[Urogenital sinus and cloacal malformations in infancy and childhood: surgical considerations].

The relative rarity of urogenital sinus and cloacal anomalies, the wide range of their anatomical variants furthermore the number of different surgical options makes the successful management of a child with such urogenital abnormality one of the greatest challenges to the paediatric surgeon. Based on their own experience and the literature the authors give a review of embryology, pathology, and diagnosis of urogenital sinus and cloacal abnormality. They detail the new surgical therapy developed by Hendren and Pena which has been adopted in their routine. This therapy has significantly improved the functional outcome and prognosis of these anomalies. Over the past 20 years 25 patients with urogenital sinus and cloacal abnormalities were surgically treated in the author's institute.

[Amiodarone-induced hyperthyroidism causing progression of arrhythmia].

A case of a 38-year-old male with supraventricular paroxysmal tachycardia existing for more than a decade is reported. He has received amiodarone in a daily dose of 800 mg for three years and the tachycardia returned in 1992. New antiarrhythmic drugs were added but no beneficial effect has been achieved and moreover, a case of ventricular fibrillation occurred. The 12-lead ECG performed during tachycardia and the electrophysiological study showed orthodromic AV reentry tachycardia. Laboratory tests performed proved hyperthyreotic state. Attacks of paroxysmal tachycardia were returned and aggravated by the hyperthyreosis induced by amiodarone. Finally, antiarrhythmic drugs were discontinued and methimazol was introduced. Gradually, the patient become asymptomatic within two months. The most important conclusion of the case reported, that the amiodarone induced hyperthyreosis can be subclinical or obscure. Consequently, a regular control of serum thyreoid hormone levels at least twice a year on patients with long term amiodarone administration should be advised.

[Treatment of childhood vesicoureteral reflux with sub-ureteral injection of teflon paste].

From March 1989 to october 1990, 35 refluxing ureters in 25 children were treated by submucosal injection of Teflon (STING). The overall success rate was 54% after the first injection. The authors detail the procedure and analyse their results which demonstrate that Teflon injection can be safe and effective treatment of vesicoureteral reflux in most cases can replace antireflux surgery.

[Duplication of the alimentary canal in infants and children].

This is a review of 30 duplications of the alimentary tract in 28 patients treated at the Surgical Unit of the Children's Department of the Medical University of Pécs, Hungary and at the Department of Pediatric Surgery of the Medical Academy of Dresden, Germany, from 1964 to 1989. The ages of patients ranged from 1 day to 13 years, 80 per cent were less than 2 years of age at initial presentation. There were 6 thoracic, 20 abdominal and 2 thoracoabdominal duplications. Distended abdomen, vomiting, bowel obstruction and palpable abdominal mass were most frequently encountered. Plain thoracic and abdominal X-rays, ultrasonography, barium esophagogram, barium meal and enema were the most common diagnostic procedures. Emergency operative intervention was required in 18 patients. One infant died of an unrelated disease. Twenty-three duplications were cystic and 3 tubular. One patient had an appendiceal duplication, and another patient a flat lumenless duplication located on the perineum close to the anal opening. The surgical procedure--removal of the duplication--should not be more radical than necessary to eliminate the potential complaints and prevent recurrence. During surgery the common blood supply shared between the duplication and the native bowel must be carefully protected to avoid undue sacrifice of normal bowel.

[Patent urachus in childhood].

Over the past 20 years (1971-1990) 12 infants and children have been treated surgically for a partial or totally patent urachus in the Surgical Unit of the University Children's Hospital of Pécs, Hungary. This anomaly is of special interest to pediatric surgeons, general surgeons, urologists, paediatricians. After a short survey of the embryology and pathology of the urachus, the authors analysed their material. All but one of their patients underwent surgery without serious complications. In a 6-year-old girl the urachus recanalized following shrinkage of the bladder. The authors emphasize the need of radical removal of the partial or totally patent urachus, however, this extirpation should be carried out in a froid state, frequently following incision and drainage of the suppurating urachus.

[Ectopic ureter in infancy and childhood].

