Cognitive deficits in schizophrenia on the WAIS-R NI Sentence Arrangement Subtest. Wechsler Adult Intelligence Scale-Revised Neuropsychological Inventory.

Performance of participants diagnosed with schizophrenia on the Sentence Arrangement subtest of the WAIS-R NI and several tests sensitive to frontal lobe dysfunction was significantly poorer than that of manic depressive or control participants. Several measures of performance of patients diagnosed with schizophrenia on the WAIS-R NI Sentence Arrangement subtest appeared to support recent interpretations of the cognitive deficit seen in schizophrenia. These data represent the first demonstration of deficit performance by patients with schizophrenia on the Sentence Arrangement subtest. This is also supportive of the prediction that one of the areas whose activity may influence scores on this subtest is the prefrontal cortex. In addition, neither positive nor negative symptoms systematically correlated with the cognitive deficits reported despite specific predictions from the current literature.

Infarct volume, neurological severity and PAF binding to platelets of patients with acute cerebral ischemic stroke.

Studies show that Platelet Activating Factor (PAF) is involved in the cerebrovascular response to ischemia, and that its binding to platelets may change in stroke victims. The purpose of this study was to determine whether binding of PAF to platelets of stroke patients could serve as an index for determining the volume of ischemic strokes and severity of neurological presentation. Thirteen stroke patients and 21 healthy controls were studied. The neurological severity of these stroke patients was evaluated by the Scandinavian Stroke Scale. Infarct volume was assessed by planimetric measures of brain CT. PAF binding to platelets was determined by use of radiolabelled PAF. (3H)PAF binding to platelets of stroke patients was lower than in controls (149.58 +/- 46.11 and 212.1 +/- 10.3 receptors cell-1, respectively, p < 0.001) and was significantly correlated with infarct volume (r = -0.606, p = 0.014) and with neurological score (r = 0.527, p = 0.032). No correlation was observed between neurological score and infarct volume. The study confirms the involvement of PAF in the pathogenesis of brain ischemia and neuronal damage. It shows that PAF binding to platelets of stroke patients correlates both with the extent of neuronal damage and the associated neurological impairment, and may serve as an additional index in the assessment of stroke severity and clinical outcome of stroke victims.

MMPI-2 schizophrenia spectrum profiles among schizotypal college students and college students who seek psychological treatment.

The MMPI-2 schizophrenia spectrum profiles of 25 college students with extreme scores on the Perceptual Aberration and Magical Ideation Scales were compared to those of 27 students who requested psychological treatment at a university psychology clinic. Moldin, Gottesman, and Erlenmeyer-Kimling's 1987 classification strategy for schizophrenia spectrum disorders identified 66% of the students who did not seek psychological treatment and had high scores on the Perceptual Aberration and Magical Ideation Scales as having an 8-6, 8-9, or a 9-8 MMPI-2 profile. Of the students who requested psychological treatment, 25% produced MMPI-2 schizophrenia spectrum profiles. Of these, 63% produced a 2-7-8 code profile. Additional analyses showed that only some of the students who requested psychological treatment and produced a 2-7-8 MMPI-2 profile exhibited schizotypal features and that this group stayed in therapy longer than students without schizophrenia spectrum profiles. These results suggest that only a subset of the students with high scores on the Perceptual Aberration and Magical Ideation Scales produce schizophrenia spectrum MMPI-2 profiles and that these profiles are substantially different from those produced by students with high scores on the Revised Social Anhedonia Scale and from schizotypal college students who seek psychological treatment.

Alexithymia and suicidality in panic disorder.

To evaluate the prevalence of suicidal behavior in patients with panic disorder (PD) and to study the role of alexithymia (AL), an affect component, as a predictor of suicidal behavior in PD, we compared 42 patients with PD with or without agoraphobia with 24 healthy controls with regards to depression, AL and suicide risk. Only 5% of the PD patients reported previous suicide attempts. A higher frequency of positive AL (score > 73) was found among the PD patients (39% v 4% among the controls). PD patients had a higher suicide risk and AL as compared to controls, but only the increased suicide risk reached statistical significance. AL subjects had higher suicide risk scores as compared to non-AL subjects. Significant correlations were found between the AL score and suicide risk, although the most significant correlation was, as expected, between the depression level and the suicide risk. A low rate of previous suicide attempts was found in the PD group, perhaps reflecting the low comorbidity in our sample. We suggest that AL may have a role in the causation of suicidal behavior in PD patients, although further studies should re-examine this issue with larger samples.
