[Primarily malignant tumors of the renal pelvis and the ureter - clinical manifestations and results of radiotherapy].

Twenty-two own cases are presented in order to describe the clinical manifestations and therapy, especially the postoperative radiotherapy, in case of carcinomas of the upper urinary tract. The authors agree with literature to recommend a high voltage radiotherapy with 45 Gy to the regional lymph node groups and with 50 to 55 Gy to the former tumor region in case of all tumors with infiltrating growth, of demonstrated lymph node metastases and of subtotally removed primary tumors. Radiotherapy should become an essential part of the therapeutic conception for carcinomas of the renal pelvis and of the ureter because of the nearly complete absence of side effects and the small rate of local recurrences.

[Clinical aspects and radiotherapy of stage III and IV neuroblastomas].

Contrary to other tumors in children, e.g. leukemia and Wilms tumor the prognosis of which has been considerably improved by the application of surgery, radiotherapy, and chemotherapy, the treatment results of one of the most frequent tumors, namely the neuroblastoma, have not essentially ameliorated during the last few years. The authors present 28 own case records of irradiated patients and describe the clinical symptoms, diagnosis and therapy, especially radiotherapy of the neuroblastoma. A prolongation of the survival time by several months could be achieved in a small group of eight patients treated additionally with an aggressive chemotherapy. The overall long-term prognosis, however, could not be improved hitherto.

Abdominal Neoplasms↗

[Computer tomography of retroperitoneal trauma (author's transl)].

Computer tomography changes after retroperitoneal trauma are described on the basis of 19 patients seen by the authors. It appears that this method is superior to conventional techniques, both as a screening method and for carrying out follow-ups. The indications for angiography are thereby reduced.

[Computer tomographic diagnosis of Caroli's syndrome (author's transl)].

Congenital cystic dilatation of the intrahepatic bile ducts--Caroli's syndrome--is a rare disease of which about one hundred cases have been reported in the literature. Based on two observations, the value of various radiological procedures is discussed. Compared with other noninvasive procedures, computer tomography has the advantage of being able to show not only the changes in the liver, but also the changes in the liver, but also the medullary sponge kidney which often accompanies this condition. The indications for ERC can therefore be more strictly defined.

[CT diagnosis of recurrent subluxation of the shoulder (author's transl)].

CT provides a clear, unobstructed and reproducible view of the shoulder joint. This permits, unlike conventional methods, for the first time a complete view of the anatomy and of the factors which predispose to a recurrent subluxation of the shoulder. In addition it is possible to see bone lesions resulting from the subluxation.

Chronic Disease↗

[Clinical symptoms and high voltage therapy (electron therapy) of the vulvar carcinoma].

A report is given about the results of radiotherapy in 139 patients with vulvar carcinomas treated at the Radiologic University Hospital in Münster. This group of patients had undergone a very heterogenic preliminary treatment. Radiotherapy carried out mainly with 18 MeV electrons brought about a five-year survival rate of 58%. Due to the favorable action of the split course method, the incidence of side effects was very low regarding the total focal dose of 60 Gy (27% exudative reactions, 11% superficial ulcerations). Taking into account the unsurmountable difficulties caused by the high age of the patients and the numerous accompanying risk factors, the treatment results achieved during the last few years can probably only be gradually improved. After an operation performed as radically as possible, radiotherapy should aim at a low recurrence rate.

[Importance of computed tomography in renal lesions (author's transl)].

The value of computed tomography in the diagnosis of renal traumas is discussed for the individual stages of injury on the basis of 14 patients. it is evident that computed tomography is of great importance for selecting the proper therapeutic procedure. Stage I injuries usually do not require any further clarification if examined by CT. In cases of stage II injuries, short-term CT check up is necessary. Stage III injuries only will continue to require examination by angiography.

Extravasation of Diagnostic and Therapeutic Materi↗

[Xeroangiographic findings in patients with haemodialysis fistulas in the arm (author's transl)].

The authors report on a simple and accurate method for xeroangiographic visualisation of haemodialysis shunts in the arm which has been performed so far in 98 patients without complications. By this method, approx. 45 ml metrizamide (80 mg iodine/ml) are injected into the venous limb during short-term hypersystolic congestion. Single xeroradiography performed subsequently shows both the arterial and the venous vessels. The pathological findings established during the examinations (stenoses, pseudoaneurysms, venous thromboses) were confirmed by subsequent surgery.

[CT morphology of biliary neoplasias (author's transl)].

In a retrospective study the Ct morphology of cholecystocarcinomas, cholangiocarcinomas and papillary carcinomas is discussed on the basis of 36 patients. Particular attention is given to the differential diagnostic criteria.

[Post-traumatic pulmonary hematomas and pneumatoceles].

The x-ray morphology of posttraumatic intrapulmonary haematomas and pneumatoceles is described on the basis of 12 cases in a total of 211 patients with blunt thoracic traumas. The article also reports on the course of these haematomas and pneumatoceles. The problem of the development of a barotrauma through PEEP ventilation is discussed, taking the clinical courses into consideration. A characteristic feature of all the discussed cases was the tendency to spontaneous regression as well as continuous decrease in size roentgenologically; this enabled safe differential diagnosis against other cystic pulmonary changes.

[Radiological diagnosis and differential diagnosis of the choledochocele (author's transl)].

The article demonstrates the radiological morphology of choledochocele, a rare form of cystic dilatation on the common bile duct, on the basis of the author's own observations. The article also goes into the aspects of differential diagnosis. Besides the constant filling defect with smooth contours of the duodenum descendens, intravenous cholangiography visualizing the choledochocele filled with contrast medium, presents a radiologically typical image.

[Sonography in pericardial effusion (author's transl)].

The examination technique, results and diagnostic value of sonography were evaluated in a retrospective study of 70 patients with pericardial effusion. Small effusions of less than 100 cc can be detected by observing a separation of the epicardium from the pericardium. With larger effusions sonography allows an approximate estimate of the volume. Since ultrasonography is non-invasive, and since it has a very high diagnostic accuracy and only few false-negative findings, it is the method of choice for the examination of patients suspected of having a pericardial effusion.

[Sonography and endoscopic retrograde cholangio-pancreatography in the diagnosis of diseases of the pancreas and biliary system (author's transl)].

The value of sonography and ERCP was studied in a retrospective series of 128 patients with pancreatic and biliary disease. Sonography and ERCP employed singly provided a correct diagnosis in 68% and 75.5% respectively; combined diagnostic accuracy rose to 85%. The inclusion of other diagnostic information produced almost identical figures for sonography and ERCP of 82% and 86% respectively. Consequently, sonography must be recommended in the first place as a non-invasive method for the diagnosis of pancreatic and biliary disease. The indications for ERCP are for the elucidation of chronic pancreatitis without pseudo-cysts and for other pancreatic abnormalities not clearly shown by sonography, and also for the demonstration of the biliary system in non-obstructive jaundice.