Two infants, 7 children and 1 young woman have been surgically treated for ureteral ectopy in the Departments of Paediatrics and of Urology of the Medical University of Pécs over the last 15 years (1974-1989). The girl/boy ratio was 8/2. In girls, who were otherwise toilet-trained with a normal voiding pattern, constant wetting and urinary infection were the leading clinical findings. The site of ureteral drainage was the vestibule in 4 patients, the urethra in 3, the vagina in 1, the prostatic utricle in 1, and it could not be determined in 1 girl. Diagnosis was based on intravenous urography, voiding cystourethrography, ultrasonography, isotope scan, endoscopy and filling up of the bladder with a methylene blue solution. The diagnosis was more obscure when the ectopic ureter drained a poorly functioning kidney. Considering that in ectopy with duplicated system the upper pole renal segment is almost always destroyed, upper pole nephrectomy and proximal ureterectomy are advocated. In 1 neonate with esophageal atresia and tracheo-esophageal fistula ultrasonography detected the ureteral malformation. In 1 girl bilateral single ureteral ectopy was found.

[Ambulatory pediatric surgery--from the parents' view].

In recent years "day-care surgery" has been the treatment of choice for an ever increasing number of paediatric surgical patients in countries with an efficient Health Service. The reasons for this are the decreased psychological trauma to the children and the dramatic fall in expenses. In view of the important role played by parental attitude in the spreading of day-care surgery, nearly 200 parental opinions about day-care surgery have been studied. From them it became clear that the parents were convinced of advantages of this form of treatment over the conventional form requiring longer hospitalization. The authors believe that nowadays "day-care surgery" should be resorted to in the majority of a well circumscribed group of paediatric surgical conditions, from which both child and parents, and also our National Health Service will benefit.

[Continent urinary diversion in children: enterocystoplasty + appendiceo-cutaneostomy].

A continent urinary diversion was done by bladder augmentation using a cecal-colonic segment in total urinary incontinence in 3 patients aged 13, 16 and 21 years. The distal end of the appendix was brought to the skin. The narrow lumen of the appendix provides a closure mechanism in a low pressure urinary reservoir. A watertight, easily catheterizable urinary diversion might mean that there is no need to implant the distal end of the appendix into the bladder or colon in a submucous non-refluxing fashion as recommended by Mitrofanoff. The intermittent clean catheterization of the augmented bladder via the appendix was carried out at intervals of 3 hrs. 12-24 months after the establishment of urinary diversion the patients are free of complaints, socially fully accepted.

Minor morphological aberrations in children with isolated urinary tract malformations.

The prevalence of 54 minor morphological aberrations (MMAs) (mild malformations, dysplasias, deformities and phenogenetic variants) was determined in 138 children with isolated malformations of the urinary tract and in 32 patients with multiple major birth defects including renal malformations. Children with infectious diseases matched to each patient by sex, age and ethnic origin served as controls. The overall prevalence of MMAs--expressed as MMA/subject ratio--proved to be significantly higher in patients with multiple malformations (2.50) than in those with isolated renal abnormalities (0.80) and in controls (0.76). The difference between the latter two groups was not significant. Apart from characteristic patterns of well-defined syndromes, the only specific association found was the higher prevalence of supernumerary nipples in patients with isolated (and hidden) urinary tract abnormalities of various types (9.4% versus 1.8% in controls; P less than 0.005).

Cytogenetic effect of the thiocarbamate herbicides butylate, molinate and vernolate in the mouse bone marrow micronucleus test.

Three thiocarbamate herbicides, butylate (S-ethyl-diisobutylthiocarbamate), vernolate (S-propyl dipropylthiocarbamate) and molinate (S-ethyl-N,N-hexamethylenethiocarbamate) were assayed for cytogenetic effect in the mouse bone marrow micronucleus test. Butylate was inactive in bone marrow, vernolate caused a marginal increase in the incidence of micronucleated polychromatic erythrocytes only at a high toxic dose level. Molinate, the N,N-hexamethylene derivative was, however, strongly active in the bone marrow, causing a high frequency of micronucleated erythrocytes, even at subtoxic concentrations.

The extent of bile reflux and development of gastric cancer after resections in rat.

Authors performed gastric resections (Billroth II, Billroth I, Billroth II+, Braun anastomosis and Roux Y reconstruction) and laparotomies in five groups, of 110 Wistar male rats. Thirty-eight weeks postoperatively the surviving 91 animals were sacrificed, and histological study was made of the frequency of gastric stump cancer in the individual groups and the extent of bile reflux characteristic of the individual GEA types was measured. Based on their results, the risk of stump cancer was higher in operation types associated with considerable bile reflux (a cancer incidence rate of 50% after Billroth II, 28.5% after Billroth I). Following gastric resections accompanied by insignificant bile reflux (Billroth II + Braun, Roux Y) the risk of gastric stump cancer was significantly lower.

