Diminished T lymphocyte proliferative response to polyclonal mitogens in acute brucellosis patients.

T lymphocytes from 21 untreated patients with acute brucellosis were tested for their proliferative response to polyclonal mitogens. The purified T lymphocytes from these patients showed a defective proliferative response to plant lectins and anti CD3 monoclonal antibodies with respect to the response observed in T lymphocytes from 21 healthy controls (p < 0.05). This defective proliferative response was not corrected by the exogenous addition of interleukin-2 or tumor necrosis factors alpha or beta to the culture medium. After antibiotic therapy, the proliferative response to the mitogens in T lymphocytes was found to be similar to that of the healthy controls (p > 0.05), and significantly higher than that found before treatment (p < 0.05). It was concluded that T lymphocytes from acute brucellosis patients have a defective proliferative response to membrane mitogenic signals, which disappears when the patients are cured after antibiotic treatment.

Uterine lipoleiomyomas: US and CT findings.

Fatty tumors of the uterus are rare. The reported incidence differs, from 0.03% to 0.2%. Two new cases of uterine lipoleiomyoma are presented with ultrasound, computed tomography, and pathologic correlation. To date, only seven cases have been reported in the radiologic literature.

Modulation of PECAM-1 (CD31) expression in human endothelial cells: effect of IFNgamma and IL-10.

Intercellular contacts formed between endothelial cells (EC) permit the formation of a confluent monolayer and play a major role in the recruitment and the migration of leukocytes in the inflammatory response. It is currently accepted that cytokines are responsible for the signals involved in the induction of such endothelial alterations. The platelet EC adhesion molecule (PECAM-1), a specific component of EC junctions, is one of many molecules which participate in the regulation of EC interaction with blood components. Given that the regulatory mechanisms which affect the expression of this adhesion molecule are only partially understood, the aim of the present study was to compare the effects of two antagonistic inflammatory cytokines, interferon (IFN)-gamma and interleukin (IL)-10, on the expression of PECAM-1. Human umbilical vein EC grown to subconfluence were stimulated with IL-10 (10 ng/ml) and/or IFNgamma (100 U/ml) for 24 h. PECAM-1 expression was determined by FACScan and immunofluorescence. Morphological analysis of the cell cultures was performed by optical and scanning electron microscopy. In the presence of IL-10, no changes in cell growth and morphology or in the intensity of PECAM-1 expression were observed. However, when the cultures were treated with IFNgamma, the EC acquired a fibroblast-like appearance, growth was disorganized and PECAM-1 disappeared from cell junctions. The mean intensity expression and the percentage of EC expression of this antigen were analyzed by flow cytometry and significantly decreased after culture in the presence of IFNgamma. The simultaneous addition of IFNgamma and IL-10 to the EC cultures induced modifications similar to those observed in the presence of IFNgamma alone. Regulation of the expression of PECAM-1 with the subsequent functional implications seems to be dependent upon the IFNgamma signal and it is unaffected by IL-10. The different effects shown by IL-10 and IFNgamma on the expression of PECAM-1 in EC could reflect opposite regulatory actions of the inflammatory response induced by these cytokines.

[Cystic hemangioblastoma of the spinal bulb].

INTRODUCTION: Hemangioblastomas are histologically benign tumors that comprise 8%-12% of all posterior fossa lesions in the adult. Brain stem hemangioblastomas may be solid or cystic tumors, and surgical removal of these tumors is usually possible and is the optimal treatment. We describe a case of hemangioblastoma in the medulla oblongata diagnosed with MRI. Using microsurgery technic a total excision of this intramedullary tumor was performed. CLINICAL CASE: A 37-year-old woman with a 3-months history of progressive dysphagia and occipital headache. Neurological examination at the time of admission revealed bilateral horizontal nystagmus, paresis of cranial nerves IX, X and XII, motor weakness of the four extremities. A MRI revealed a cystic intramedullary tumor. Using a suboccipital craniectomy the tumor was totally removed. CONCLUSIONS: Cystic hemangioblastomas of the medulla oblongata represent a small segment in the spectrum of brain stem tumors. Surgical removal of these tumors can be accomplished successfully with a low risk of neurological injury.

[Oropharyngeal morphology and food habits of Micropogonias furnieri (Pisces: Sciaenidae) in the North coast of Estado Sucre, Venezuela].

