP infection as compared with healthy ones. 2. In those children treatment of HP infection is efficient. 3. 13C UBT allows to estimate HP infection treatment efficacy in a non-invasive way.

[Magnesium and zinc levels in blood serum and cerebrospinal fluid in children with febrile convulsions].

Magnesium and zinc levels in blood serum and cerebrospinal fluid were estimated in 18 children aged 8 months-5 years with febrile convulsions. Control group consisted of 15 apparently healthy children in the same age. On the ground of ethical reasons no control values for cerebrospinal fluid were evaluated. The mean serum concentration of magnesium and zinc was significantly lower in the sick children (p < 0.001 and p < 0.05 resp.). The magnesium levels in cerebrospinal fluid were evidently lower than those quoted in the literature and the mean concentration of zinc was higher in comparison with literature data. The presumable pathogenetic role of disturbed magnesium and zinc metabolism in febrile convulsions is discussed.

The effect of a single dose of morphine and ethanol on magnesium level in blood serum and tissues in mice.

The effect of a single dose of morphine (50 mg/kg) and ethanol (2 g/kg) on total magnesium content in blood serum, brain, heart, lung, kidney, liver, femoral muscle and spleen in mice was studied. Significant decrease of magnesium serum concentration was observed after morphine and ethanol administration but not after both drugs given simultaneously. Morphine caused the evident decrease of magnesium content in brain, lung, kidney and muscle, while it was elevated in heart and spleen and unchanged in liver. Ethanol produced significant decrease of magnesium content in heart, lung and kidney and it's increase in liver and spleen. Concomitant administration of both drugs was connected with the diminished amount of magnesium in heart, lung, kidney and muscle and led to the rising of magnesium content in spleen. It is concluded that even a single dose of investigated drugs is sufficient to produce promptly some risk of alterations in magnesium homeostasis.

[Henoch-Schoenlein purpura and toxocarosis].

Henoch-Schoenlein purpura is a common vasculitic syndrome in childhood. Diagnosis is based on the presence of nonthrombocytopenic purpura arthritis or arthralgia, abdominal pain and glomerulonephritis. In our case report a causal relationship between toxocarosis and Henoch-Schoenlein purpura is suggested.

[Kawasaki disease--two case reports].

Kawasaki syndrome is an acute multisystem vasculitis of infancy and early childhood associated with high fever, mucocutaneous inflammation, and the development of coronary artery abnormalities. We describe the clinical evaluation and treatment with intravenously administered immune globulin in two children with Kawasaki syndrome.

Serum concentrations of individual monocarboxylic short-chain fatty acids C2-C6 in underweight children.

The serum concentrations of individual short-chain fatty acids containing 2-6 carbon atoms were studied in 12 underweight children. Mean n-butyric and iso-valeric acid levels were significantly elevated in underweight children, while those of n-valeric and iso-caproic acids were not significantly higher than in the control group. The level of total free fatty acids was also increased in the underweight group. A reduced enzymatic activity in fatty acid synthesis is supposed to be the factor underlying the impaired lipogenesis.

[Electrical resistance of cerebrospinal fluid in inflammatory diseases of central nervous system in children (author's transl)].

The specific electrical resistance of the cerebrospinal fluid was measured by means of conductometry in 14 cases of meningitis purulenta, 17 cases of meningitis serosa, 10 cases of encephalitis and in 32 control subjects. The mean value was found to be significantly increased in meningitis purulenta in comparison to the control group. The differences between the average values of the electrical resistance concerning other groups studied were found to be unsignificant. In some cases the initially increased electrical resistance of cerebrospinal fluid observed in the acute state of the disease reached the normal levels at the time of clinical improvement. The presumable causes of these phenomena are discussed.