Mutagenicity of emission and immission samples around industrial areas.

Urban air particulates (suspended particles and settling dusts), as well as dusts emitted by a Söderberg aluminium production plant and a coal-burning power plant, in the industrial town of Ajka were analysed for polycyclic aromatic hydrocarbon (PAH) content (liquid chromatography) and mutagenicity (Salmonella microsome test). The same parameters for air particulates from Pápa (a town of similar size without much heavy industry) were analysed and compared to those from Ajka. The dust content, PAH concentration and the mutagenic potency of the air in Ajka were higher than in Pápa. Mutagenicity of airborne particulates showed a clear seasonality with a winter maximum and a summer minimum in both towns. The mutagenic potency of air correlated well with the benzo[a]pyrene (BP) and PAH content in Ajka but not in Pápa. The amounts of extractable organic material and mutagenic potency, as calculated for unit quantity of airborne particulate matter, were higher in the Pápa samples. Similar differences between the two towns were observed in the case of deposited dust. As regards the emission dusts, the aluminium plant emissions are responsible for much more of the mutagenicity of the urban air than the power plant emission, which is the main component of air dust pollution in Ajka.

[Histological changes in the appendix in the prodromal stage of measles].

Morphological changes of adenoid tissue associated with morbilli are known. However, it is less known that the disease may start with symptoms of acute appendicitis and on the basis of histological picture morbilli can be raised or rendered probable, respectively, before appearance of typical clinical symptoms. Among morbilli cases of recently increased number the later developed morbilli was diagnosed from appendix in an appendectomy performed on a patient being in prodormal stage.

Long-term carcinogenicity bioassay of the herbicide atrazine in F344 rats.

In Fischer F344/LATI rats of both sexes the herbicide atrazine was given in the diet at concentrations of 0, 375 and 750 ppm for 126 weeks. Food and water consumption was similar in the treated and control groups. Feeding of atrazine resulted in dose-dependent depression of body weight gain in both sexes. There was no difference in the survival among the females. The males in the treated groups lived significantly longer than the controls. Exposure to atrazine resulted in significantly increased incidence of mammary tumors in the high dose male group. Uterine carcinomas were observed at a dose-related, significantly increased incidence. The number of combined leukemias/lymphomas increased in the treated males and females, but it was statistically significant only for females. The total number of malignant tumors showed a dose-related increase in both sexes. Other tumors and nontumorous lesions occurred at background level and were not influenced by treatment.

Effect of small bowel resection on fecal bile acid excretion and on experimental colon tumour in rats.

Ileal and jejunal resections were carried out to investigate their effect on the faecal bile acid excretion and on the development of 1,2-dimethylhydrazine (DMH)-induced colonic cancer in rats. Both resection types raise the total daily faecal bile acid level compared to the control sham-operated group, whereas ileal resection has a more pronounced effect. The incidence of tumours was found higher in groups with enhanced faecal bile acid level. Our findings show a connection between the daily faecal bile acid excretion and the incidence of DMH-induced colonic cancer.

1,2-Dimethylhydrazine

Surgical management of neonates and children with ambiguous genitalia.

Gender assignment is a neonatal surgical emergency. Early evaluation and treatment of intersexuality must be done as a team approach with a genetist, a paediatric endocrinologist and a paediatric surgeon/urologist taking part in it. Change of gender after 2 years of confusion of gender identity often results in social disaster. Over a period of 20 years the authors managed a total of 27 patients with ambiguous genitalia. They favour early reconstruction of most anomalies at about 3 years of age, which seems to be optimal in terms of surgical feasibility and avoidance of the obvious emotional problems caused by delayed intervention. A detailed description of policy and techniques is given for surgical intervention in the patient to be raised as a female or as a male. They changed the gender in three patients, twice in neonates and once before two years of age.

[The role of bile in the development of colon cancer induced by 1,2-dimethyl hydrazine in rats].

Experimental operative models were created in order to investigate the role of the bile acids in the development of colon cancer in rats. These techniques changed the concentration of bile acids in different parts of the colon, and the daily fecal total bile acid levels. The relationship between these changes and the development of 1,2-dimethylhydrazin-induced colon cancer was studied. Significantly more tumors than in the control group were found if the daily total bile acid level and the concentration of bile acids in the left subsite of the large bowel was increased. Our results suggest that bile acids, as promoting factors, may play a role in the development of colon cancer in rats.
