[Plasma renin activity in renal veins in different types of arterial hypertension (author's transl)].

Peripheral plasma renin activity (PRA), vena cava and renal veins PRA were measured in 85 patients; arterial PRA was also measured in several cases. 25 patients had essential hypertension or primary aldosteronism; whereas in 60 patients renal or renovascular abnormalities were present. Peripheral PRA was elevated in 17/30 patients with parenchimal or excretory tract diseases. The following ratios came from the results of vena cava, and renal veins PRA were calculated: Ra/Rc and Rc/P according to Stockigt et al. and V-A/A according to Vaughan et al. The ratios were calculated only when v. cava PRA was greater than 4 ng/ml/3 hrs. A good correlation was found between v. cava can arterial PRA. Among the 60 cases with renal hypertension, 26 underwent either revascularization or nephrectomy surgery. Eleven of the 15 patients operated on for renovascular diseases showed a significant reduction of blood pressure (greater than 30 mmHg); all had high peripheral PRA and 6 had significant Ra/Rc and Rc/P (respectively greater than or equal to 1,5 and less than or equal to 1,3). The same patients also had V-A/A of the affected kidney greater than or equal to 0,48 and 5 also V-A/A of the controlateral kidney less than or equal to 0,23, both values being significant for a unilateral renin secretion and controlateral suppression. Among the 4 patients who were unsuccessfully operated on, only 2 had elevated peripheral PRA and 1 had Ra/Rc and Va-A/A greater than normal, but not significant values of Rc/P and Vc-A/A. 11 patients with small kidney or other forms underwent surgery; among the 6 cases with satisfactory results, 5 had high peripheral PRA, 2 showed significant ratios Ra/Rc and 3 a significant V-A/A for both kidneys. Only 1 of the patients not cured by surgery had supernormal peripheral PRA and none had any significant ratio. Therefore both methods for elaborating data obtained from the measurement of PRA in renal veins seem to offer similar prognostic indications. The finding of significant ratios is an almost sure criterium for predicting a surgical cure of renal hypertension, whereas the cases where we may expect a failure present ratios which are not significant. However, patients of the latter group may sometimes have successful results at surgery too, which demonstrates that other mechanisms may also be involved in the pathogenesis of this form of hypertension.

[Surgery and chemotherapy in the treatment of primary gastric lymphomas: case review].

Fifty-seven patients with primary gastric non-Hodgkin's lymphoma are here reported. All our patients were treated with surgery as first step procedure. Thirty-six were treated in addition with chemotherapy (four of them with radiotherapy also). Thirty-eight were stage IE and nineteen IIE. According to the Working Formulation, 12 cases were classified as low, 25 intermediate and 20 high malignancy. 55 patients achieved a complete remission (96%) and only 6 relapsed (11%). The 10-year disease-related survival is 89%. Patients treated with combined surgery and chemotherapy were mainly a high risk group (high grade histologic subtype, stage IIE, incomplete resection). Nevertheless the survival of this group was similar to the group initially treated with surgery alone (p = 0.17) in which such unfavorable prognostic factors were not present. Stage, presence of residual tumor after surgery, sex and age did not statistically correlate with survival. Furthermore, there was not statistical difference among the various histological subgroups (p = 0.16). We stress the importance of cooperation among surgeons and oncologists in order to plan an appropriate treatment.

[Control of anticoagulant therapy with coumarin derivatives. Evolution of the tests used and their standardization].

Levels of hypocoagulabity induced with coumarin derivatives (aconocoumarin) were determined in 10 patients Quick's test was used to calibrate commercially available thromboplastin reagents against a reference reagent. The results were then standardised in the form of a correct prothrombin ratio. It was found that comparable results could be obtained, even with poorly sensitive thromboplastins, by determining a thromboplastin sensitivity index and using it to convert the prothrombin ratio obtained with each thromboplastin into a "correct" ratio, i.e. one that allowed for the depressed sensitivity of the thromboplastin concerned. It was found that thrombotest was more sensitive than the commercial thromboplastins; it was also superior to the reference reagent in expressing the level of drug-induced hypocoagulability. Attention is drawn to the soundness of expressing prothrombin times in terms of the correct ratio to cut out the effect of variations in sensitivity on the part of commercial thromboplastins. At the same time, stress is laid on the superiority of thrombotest in the monitoring of oral anticoagulant therapy.

[Natural plasma inhibitors of factors activated by coagulation. Their behavior under various clinical conditions].

Levels of anti-Xa and anti-thrombin III activity have been determined in a series of patients clinically definable as "thrombophiliac" and of women undergoing oestroprogestinic contraceptive treatment. The levels of plasma Co-Ep activity for potentiating anti-Xa activity on the part of differing in vitro added doses of Ep have also been determined. The fact that anti-Xa fell in more marked a fashion than the corresponding anti-thrombin III may be the humoral equivalent of a condition of plasma hypercoagulability. Treatment with coumarin derivatives may bring about an appreciable increase anti-Xa plasma levels. The significance of these variations is discussed and the sensitivity of the anti-Xa method for monitoring small dose heparin therapy, i.e. based on antithrombotic rather than anti-coagulant effect, is confirmed.

Desmopressin and imipramine in the management of nocturnal enuresis: a multicentre study.

The efficacy and safety of desmopressin (Minirin/DDAVP) treatment compared with imipramine were investigated in a multicentre, open, cross-over design in 57 patients, aged 6-15 years, affected by nocturnal enuresis to establish the best therapeutic approach to this condition. After a two-weeks observation and control period, patients were randomised to one of two groups: intranasal administration of desmopressin, 30 micrograms/day for three weeks, followed by imipramine, 0.9 mg/kg for a further three weeks, or imipramine 0.9 mg/kg for three weeks, followed by desmopressin, 30 micrograms/day for a further three weeks. Following treatment, all patients were observed for a further two weeks. Administration of either treatment protocol resulted in a statistically significant decline in the number of enuretic episodes per week compared to the control. The greater antidiuretic effect observed in the group receiving imipramine followed by desmopressin suggests the two compounds have different profiles. Also, when the treatment period was compared with the follow-up, the antidiuretic effect had a longer duration in the group initially given imipramine. No further improvement was seen when desmopressin was administered first, with a mild worsening of the effect sometimes occurring, suggesting a different carry-over effect between the two treatments. This suggests that desmopressin offers a better approach to the management of nocturnal enuresis.
