[Evaluation of the reliability of the Heremans serum protein index in the differential diagnosis of jaundice].

Heremans' serum proteins index was evaluated in the diagnosis of 58 cases of icterus. Analysis of variance was employed in assessing the behaviour of the index in various forms of the disease. Its reliability was evaluated in the light of its sensitivity, specificity, suspicion, positivity, confirmation, validity and accuracy indices, information content and entropy. Multivariance analysis was also used to determine the mean and optimal values for the discriminating functions of neoplastic and non-neoplastic icterus forms.

Evaluation of 4'-iodo-4'-deoxydoxorubicin-induced cardiotoxicity in two experimental rat models.

In the present study, 1 single-dose and 1 multiple-dose models were applied in studying 4'-iodo-4'-deoxydoxorubicin (I-DX) cardiotoxicity. Anthracycline cardiotoxicity has been reproduced in several animals including mice, rats, hamsters, rabbits, dogs, and monkeys. Of these species the rat can be considered the most suitable species for the study of anthracycline-induced cardiomyopathy. The cardiotoxicity induced by I-DX in male Sprague-Dawley rats was compared to that of doxorubicin (DX), used as standard positive control. Groups of 36-42 rats were observed for up to 35 wk to follow the progression of the lesions. Cardiomyopathy was evaluated through well-established qualitative/quantitative morphological grading. The new DX derivative proved to be clearly less cardiotoxic than DX with both treatment schedules. Although both models can be considered useful for evaluating and comparing the cardiotoxicity of new anthracycline derivatives and mimicking the transvenous endomyocardial biopsies in humans, the chronic test seems to be more suitable for compounds like I-DX, which possess a low cardiotoxic potential and which could go undetected in the single-dose test.

Erythropoietin in Jehova's witness heart surgery.

The patients, reported here, needed open heart surgery, but religion obliged them to refuse blood transfusion. Three of the four patients suffered from obstructive coronary diseases and one from mitral valvular disease, prevalently stenosis. All of them refused blood transfusions. One of the three patients presented, was refused by an other Cardiovascular Surgery Center because of his low blood values (Haemoglobin 9.2--Haematocrit 26.7). All these patients had been treated with subcutaneous injection of epoetin alfa 10,000 U twice a week and ferrous sulphate 525 mg three time a day per os, for three weeks before operation. Haemoglobin, haematocrit and reticulocytes values were controlled in pre, postoperative and at discharge. With this treatment the authors found haemoglobin and haematocrit values so increased to allow surgery without blood transfusion during and in the post operative period.

Synthesis, antitumor activity, and cardiac toxicity of new 4-demethoxyanthracyclines.

The new anthracycline glycosides 4-demethoxy-4'-deoxydaunorubicin and 4-demethoxy-4'-O-methyldaunorubicin, synthesized by coupling 4-demethoxydaunomycinone with 1-chloro-derivatives of protected 4-O-methyl and 4-deoxydaunosamine derivatives, have been converted into the corresponding doxorubicin analogs. The new compounds have been compared for antitumor effect with the parent drugs and with the previously described 4-demethoxydaunorubicin, 4-demethoxy-4'-epidaunorubicin, and their doxorubicin analogs. All of the new analogs were more cytotoxic against HeLa cells in vitro and were more toxic and more potent in mice than the parent drugs. Comparison at optimal antitumor doses showed that the new analogs were as active as the parent drugs against ascitic P388 leukemia and disseminated Gross leukemia. They were also active when administered orally. The new doxorubicin analogs were slightly more active than doxorubicin against ascitic L1210 leukemia and were markedly more active against disseminated L1210 leukemia. In a parallel activity-cardiotoxicity test in C3H mice repeatedly treated iv, 4-demethoxydoxorubicin, 4-demethoxy-4'-epidoxorubicin, 4-demethoxy-4'-O-methyldoxorubicin, and 4-demethoxy-4'-deoxydoxorubicin showed antitumor activity against mammary carcinoma without inducing the typical myocardial lesions observed after doxorubicin treatment, 4-Demethoxy-4'-O-methyldoxorubicin, because of its high antitumor effectiveness, lack of cardiac toxicity in mice, and activity by the oral route, deserves further study.

[Heart transplant: an emotional storm for the child and his family].

The purpose of the present study was a psychological analysis of the emotional state of a family awaiting heart transplantation. Current practice suggests that such emotional reaction is characterized by mixed and contradictory feelings which require further investigation. Subjects were 5 children waiting for transplant and 5 with previous transplant. Methods were: 1) open interview with children and parents; 2) projective drawing test for children (Corman family test, H.T.P., L.A.D.S., M.U.C.T., K.F.D.); 3) "palinomas" questionnaire which investigates: awareness of the illness, child relation with the food, feelings of competence and control, self-esteem and perception of the self. "Palinomas" was created in Italy by a hospital psychological team in order to give a chance to the child to talk about himself and to maintain control over the situation. The information we obtained by "palinomas" revealed to be very useful to understand the personality of the child through the main areas reported above. Within the limits of an introductory study, results indicate that family needs an active psychological support in order to develop a more positive attitude.
