[CT-guided punctures: diagnostic value, therapeutic consequences and economic aspects].

PURPOSE: To evaluate the diagnostic and clinical relevance and therapeutic consequences of CT-guided biopsy with regard to economic aspects. METHODS: 213 CT-guided biopsies in 190 patients were evaluated. All information regarding patient referral, reason for request, body region, underlying diagnosis, and clinical consequences were registered over a period of 22.2 +/- 9.4 month. RESULTS: Patient referral to biopsy was mainly from the departments of surgery, internal medicine, and radiotherapy with the question of tumor and metastasis. Less than 5% of biopsies were performed in outpatients. Main regions were the lung (39%), the abdomen (35%), and the skeleton (11%). Biopsy and surgical histology corresponded in 73%. Largest diagnosis groups were benign unspecific tissues or other benign lesions in 24%. As a result of CTP no further procedures were necessary in 22.5%. Follow-up studies or conservative treatment were indicated in 11.3%. Surgical procedures were needed in only 15.5%. There was only one complication requiring therapy. CONCLUSION: CT-guided biopsy is a safe procedure, which helps to avoid unneccessary cost-intensive diagnostics and surgical treatment. If CTP is performed early and in outpatients residence time in the hospital is reduced and thus money is saved.

[Lung metastasis after pelviscopic salpingectomy in therapy of tubal pregnancy].

UNLABELLED: A 34-year old VI gravida, II para underwent a pelviscopic salpingectomy of the left fallopian tube for treatment of a repeated ectopic pregnancy. Histology revealed an invasive hydatidiform mole. The declining beta-hCG titers after surgery were not further controlled. Seven months later a secondary amenorrhoea occurred again; beta-hCG levels increased steadily. We could not obtain trophoblastic tissue by operative pelvis-copy (excision of the tubal stump on the left side and tubal sterilisation on the right side) nor by curettage of the uterine cave. A lesion of the lung was interpreted as a metastatic lesion following malignant transformation of the invasive hydatidiform mole and disappeared under cytotoxic therapy containing methotrexate with an uneventful follow-up period of now 4 years. RECOMMENDATION: In order not to overlook the presence of persistent trophoblastic tissue and the possible development of a choriocarcinoma, beta-hCG serum titers should be measured until they disappear, following the surgical therapy of an ectopic pregnancy.

[Cystic space occupying lesion of the upper pole of the kidney in a newborn infant. Case report and differential diagnosis].

In a newborn female infant a cystic lesion in the upper pole of the right kidney was detected by sonography 4 weeks before an uneventful delivery. In the postnatal period the sonographic examination, IVP, CT, and the biochemical data as well were not able to classify definitely this lesion as a benign cyst, adrenal hemorrhage or cystic malignoma. Therefore a laparatomy was performed, which revealed a cystic neuroblastoma.

[Practical application of digital therapy verification].

The following describes the technical details which are of importance preparing portal radiographs using digital luminescence radiography. There are some advantages to this method: In comparison with the conventional portal radiographs using a low speed film, the new doses is ten to thirty times less than generally required. Double exposure techniques can be more easily applied than with conventional portal films. The contrast may be subsequently manipulated. In addition the dynamic range of the radiation detector is automatically adjusted to the exposure range. These facts may improve the verification quality. However, there are also some disadvantages: You need more apparatus, and the whole procedure is more time-consuming. Using typical examples we shall compare the procedure of digital portal radiographs and its results to traditional techniques.

[Comparative contrast media studies using conventional film-screen systems and digital luminescence radiography].

This is a report on the clinical application of digital luminescence radiographs compared to conventional x-rays during examinations with contrast medium. We present selected examinations of the period October to December 1988. By a constant quality of hard copies the digital examinations are equal to conventional x-rays. Regions of little difference in density are better represented by digital luminescence radiographs.

The field integrated dose modification (FIDM): three typical clinical applications of a new irradiation technique.

In addition to an earlier developed "compensator integrated modification technique", a method is derived which uses as basic principle the "field integrated dose modification" (FIDM) without need for computer tomographic data. The method enables the radiotherapist to introduce one or more volumes within which doses can be delivered which are different from the one in the main irradiation volume. The steps of practical preparation and clinical application are described in detail. The radiobiological aspects of FIDM, especially those in case of acceleration and hyperfractionation, are elucidated by discussing three typical examples.

[Optimization of combined brachy-teletherapy by the modification technic].

A method is presented for combined brachy-teletherapy by which the dose distribution of percutaneous irradiation is adapted to the distribution of short-distance irradiation with 192iridium by means of the so-called "field integrated dose modification". Insufficient or excessive doses within the area of superposition of both therapy modalities can so be avoided. Moreover, the therapist can define zones of deviation from the reference dose, if such a dose heterogeneity seems desirable in the individual case. The combined irradiation of prostata and cervix is given as an example to describe in detail the technical, clinical, and radiobiological aspects of this method.

[Craniometaphyseal dysplasia--characteristic roentgen findings].

Craniometaphyseal dysplasia is a rare disorder of bone remodeling, which is characterized by hyperostosis or sclerosis of the skull combined with metaphyseal flaring of the long tubular bones. Diagnosis is only possible on the basis of the characteristic radiographic findings, these are shown in a case report of a 2 1/2 year old boy. There is no therapy of craniometaphyseal dysplasia.
