[Study of myocardial perfusion at rest using technetium Tc 99m methoxy-isobutylisonitrile: comparison with labeled microspheres and thallium 201].

In order to evaluate the potential usefulness of 99m technetium (Tc)-methoxy-isobutyl-isonitrile (MIBI) as a myocardial perfusion tracer in man, its myocardial distribution at rest was compared with that of 201thallium (TI). Perfusion images obtained with 99mTc-labelled microspheres, whose myocardial distribution is exclusively coronary flow dependent, were used as reference. The study was performed on twelve patients (10 males and 2 females, mean age 55.4 +/- 6.4) with suspected effort angina and without a history of previous myocardial infarction. In the space of two weeks patients underwent rest and exercise 99mTc-MIBI myocardial scintigraphy, exercise-redistribution 201TI myocardial scintigraphy, 99mTc-labelled microsphere scintigraphy after microsphere injection in the left ventricle at rest during catheterization before performing left ventriculography and coronary angiography. The comparison with microsphere distribution was limited to rest 99mTc-MIBI scintigraphy and redistribution 201TI images. Single photon emission tomography (SPECT) was employed. SPECT was performed using a double-head Rotacamera with a 360 degrees rotation arc. Ninety projections of 20 seconds each were acquired; subsequently image reconstruction was performed using an iterative algorithm. To evaluate regional perfusion the left ventricular wall was divided into 18 segments; the uptake pattern of each segment was graduated according to a qualitative score (0: severe defect; 1: moderate defect; 2: normal uptake). The quality of tomographic myocardial 99mTc-MIBI and 99mTc-microspheres images was higher than that of 201TI tomograms in account of the different physical characteristics of the two radionuclides.(ABSTRACT TRUNCATED AT 250 WORDS)

Usefulness of the TRH test in the management of patients with differentiated thyroid cancer.

Thirty patients thyroidectomized for differentiated thyroid cancer were studied. Serum TSH was assayed in basal conditions and after TRH stimulation, while patients were in suppressive therapy with thyroid hormones. The basal TSH was normal in all the patients and less than 2 microU/ml in 20 patients. The TRH test was negative (no TSH response) in 27 patients and in all the cases with the basal TSH lower than 2 microU/ml.

Staging breast cancer--screening for occult metastases.

The authors report on 1,017 consecutive breast cancer cases without symptomatic metastases staged by means of chest X-ray (CXR), skeletal survey (BXR) and bone scintigraphy (BS). Occult metastases (DM) detection rate was 0.88%: 0.29% for lung and 0.59% for bone DM. The detection rate was correlated with clinical stage: 0.36% for stage I, 0.20% for stage II, 0.26% for stages I and II, and 2.77% for stage III cases. The sensitivity based on DM cases prevalent or surfacing within 6 months of follow-up was 0.30 for CXR, 0.22 for BXR and 0.55 for BS; specificity was 0.99, 0.98 and 0.90, respectively. The study confirms the possibility of early detection of DM with preoperative staging, but the extremely low detection rates in stage I and II cancers do not advise such a routine procedure. The higher detection rate of DM may suggest adoption of the routine staging procedure in stage III cancers. In these cases, although no evidence is available of a favorable prognostic impact of early detection and treatment of DM, an unnecessary mastectomy could be avoided in about 3% of cases in the presence of DM detected by the staging procedure.

Papillary carcinoma of the thyroid. A clinicopathologic study of 241 cases treated at the University of Florence, Italy.

A clinicopathologic study of 241 cases of papillary thyroid carcinoma treated at the University of Florence Medical School, Florence, Italy is presented. The features of greatest prognostic value were patient's age at presentation, small tumor size, total encapsulation, extrathyroid extension, multicentricity, and presence of distant metastases. The prognosis of the disease was not influenced by the pattern of tumor growth, presence of solid areas, initial presence or subsequent development of cervical lymph node metastases, type of initial thyroid operation, performance of neck dissection, or prophylactic administration of radioactive iodine. It is concluded that conservative thyroid surgery in the form of lobectomy, without neck dissection or prophylactic administration of iodine 131 (131I), constitutes adequate therapy for most cases of papillary thyroid carcinoma. More extensive therapy should be considered for older patients and for those in whom the tumor exhibits extrathyroid extension or easily detectable multicentricity.

