[Theoretical bases of health education in cardiology: the secondary prevention of ischemic heart disease].

The secondary prevention of ischemic heart disease must, at the right time, recognize and efficaciously treat the categories of the illness (myocardial infarction, angina pectoris, heart failure, arrhythmias, sudden death) and restrain its development. The success of such a preventive program involves the active cooperation of the patient who, for this reason, has to be motivated by suitable information. So a proper cardiac information technique is indispensable; it has to involve: the patient, his family, his physician and, more generally, politicians, medical workers, employers. The Authors explain the education and counseling program, started 15 years ago, in accordance with the following scheme: users, goals, educational and counseling methods.

[An experience with a mobile coronary care unit: from a study project to a permanent community service (author's transl)].

Mobile coronary care units and stratified systems for coronary care have received rather poor attention in Italy up until now. To evaluate advantages and disadvantages, organizational problems, costs, etc. of a mobile unit in an Italian community provided with a reference centre for acute coronary artery disease, a study project was started in 1977. The organization of such unit was based on that of the hospital in Belfast. The covered area (200,000 inhatitants) was carefully limited to keep delays at a minimum. A program of public education, stressing the necessity for early calls for medical intervention was developed. All calls were evaluated by a physician on duty in the CCU before intervention. The results of this experience, extended to four years, are reported showing that a Mobile Coronary Care Unit is valuable from a medical point of view, is economically sound and practical in a medium size Italian city such as Udine. According to this, the Mobile Coronary Care Unit was changed from a study project to a permanent community service.

[Old and new methods for control of pacemakers (author's transl)].

First there is a description of present methods available for controlling the functioning of pacemakers, most of which are already being used in traditional clinic-electronic control. There follows a presentation of telephone monitoring (ECG + Pacing Rate) and its limits so far as photoanalysis is concerned. Mention is made of the new italian system of telephone monitoring which also includes measurement of amplitude and width of the pulse. Personal experience of the diagnostic value of photoanalysis is given. This concords with others as to its necessity to detect any failure from circuitry defects or from breakage or insulation of electrode. In conclusion the hope is expressed of actuating a completely trustworthy control system which includes a high degree of foreseeing failure (telephone monitoring) and diagnostic value (Rx + photoanalysis). At last the advantages of an automatic follow-up system in a pacemaker patient clinic are discussed.

[Medico-social aspects of the post-infarct state. Clinical, functional, psychological and return-to-work studies of 100 patients with myocardial infarct treated in the coronary unit].

To determine whether intensive care unit are able to improve long term as well as immediate prognosis of acute myocardial infarction, one hundred patients, aged 40 to 60 years, six months to three years after discharge have been studied. All patients were examined with ergometric test, psychological assessment and medico-social survey. Comparison was made with patients treated in general medical wards. Correlations between clinical picture, psychological pattern, functional capacity and return to work prove that treatment in a coronary care unit positively affects the future of these patients. The importance of a rehabilitation program and its principles are also discussed.
