A simple technique for shoulder arthrography.

PURPOSE: To present a systematic approach to teaching a technique for arthrography of the shoulder. Using an adhesive marker-plate with radiopaque coordinates, precise sites for puncture can be identified and the need for fluoroscopic guidance obviated. MATERIAL AND METHODS: Forty-six glenohumeral arthrograms were performed in 45 patients; in 1 case involving examination of both shoulders. The stages of the technique are described in detail, as are the fundamental aspects of achieving an effective glenohumeral injection. Pain intensity was measured in all patients using a verbal description scale. RESULTS: Shoulder arthrography was successful in all cases. Average time taken for the procedure was 7 min, with no difference in the respective times required by an experienced radiologist and a resident. The procedure was well tolerated by most patients, with slight discomfort being observed in a very few cases. CONCLUSION: The arthrographic technique used in this study is simple, safe, rapid, and reproducible, and has the advantage of precise localization of the site for puncture without need for fluoroscopic guidance. The procedure described in this study can be of help in teaching residents and can reduce the learning curve for radiologists with no experience in arthrographic methods. It also reduces the time of exposure to fluoroscopy.

Can we expand the indications for liver transplantation among hepatocellular carcinoma patients with increased tumor size?

INTRODUCTION: Due to the scarcity of donors and the fact that size is the main prognostic factor, Milan criteria have been used since 1996 to select hepatocellular carcinoma (HCC) patients for liver transplantation. In 2001 UCSF criteria showed that including layer tumors did not reduce the survival results. The objective of this paper was to evaluate whether HCC tumor sizes exceeding the Milan criteria adversely influence survival rates. PATIENTS AND METHODS: Between May 1988 and July 2001, 53 patients were transplanted due to HCC and cirrhosis. The etiology of cirrhosis was HCV in 23 cases and HBV in 6. In 11 cases the HCC were incidental by discovered namely, a mean/ diameter of 1.8 cm (versus 2.6 cm in nonincidental HCC). Sixty-two percent of tumors met the Milan criteria, and 68% the USCF criteria. RESULTS: The actuarial survival was 79% at 1 year and 62% at 5 years. The survival of patients with incidental HCC was 82% at 1 year and 82% at 5 years, which is better than the survival of those with nonincidental HCC (78% at 1 year and 57% at 5 years, P<.05). According to Milan criteria, the survival patients with early tumors was 82% at 1 year and 68% at 5 years, and for advanced tumors (NS), 75% and 54%, respectively. Comparison of early versus advanced tumors according to UCSF criteria showed survivals of 84% versus 64% at 1 year (P<.05) and 67% versus 48% at 5 years (P<.05), respectively. CONCLUSION: Increasing the HCC size among LT according to the California criteria did not reduce survival rates compared with the Milan criteria.

Synthesis of trifluoromethyl ketones as inhibitors of antennal esterases of insects.

A variety of long chain aliphatic and aromatic trifluoromethyl ketones I-XIV has been conveniently prepared, many of them for the first time, from the corresponding Grignard or organolithium derivatives. Two of them, (Z)-1,1,1-trifluoro-15-octadecen-13-yn-2-one (XV) and (Z)-1,1,1-trifluoro-16-nonadecen-14-yn-2-one (XVI), structurally-closed analogues of (Z)-13-hexadecen-11-ynyl acetate, the sex pheromone of the processionary moth Thaumetopoea pityocampa, have been stereospecifically synthesized in excellent yield by a convenient new method. The procedure involves lithiation of the corresponding iododerivative XXIX and XXX with one equivalent of tert-BuLi to obviate addition of the reagent to the enyne system. Some of the compounds have already been tested and found to be good inhibitors of antennal esterases in the Egyptian armyworm Spodoptera littoralis and the pheromone action in the processionary moth Thaumetopoea pityocampa. beta-Thiotrifluoromethyl ketones XVII-XX, which are expected to enhance the inhibition activity of the parent ketones due to their higher hydration constants, have also been prepared in good yields.

Trifluoromethyl ketones as inhibitors of the processionary moth sex pheromone.

Aliphatic and aromatic trifluoromethyl ketones have been evaluated in the laboratory and in the field as inhibitors of the pheromone response of the processionary moth Thaumetopoea pityocampa males. Among them, two compounds, (Z)-1,1,1-trifluoro-15-octadecen-13-yn-2-one and (Z)-1,1,1-trifluoro-16-nonadecen-14-yn-2-one, are closely related analogs of the natural pheromone (Z)-13-hexadecen-11-ynyl acetate. In the laboratory experiments, carried out by pre-exposure of males to vapors of the chemicals, alpha-naphthyl trifluoromethyl ketone, beta-naphthyl trifluoromethyl ketone, 1,1,1-trifluorotetradecan-2-one and (Z)-16-nonadecen-14-yn-2-one displayed notable blockage of the pheromone detection on EAG. The activity of 1,1,1-trifluorotetradecan-2-one is postulated to be due to the inhibition of the pheromone-degrading esterase. In general, the compounds have shown low specificity for the substrate and exhibited only a modest or null EAG intrinsic activity. In the field, benzyl trifluoromethyl ketone, trifluoroacetophenone, (Z)-1,1,1-trifluoro-15-octadecen-13-yn-2-one, (Z)-1,1,1-trifluoro-16-nonadecen-14-yn-2-one and beta-naphthyl trifluoroacetate showed a remarkable disruptant effect when mixed with the pheromone in 1:0.1, 1:1 and 1:10 ratio. (Z)-16-Nonadecen-14-yn-2-one has been found to be a modest agonist of the natural pheromone, exhibiting an attractant activity threefold lower than the parent molecule.

Mucinous metaplasia to neoplastic lesions in endometrial samples with cytohistologic correlation.

Seven cases of endometrial mucinous metaplasia and five of well-differentiated mucinous adenocarcinoma of the endometrium were studied. Cytologic specimens were obtained by Isaacs endometrial sampler, avoiding endocervical contamination. The histologic diagnosis between the intermediate type of mucinous metaplasia and the well-differentiated mucinous adenocarcinoma posed no problems. DNA analyses of the histologic samples showed a euploid pattern in benign and intermediate types of metaplasia, while well-differentiated mucinous adenocarcinoma showed a hyperdiploid pattern. The cytologic diagnosis of benign mucinous metaplasia should be suggested in the presence of abundant mucinous cells in endometrial samples in the absence of nuclear abnormalities.

Fusion polypeptides in gene cloning: potential problems due to conformational alterations at the junction.

Many eukaryotic genes are cloned in bacterial hosts as fusion polypeptides. Prediction of the secondary structures for some common prokaryotic fusion polypeptides shows that many junction sites correspond to important secondary structures. It is suggested that such structures could affect (hinder, etc.) the conformation or drive the folding of the neighboring eukaryotic counterparts. Thus the prokaryotic junction should be better performed in random coil regions, or short fusion prokaryotic polypeptides should be used.
