Fractures of the facial skeleton in children.

Fractures of the facial skeleton are infrequent in children. A series of 147 fractures in 116 children aged from 10 days to 13 years old was retrospectively reviewed. The male to female ratio was 1.4:1 and the highest incidence occurred in the age group 8 to 10 years. The main aetiological factor was road traffic accidents. The mandible was predominantly involved (83.7%). The incidence of fractures of the middle third of the face was very low (4.0%). Closed reduction was undertaken most frequently. Complications were found to be minimal.

Oral spindle cell lipoma. Report of a case.

A case of a spindle cell lipoma of the hard palate of a 58-year-old male is reported. The histologic diagnosis is discussed. Conservative surgical removal is the treatment of choice.

Diagnosis, Differential

Anaerobic bacteria in jaw cysts.

The bacterial flora of the fluid content of 54 cysts of the jaws with a history suggestive of infection were studied. Forty-seven of these cysts were subsequently considered to be infected and from 78.6% of these, positive bacterial cultures were developed. Of the bacterial strains 89.2% were anaerobes and only 10.8% were pure aerobes or facultative anaerobes. Microorganisms isolated from each specimen ranged from 1 to 4 bacterial species. Gram positive anaerobic cocci revealed to be the most frequent bacterial group (36.9%), followed by Gram negative anaerobic rods (29.8%), Gram positive aerobic cocci being the third most common group of the isolates (19.0%). Antibiotic sensitivity tests of the isolated anaerobic cocci to a group of nine antibiotics revealed chloramphenicol and minocycline as the most effective. All anaerobic rods tested, were sensitive to metronidazole.

Concentration of metronidazole in rat serum and mandible.

The levels of metronidazole in rat serum and mandibular bones was measured 2, 5 and 8 h after oral administration of three doses of 200 mg/kg. Effective levels of the drug in serum and mandible were found at all time intervals as well as a higher concentration in mandible than in serum.

Administration, Oral

Concentration of cefuroxime in the bones of albino white rats: a comparative study of femur, humerus, and mandible.

The levels of cefuroxime in rat bones with a different morphology, function, blood supply, and proportion between lamellar and cancellous bone were studied, after a 24-h administration of 750mg/kg X 3. The concentrations in the right and left femora, humeri, and mandibles of the same animal were measured 2 h after the last administration. The result indicate statistically significant differences between femur and humerus (P less than 0.001), femur and mandible (P less than 0.05), and humerus and mandible (P less than 0.05).

Anaerobic bacteria in dentoalveolar abscesses.

The present study was initiated to determine the bacteriology of 40 orofacial abscesses of dental origin in patients who had taken antibiotics for several days. Bacteria were isolated from all but 2 specimens. Aerobic or facultative anaerobic bacteria were isolated in 21 cases, obligate anaerobic bacteria in 17 cases, whereas in 11 cases, polymicrobial growth was revealed. The average number of bacterial species was 2.1 per specimen. Gram positive aerobic micro-organisms predominated, namely, Staphylococcus epidermidis followed by Streptococci (group A) and Staphylococcus aureus. Among obligate anaerobes, Gram positive micro-organisms, peptostreptococci and peptococci were more often isolated, in the following decreasing order: Ps. productus, Ps. intermedius, Ps. parvulus, Ps. anaerobius, Pc. constellatus, Pc. prevotii. Gram negative anaerobic rods were detected in a small number of cases, namely B. corrodens, B. fragilis, B. melaninogenicus, B. ochraceus, B. oralis. Quantitative determination did not show any meaningful difference between aerobic and anaerobic isolates. Susceptibility tests against a series of antibiotics showed that ampicillin was the most active in aerobes and cefoxitin in anaerobes.

Adolescent

Central odontogenic fibroma.

The article presents a case of central odontogenic fibroma occurring in the maxilla. The literature is reviewed, in the belief that a discussion of the controversial issues surrounding this neoplasm, its treatment and follow-up information, is warranted, due to the small number of cases (13) of this tumor that have been recognised and reported up to the present.