Prognostic value of the 131I whole-body scan in postsurgical therapy for differentiated thyroid cancer.

Seventy-two patients affected by differentiated thyroid cancer underwent whole-body scan seven days after the postsurgical thyroablative treatment with 131I. In 40 patients this scanning did not reveal any area of 131I uptake outside the residual thyroid parenchyma. During the follow-up period, no signs of functioning tumors were detected in these patients and therefore, there was no need for further therapeutic treatment with radioiodine. From this results it is legitimate to conclude that whole-body scan control can be significantly postponed without diagnostic inaccuracy for those patients whose postthyroablative scans do not reveal diffuse tumor localizations.

Plasma dehydroepiandrosterone sulphate in hypothyroid premenopausal women.

Plasma dehydroepiandrosterone sulphate (DHEAS), cortisol (F) and prolactin (PRL) were measured in seventeen premenopausal women with primary hypothyroidism and in fifteen normal premenopausal women. Significantly lower (P < 0.001) DHEAS levels were found in hypothyroid women while F and PRL were in the range of normal controls. No significant relation was found between DHEAS and total thyroxine (T4) and TSH. Our results support the hypothesis that DHEAS secretion is impaired in some women with primary hypothyroidism.

[Sequential scintiscanning with 99mTc-HIDA in the study of some aspects of post-operative dynamics of the proximal digestive tract].

The authors exploited the elective biliary excretion of 99mTc-HIDA at adequate concentrations to afford visibility of intestinal segments more directly wetted by bile in a study of sequential hepatobiliary scintigraphy designed to explore the parts of the duodenum and jejunum that are excluded to the transit of gastrointestinal contents in many surgical procedures involving the proximal digestive tract. They illustrate their case material with several representative examples; these are described in detail to show the validity, usefulness and versatility of the scintigraphic technic under discussion.

The role of whole body scan and skeletal radiography in the preoperative staging of breast cancer.

442 patients with breast cancer were studied by means of radiologic and scintigraphic examination of the skeleton to detect the presence of bone metastases. All the patients have been clinically followed-up for a mean period of 22 months. The overall prevalence of bone metastases at staging was 4.5%. Bone scan showed a higher sensitivity in detecting bone metastases (90% vs 45%) and a lower positive predictive value (60% vs 75%) with respect to X-ray examination. In the presence series no asymptomatic case with bone metastases was found in limited stages (T1-T2 N0-N1). The authors suggest that the bone scan be routinely included in the staging of advanced breast cancer cases and that the use of this procedure be restricted to the symptomatic patients in the more limited stages.

[MRI and HM-PAO SPECT in 20 patients with drug-resistant partial epilepsy].

Twenty consecutive patients with chronic partial seizures with onset before twenty years of age were investigated by means of 0.5 T MRI (20) and HM-PAO (19) in order to identify focal alterations amenable to surgical therapy. MRI evidentiated parenchymal lesions in 7 patients. Findings consistent with unilateral medial temporal sclerosis and cortico-subcortical parietal scars were found in two patients each. Small solid nodular lesions in the temporal lobe were observed in two patients. These and one additional patient with nodular partially cystic lesions in temporal lobe were administered a paramagnetic contrast agent (Gadolinium DPA) intravenously. In one case a contrast enhancement was observed. Histologic examination post surgery revealed a low grade glioma in one patient. HM-PAO SPECT examination showed area of abnormal captation in 9 of 19 patients. Aspects of EEG correlation with the MRI and SPECT findings are discussed. Our data supported the usefulness of magnetic resonance and SPECT imaging in the completion of pre-surgical assessment in this kind of patients.
