Early childhood caries. Oral health survey in kindergartens of the 19th district in Rome.

AIM: The aim of this cross-sectional survey was to determine the prevalence of early childhood caries (ECC) in a pediatric sample randomly chosen in public kindergartens or the 19th district in Rome, according to the WHO guidelines and to access the dental and orthodontic status of the affected children. METHODS: A total of 546 children aged 3-to 5-years were examined. Clinical examination had been executed from January 1999 to June 2000 by 4 calibrated operators; WHO clinical assessment form and anamnestic questionnaires for parents were performed. RESULTS: Children affected by ECC with a dmft score of 6.3 were 8.2% and 93% were ''treatment need'' (TN); low levels of oral health was significantly related with father's low socioeconomical status, both occupational and educational: most of these families were from less industrialized countries; 40% of these kids used to suck sucking-bottles with honey: many others used to drink too much sweet things (milk and sugar, fruit juice etc.); the most frequent orthodontic features in the ECC group were: brachifacial (26.7%), convex profile (70%), 3rd class terminal plane (30%). CONCLUSIONS: The results obtained point out that information, health education and health promotion are lacking throughout this part of roman population. Primary preventive dentistry programs should be therefore organized targeting parents, nursery schools and kindergartens' operators and pediatricians.

[Periodontal disease in children].

The authors analyze the most common periodontal diseases in pediatric age. They demonstrate that the control of the bacterial plaque is the determinant element to resolve these diseases.

[Adverse effects of wrong brushing technic].

In the present study brushing lesions have been considered clinically. Toothbrushing type, frequency, and lasting of brushing seem to be not involved in the formation of such lesions, directly connected with brushing technique and pressure.

[Cholestatic hepatopathy in a child with congenital hypopituitarism].

The present case report concerns a male infant affected by congenital hypopituitarism presenting with neonatal hypoglycaemic attacks, microphallus and bilateral cryptorchidism. A transient cholestasis observed during the 2nd month of life and reversed by hydrocortisone treatment, is to be considered, in Authors' opinion, in the context of the endocrine syndrome, probably as consequence of the adrenal failure.

[A new device for the disinfection of handpieces and turbines].

Dental handpieces are often difficult to disinfect. This is one of the main reasons for the considerable risk of cross-infections in dental offices. The aim of the present study was the evaluation of the disinfectant property of a recent, commercially available, automatic instrument, described as capable to clean, disinfect and lubricate dental handpieces. The following experimental evaluations were made: 1) antimicrobial activity of the disinfectant (glyoxalaldehyde) used. The method described by the European Committee for Standardization was followed. Test microorganisms were Pseudomonas aeruginosa and Staphylococcus aureus. 2) disinfection of dental handpieces (69 contra-angles and 97 turbines of different marks). They were naturally infected using them on patients for 30 minutes at least. 3) disinfection of dental handpieces infected with bacterial suspensions of Staphylococcus aureus, Streptococcus pyogenes (beta-haemolyticus, group A), Candida albicans and Pseudomonas aeruginosa. The results of the first experiment showed a strong bactericidal power of the disinfectant with both the tested strains, after a contact time of only 1 minute. A great proportion of the dental handpieces tested during the second experiment were found disinfected: from 84% through 89% out of the various models of turbine handpieces; from 89% through 100% out of the models of contra-angle handpieces. Even though bacterial contamination level was low (about 10(3) microorganisms per handpiece), a satisfactory disinfectant ability in natural conditions was found. The results of the third experiment were unclear. The tested instrument reduced 10(5)-10(8) times the original bacterial count when the gram positive microorganisms (Staphylococcus aureus, Streptococcus pyogenes) were used. On the other hand, when Pseudomonas aeruginosa and Candida albicans were used, the results were different: the bacterial count was reduced 10(6)-10(7) times in some cases, and only 10(2) times in other cases. This difference was found in the tests made using the same attachment and in those made using various attachments. In conclusion, the tested instrument showed, in most cases, a good disinfectant property, but the presence of unclear results suggests that some technical modifications are required.

[Tests on ionic release from glass-ionomer cements].

BACKGROUND: Dissolution process in oral liquids by the presence of glass-ionomer systems (due to surface corrosion, to diffusion through solutions and through mass) make an ionic release (particularly F, Al, Pb, As) which is a non secondary problem, due to the usual utilization of these materials in pedodontic and restorative dentistry. METHODS: In this work, considering the high toxicity of low quantity of Arsenic ion, a comparative research has been made in order to determine, by using high level liquid Cromatography (HPCL), the quantity in ppm of As hydro- and acid soluble given by five ionomeric products, in water and in nitric acid concentrated solution. RESULTS AND CONCLUSIONS: The results show that in some products arsenical concentrations are higher then the quantity accepted by ISO-FDI; therefore, a better control in the production of these products is needed as well as a limited use in dentistry. It is suggested to use glass-ionomer systems in patients with dental dike and varnish on the surfaces that are in contact with oral liquids action.
