[Laparoscopic fenestration in the management of symptomatic polycystic liver disease].

We report the case of a 70-year-old woman with polycystic disease of the liver and kidney, complicated with rupture and hemorrhage of the hepatic cysts located in the right lobe. A laparoscopic approach was used. After identifying the polycystic mass, the cytologic examination of the aspirated hematic fluid and the intraoperative biopsy showed its benign nature. An hepatic fenestration was performed. Hemostasis was achieved by electrosurgical and argon probes. The postoperative course was satisfactory and radiological improvement was demonstrated on the CT scan 3 months later. Follow-up examination one year later has been uneventful. Fenestration is one of the less stressing surgical options in the adult polycystic liver disease when treatment is indicated. The laparoscopic approach may be used with safety to perform a similar technique in selected cases, establishing a communication between the cysts and the peritoneal cavity. Nevertheless, it may be difficult to reach deeply situated cysts and to differentiate them from vascular structures. Laparoscopic intraoperative ultrasonography might be of great value in these cases. Likewise, long term studies are needed to determine the benefits and the indications of the laparoscopic fenestration.

Solitary hamartomatous duodenal polyp; a different entity: report of a case and review of the literature.

Solitary hamartomatous duodenal polyps are a clinical entity considered until now to express an incomplete or initial form of Peutz-Jeghers syndrome (PJ syndrome). Following our experience of 1 case, we analyzed the clinical characteristics of the 12 previously reported cases of solitary hamartomatous duodenal polyps. In none of these cases did we find any documentation of mucocutaneous pigmentation, a family or personal history of intestinal polyps, or local neoplastic degeneration of the disease, and all cases presented during the fifth or sixth decade of life. Thus, we conclude that solitary hamartomatous duodenal polyps constitute a different entity to PJ syndrome, the most appropriate elective treatment for which is endoscopic electrosurgical polypectomy.

[Surgical indications in acute pseudo-obstruction of the colon (Ogilvie syndrome)].

Acute pseudo-obstruction of the colon (APC), popularly known as Ogilvie's syndrome, has been the subject of numerous medical communications in the past two decades. In this paper three patients with APC managed surgically are presented. In two patients cecal perforation developed; in the third patient a discharge cecostomy was carried out because of a caecal diameter of 16 cm. Indications for the operative management of these patients include pneumoperitoneum, development of peritonitis in the area of the cecum, continued cecal distention after 48-72 hours of therapy with or without colonoscopy, cecal diameter greater than 12 cm, respiratory failure and uncertain diagnosis.

[Cecovesical fistula of inflammatory origin simulating neoplasm of the cecum].

We describe a case of a caecovesical fistula in a 60-year-old woman caused by an inflammatory tumour of a probably appendicular origin which mimicked cancer of the caecum. The patient, had a history of idiopathic peripheral polineuropathy and chronic constipation. Presented urinary tract infection and fecaluria, without symptoms of appendicitis. At operation, a solid mass including the caecum and the fundus of the bladder was found, and the appendix was not identified. In view of the neoplastic appearance and the impossibility of separating the caecum from the bladder, "en bloc" right hemicolectomy and partial cystectomy were performed. The pathologic study showed the inflammatory nature of the lesion.

[Importance of surgical staging in patients with cancer of the exocrine pancreas].

A retrospective analysis of 64 patients with adenocarcinoma of the exocrine pancreas was carried out to determine the impact of stage of the disease and surgical treatment on survival. Significant differences (p < 0.05) were observed in the survival rates of stage I (36%), stage II (12%), stage III (15%) and stage IV (0%). Patients with pancreatectomy had better prognosis (32%) than those with palliative procedures (11%) (p < 0.025). The presence of positive lymph nodes in patients with tumor resection didn't show significant differences on survival. Survivors showed a short prodome of symptoms (< 3 weeks) and a resected neoplasm (p < 0.025).