Morphology of the mandibular and pharyngeal region, and the feeding habits of Micropogonias furnieri were examined in 256 male and female specimens, between 28.7 and 54.3 cm total length, collected around Morro Puerto Santo, in northeast Sucre State, Venezuela (10 degrees 45'00" N-63 degrees 8'0" W), from May 1989 to April 1990. Micropogonias furnieri has a small ventral mouth; protrusible premaxillary and dentary; first branchial arches with 21 to 27 gill rakers, generally 24 to 25; and 7 to 11 pyloric caecae. The index of vacuity was low, with mean value of 7.00%. The mean intestinal index was 0.72, indicative of carnivorous species. Analysis of frequency of occurrence, indicates preference for crustaceans (45.70%), mainly crabs (34.90%), followed by polychaetes (28.00%), fishes (11.41%) and occasionally mollusks and echinoderms (4.0 and 3.9%, respectively). The diet of this species did not vary with sex.

[Cervical infection epidemiology of human papillomavirus in Ushuaia, Argentina].

Genital infection with human papillomavirus (HPV) is decisive in the causation of cervical cancer. In order to evaluate the epidemiology of HPV infection in Ushuaia, Province of Tierra del Fuego, Argentina, 132 endocervical cytobrushes from preneoplastic and neoplastic cases and controls were studied. Detection and typing of the viral genome was performed by polymerase chain reaction, combined with a restriction fragment length polymorphism assay or hybridization. The overall prevalence of HPV infection was 41% in the population examined, with a frequency of 26% in the controls and 71% in the cases under study. The 14-24 age group showed the highest HPV prevalence. The most common viral types in the infected population were HPV 16 (23%), HPV 18 (11%), HPV 33 (8%) and HPV 35 (8%), while high risk viral types were detected in 30% of the samples, 16% of the controls and 60% of the cases. This study provides the first data on the predominant viral types in Ushuaia. Our results show lower levels of infection than in regions with a high incidence of cervical cancer, HPV 16 being the most prevalent viral type. This research may be useful for selecting a specific vaccine targeting the population examined.

Cyclosporine A shows immunosuppressor activity on T lymphocytes from the peripheral blood and synovial fluid of patients with autoimmune arthritis.

OBJECTIVE: This work studies the effects of Cyclosporine A (CsA) upon the activation and proliferation of mononuclear cells (MNC) from the peripheral blood (PB) of patients with chronic autoimmune arthritis and from healthy controls, and from the synovial fluid (SF) of patients. METHODS: In vitro studies of activation, proliferation, mRNA expression and lymphokine production were carried out. RESULTS: We found in the PB and SF MNCs from patients with autoimmune arthritis that CsA inhibits the proliferative response, activation antigen expression, IL-2 mRNA expression and IL-2 production induced by polyclonal mitogens in a dose dependent manner. CONCLUSION: CsA blocks lymphocyte activation in PB and SF MNCs from patients with autoimmune arthritis.

Recurrent fixed drug eruption caused by citiolone.

Citiolone (N-acetylhomocysteinethiolactone) is a thiolic-derived medication frequently used in Spain and in other countries as a mucolytic agent for the treatment of certain hepatic disorders. Mucolytic drugs have rarely been implicated in the fixed drug eruption etiology. We report on a patient who presented several episodes of fixed exanthema related to citiolone intake. The patch test with citiolone (10% in dimethyl sulfoxide) was negative. The diagnosis was confirmed by a positive controlled oral challenge test. Other mucolytic thiolic-derivatives (N-acetylcysteine) were tolerated by the patient, thus crossreactivity between these drugs seems to be unlikely.

Acute IgE-mediated generalized urticaria-angioedema after topical application of povidone-iodine.

Povidone is a synthetic polymer mainly used as a dispersing or suspending agent for many pharmaceutical products. It is also used as a carrier for iodine. In many countries it is not mandatory its inclusion in drug labels so many adverse effects may be under diagnosed. Povidone-iodine used as a topical antiseptic solution may produce allergic contact dermatitis and irritation. Povidone, when systemically administered may deposit in tissues causing local lesions and pain. Three cases of immediate hypersensitivity to this agent have been reported. We present a patient who developed generalized urticaria and angioedema immediately after the first topical use of Betadine (povidone-iodine) on a right arm wound. The positive skin prick test to Betadine and povidone extract and the demonstration of serum specific IgE to povidone, confirmed an IgE-mediated hypersensitivity.
